Introduction: Positional changes may contribute to progression of labor in nulliparous women. We investigated the use of non-invasive, non-pharmacological interventions during the first stage of labor in nulliparous, term, singleton, vertex (NTSV) women at a community hospital in the U.S. Another aim was to compare the times from the first intervention to completion of the first stage of labor across three interventions.

Methods: Ours was a descriptive, retrospective study of 98 NTSV women from January to June 2025. Descriptive and inferential statistics were analyzed using Statistical Package for Social Sciences version 27.

Understanding the initial signature of noise-induced auditory damage remains a significant priority. Animal models suggest the cochlear base is particularly vulnerable to noise, raising the possibility that early-stage noise exposure could be linked to basal cochlear dysfunction, even when thresholds at 0.25-8 kHz are normal. To investigate this in humans, we conducted a meta-analysis following a systematic review, examining the association between noise exposure and hearing in frequencies from 9 to 20 kHz as a marker for basal cochlear dysfunction. Introduction: Evidence supports a healthy diet for secondary stroke prevention; however, programs to implement nutrition education remain inconsistent. This study assessed the feasibility and efficacy of using a multisensory approach called the “Hands-on Training” program (HOT) to improve healthy diet adherence among patients rehabilitating after stroke.

Methods: This pilot randomized controlled study was conducted at an inpatient rehabilitation facility. Stroke patients were randomly assigned to either an experimental group who received HOT or a control group who received conventional education. Introduction: Fragmented community efforts often dilute health‐improvement initiatives. The Communities of Resilience and Excellence (C.O.R.E) Guide offers a Baldrige-based systems approach that aligns cross‐sector partners around shared metrics and continuous improvement cycles for collective impact. West Kendall, an unincorporated area of Miami-Dade County with over 340,000 residents, adopted the framework in response to rising chronic disease burden and socioeconomic disparities.

Introduction: According to the Nursing Council of State Boards of Nursing (NCSBN), more than half of all errors among new graduate registered nurses are related to a lack of clinical judgment, a critical component of nursing practice. Considering the Next Generation National Council Licensure Examination (NCLEX) focuses on clinical judgment, a greater understanding of faculty’s Next Generation NCLEX (NGN) teaching preparedness is needed. Introduction: Acute skin failure (ASF) in hospitalized patients is a growing concern and a product of multisystem organ failure. Unlike hospital-acquired pressure injuries (HAPIs), ASF is not preventable despite prevention measures. The Skin Failure Clinical Indicator Scale (SFCIS) was recently developed to assess the probability of ASF. The purpose of this study was to retrospectively examine the probability of and factors associated with ASF in hospitalized individuals with HAPIs.

Introduction: COVID-19 had a profound impact on cancer screening, diagnosis, and treatment worldwide. The purpose of this study was to examine the role that the COVID-19 pandemic had on delays in breast cancer screening, diagnosis, treatment, and other related variables, including cancer staging and oncology measures of performance (ECOG Performance Status), in the inpatient and ambulatory oncology settings.

Methods: This was a multi-site, interdisciplinary retrospective review study including 1,514 participants. Four cancer centers collaborated, submitting de-identified data in accordance with their policies and procedures.

Results: Our study's results showed significantly higher delays in screening mammogram appointments (p < .001) during the COVID-19 pandemic (N=142) than before the COVID-19 pandemic (N=56), with a 52% higher likelihood of canceling screening mammogram appointments during COVID-19. Our findings also revealed a statistically significant difference (p < .001) in the time from diagnosis to the first recommended treatment for scheduled initially appointments, with an increase of 18 days, suggesting delays in cancer treatment during the COVID-19 pandemic.

Introduction Internal medicine training has historically relied on classroom-based lectures, typically delivered through morning and noon conferences, which offer broad content coverage but limited experiential learning. Simulation-based education improves resident confidence, procedural ability, and clinical reasoning; yet it is often delivered in brief, standalone activities that lack longitudinal structure or integration within the broader curriculum. Few programs have a longitudinal simulation curriculum that spans the academic year and incorporates multiple modalities and topics. Introduction

Middle meningeal artery embolization (MMAE) has emerged as a promising new treatment for patients with chronic subdural hematomas (cSDH). Its efficacy, however, upon the subtype with a high rate of recurrence—septated cSDH—remains undetermined.

Methods

From our prospective registry of patients with cSDH treated with MMAE, we classified patients based on the presence or absence of septations. The primary outcome was the rate of recurrence of cSDH. Secondary outcomes included a reduction in cSDH thickness, midline shift, and rate of reoperation.

Results

Among 80 patients with 99 cSDHs, the median age was 68 years (IQR 59–77) with 20% females. Twenty-eight …

The Advisory Committee on Immunization Practices recommends human papillomavirus (HPV) vaccination for inadequately vaccinated adults aged 27–45 based on shared clinical decision-making (SCDM). However, little is known about awareness and barriers to SCDM among US healthcare providers (HCPs), especially in Texas, where HPV vaccination rates are below the national average. Between January and April 2021, we conducted a population-based cross-sectional survey (12% response rate) of HCPs licensed in Texas to assess the prevalence and factors associated with awareness of the SCDM recommendation. Prediabetes is one of the main health concerns in public health, and various etiological factors contribute to its onset. This study aimed to evaluate genetic associations and gene‐macronutrient interaction with prediabetes‐related metabolites to understand how genetic variation and dietary intake contribute to dysglycemia. We analyzed a total of 482 self‐identified Mexican American participants recruited from Starr County, Texas in 2018‐2019. Untargeted metabolomic profiling was performed using LC‐MS. Nutrient densities of six macronutrients were derived from a 106‐item food frequency questionnaire. Background: Previously considered a rare opportunistic pathogen, Serratia is now an emerging cause of bacteremia. We compared the risk factors and outcomes of patients with Serratia bacteremia relative to other Enterobacterales in a cohort of hospitalized patients.

Methods: Patients were prospectively enrolled from 2002-2021 at Duke University. Characteristics and outcomes were compared among bacteremic patients due to Serratia and other Enterobacterales. Regression models were used to determine features associated with outcomes. Whole genome sequencing (WGS) was performed to characterize the phylogenetic diversity of Serratia isolates associated with device infections.

Background: Over half of the people with dementia visit the emergency department (ED) each year. Little is known about how people with dementia and their care partners conceptualize or communicate about health outcome goals or care preferences during acute care episodes.

Participants and setting: We conducted interviews with 14 Hispanic people with dementia and 19 care partners recruited from a public safety net hospital system in the southeastern United States. Participants were community-dwelling and had recently visited the ED.

As artificial intelligence (AI) continues to evolve rapidly, governments worldwide face increasing pressure to regulate it. This Article focuses on regulatory approaches to AI in the United States, the European Union, and China, examining the underlying rationales: why have they taken these approaches? Each jurisdiction’s unique political institutions, historical preferences, and strategic priorities shape its approach to governance. The United States’ Countervailing Duties Law protects domestic markets by offsetting any benefit foreign manufacturers receive from their domestic governments’ subsidy programs. The benefit analysis the Department of Commerce uses under the Countervailing Duties Law, however, is too vague. Moreover, the sole environmental exemption for otherwise countervailable subsidies is too narrow. As a result, Commerce assesses duties on an array of manufacturers burdened by foreign governments’ climate policies. The case study BGH Edelstahl v. In 2025, it was estimated that over 35% of international fish stocks were overfished, risking severe depletion of high seas fisheries. In general, countries have control over fisheries within 200 nautical miles of their coastal border. Many countries have established fishery conservation laws to help prevent overfishing. Some examples of the largest domestic fishing laws are the Magnuson-Stevens Fishery Conservation and Management Act and the European Union’s Common Fishery Policy (MSA), which functions as an international policy that governs all of the United States’ fisheries and the European Union’s Common Fisheries Policy (CFP). This study addresses the critical need for self-powered, durable pressure sensors in Total Knee Replacement (TKR) implants to enable the collection of post-operative information. The scope of this research encompasses the design, development, and testing of a triboelectric nanogenerator (TENG) integrated into an instrumented knee implant for energy harvesting and pressure sensing. The authors’ unique approach involves utilizing a porous silicone rubber as a dielectric material. This allows the TENG to withstand forces up to 2000 N and generate a maximum power output of 18 μW. Theoretical modeling combined with experimental validation provides deeper insight into the fundamental operating principles. …

Parenting While Mad: Altered States, Carceral Psychiatry, And The Fight For Abolitionist Access, Stefanie Lyn Kaufman-Mthimkhulu

Journal of Law and Policy

Legal and psychiatric systems respond to parents in distress-oriented altered states (often labeled “postpartum psychosis”) in ways that reveal deep structural failures. Rather than providing genuine care, these states are criminalized, pathologized, and met with coercive treatment, family separation, and surveillance. Drawing on Silvia Federici’s Caliban and the Witch, this article traces these systems to the rise of colonial-capitalist-patriarchy, when land was enclosed, reproductive labor weaponized, and community healing knowledge expropriated. Clinical mental health and family regulation systems continue this legacy today, punishing Madness, disempowering caregivers, and undermining community capacity to respond to distress. In an increasingly interconnected world, cybersecurity professionals play a pivotal role in safeguarding organizations from cyber threats. To secure their cyberspace, organizations are forced to adopt a cybersecurity framework such as the NIST National Initiative for Cybersecurity Education Workforce Framework for Cybersecurity (NICE Framework). Therapeutic hypothermia (TH) has demonstrated neuroprotection in instances of cardiac arrest and neonatal hypoxia/ischemia but faces different challenges in application to stroke due to the activation of cold defense mechanisms in conscious patients. This dissertation examines the efficacy and specificity of capsinoids (a purified mixture of capsiate and dihydrocapsiate) to induce a sustained mild hypothermia in conscious mice that is neuroprotective after stroke. Capsinoids function as TRPV1 agonists. However, unlike capsaicin, capsinoids are vulnerable to esterase-mediated breakdown, thus significantly restricting their action to the site of delivery.
