Psoriasis is associated with an increased risk of major adverse cardiovascular events (MACE), and biologic therapy may reduce this risk by targeting systemic inflammation. However, limited data exist on whether long term MACE risk differs by biologic class or across racial groups. We conducted a retrospective cohort study to assess the risk of primary MACE in psoriasis patients receiving biological therapies, stratified by biological class, compared with biologic-naïve psoriasis patients. Whether MACE risk among biologic-treated patients varied by race was further examined. After 1:1 propensity score matching, 36 330 biologic-treated patients were compared with 36 330 non-biologic-treated patients. Incident MACE …

This study describes the development and initial validation of the Mentoring Engagement Scale (MES), a self-report instrument designed to measure a person’s level of engagement in professional mentoring. Despite emerging evidence that professional mentoring may serve as a protective factor against the burnout and stress associated with school counselors’ roles and responsibilities, existing measures of mentoring are limited by weak documentation of development procedures, mixed psychometric support, and limited practical applicability. Magnetic resonance guided radiation therapy (MRgRT) combines magnetic resonance imaging (MRI) and radiation therapy to enable the visualization of anatomy in real time during treatment. The presence of a magnetic field in MRgRT affects charged particle radiation and detector responses, necessitating changes to traditional reference dosimetry procedures. The American Association of Physicists in Medicine (AAPM) Task Group 351 (TG-351) provides a formalism for reference dosimetry in MRgRT. In this study, the TG 351 formalism is used to perform reference dosimetry for a 6 MV flattening filter-free (FFF) photon beam from a Viewray MRIdian MR-Linac radiation therapy system. This thesis qualifies and documents a new genre of interdisciplinary, interactive theatre called Dodgeballs and Dragons. It covers a brief history of Dungeons and Dragons and Theatre of the Oppressed (the two foundational works for the new genre) as well as previous uses of D&D as a form of theatrical entertainment. After establishing the precedence used to create this product, the rest of the thesis breaks down how to write, lead, and play these shows on a regular basis. The document culminates in collected data providing concrete evidence for the future of the form and its many applications.

The diagnostic classification of gender dysphoria in the DSM-5-TR elicits a number of clinical, political, and philosophical concerns, ranging from those that are diagnostic and taxonomical in nature to those that fall within the domains of ontology, phenomenology, and hermeneutics. Part of the reason for the complexity of studying the diagnostic and nosological validity of gender dysphoria as an extant diagnostic classification is that, unlike many disorders, gender dysphoria is inextricably tethered to identity. Persistent Sympathetic Activation (PSA) has emerged as a potential unifying mechanism underlying a range of complex chronic health conditions yet remains poorly characterized within biomedical and psychosocial research. Limited research has delineated how PSA manifests across diverse populations or how diagnostic and sociocultural factors shape lived experiences of this phenomenon; thus, the objective of the present study was to examine whether diagnostic certainty – operationalized as provider-confirmed versus self-suspected PSA-related diagnoses – is associated with symptom catastrophizing, and whether this relationship varies as a function of sociocultural factors. This qualitative study examines how highly skilled foreign-national parents navigate parenting and educational decision-making within the context of cultural transition in the United States. While existing research on foreign-national parenting has largely focused on socioeconomically disadvantaged populations, less is known about families who possess substantial educational and professional capital. Despite these advantages, these parents encounter unique challenges related to institutional navigation, identity negotiation, and social constraints. Background/objectives: Phenylketonuria (PKU) is an autosomal recessive inborn error of metabolism. If untreated, elevated blood phenylalanine (Phe) levels lead to neurological and behavioral impairments. Phe levels can be managed through a lifelong Phe-restricted diet; however, this can be challenging to maintain. Sepiapterin is an adjunct therapy for PKU that has shown efficacy in reducing blood Phe levels in both tetrahydrobiopterin (BH4)-responsive and BH4-non-responsive people with PKU in clinical trials. Practical guidance is needed for dietitians and other healthcare professionals supporting the dietary management of people with PKU who are initiating or receiving sepiapterin.

Gastrointestinal (GI) cancers remain among the leading causes of cancer-related mortality worldwide, with therapeutic resistance continuing to limit the effectiveness of current treatments. Three complementary studies were conducted to identify therapeutic vulnerabilities and evaluate novel combination strategies in pancreatic ductal adenocarcinoma (PDAC) and colorectal cancer (CRC). First, dual inhibition of fibroblast growth factor receptor 4 (FGFR4) and phosphoinositide 3-kinase (PI3K)/mammalian target of rapamycin (mTOR) signaling was investigated in PDAC models. A combination of targeted drug treatments reduced viability, clonogenic survival, migration, and suppressed signaling pathways involved in translational control. Background: Neurodevelopmental disorders (NDDs) in children present significant challenges for healthcare systems globally. In Tanzania, children with NDDs and their families face significant challenges due to late diagnosis, lack of standardized screening tools, widespread stigma and cultural beliefs that deter health-seeking behaviours. Awareness about NDDs is low, and financial constraints further complicate access to necessary medical interventions. The country suffers from a severe shortage of specialized healthcare providers, and general healthcare providers often lack adequate training. Attention to the trial of Luigi Mangione has renewed debate about nullification—the jury’s inviolate power to acquit a defendant contrary to the evidence presented. Jury nullification proponents hold that it is a mechanism of last resort for refusing to apply laws that conflict with the public conscience. Opponents, by contrast, argue that nullification undermines the law by diminishing its legitimacy. The anticipated demise of the independent agency is a saga that implicates Congress as much as it does the President or the Supreme Court. By capitulating to presidential initiatives and failing to defend their institutional prerogatives, Congress has facilitated presidential control of the administrative state. Indeed, irrespective of whether the Court formally repudiates Humphrey’s Executor, Congress has largely acquiesced to a regime whereby presidents essentially control independent agencies. Modern machine learning systems are increasingly deployed in streaming environments where data arrive sequentially and the underlying data-generating process may evolve over time. This phenomenon, known as concept drift, can significantly degrade model performance if not detected and addressed in a timely manner. This dissertation proposes a principled framework for concept drift detection based on one-class classification, integrating neural network embeddings with Support Vector methodologies.

Clinical neurorehabilitation requires objective biomarkers to accurately detect and monitor neuromotor impairments. While traditional metrics are favored for their efficiency and minimal equipment demands, they typically rely on subjective observation and lack physiological resolution. High-resolution electrophysiological technologies, such as electroencephalography (EEG) and electromyography (EMG), offer objective insights into nervous system dynamics; however, prolonged data acquisition times have hindered their translation into frontline clinics. Strategic core theory makes a distinction between roles within a team that are central to the team’s workflow (i.e., core roles) and those that are not central (i.e., non-core roles) (Humphrey et al., 2009). A strong consensus has taken hold in the legal academy. Criminal Justice Localism, espoused by scholars of every stripe, promotes greater local control of every facet of the criminal justice system. But Criminal Justice Localism largely overlooks an institution that plays a critical role in the criminal justice system, and which has been the subject of intense public criticism: the police. The emergence of large-scale foundation models has reshaped computer vision and multi-modal learning, but their substantial computational and communication demands pose significant challenges for deployment in federated learning (FL) systems. Federated learning enables collaborative model training across decentralized clients without sharing raw data, offering strong privacy guarantees. However, adapting foundation models in FL remains difficult due to communication bottlenecks, heterogeneous data distributions, limited device resources, and scarce labeled data. The purpose of this dissertation was to evaluate the reliability, validity, and responsiveness to training intervention of a pain-anchored capacity threshold, the Physical Working Capacity at the Pain Intensity Threshold (PWCPIT), as a practical submaximal marker of endurance-related exercise tolerance. Study one examined the test-retest reliability of pain-based, perceptual, and physiological physical working capacity measures in recreationally active men using a standardized multi-bout cycling protocol, with pain intensity responses demonstrating strong reproducibility and low measurement error, supporting the stability of pain-anchored thresholds when standardized procedures are applied. This dissertation examines how medieval historical narratives circulate within contemporary Western digital discourse surrounding Russia’s invasion of Ukraine. Russian political rhetoric has frequently invoked the medieval polity of Kyivan Rus’ to assert civilizational continuity between the medieval past and the modern Russian state. While these claims have been widely discussed in political and media contexts, less attention has been given to how such narratives appear within everyday online discourse. This study investigates how medieval references are mobilized, circulated, and interpreted within social media environments during the early conflict.

The introduction of invasive species can disrupt ecosystems not only through competition and predation but also by introducing novel pathogens to naïve host populations. In the southeastern United States, the invasive Burmese python (Python bivittatus) has been implicated in the introduction of the pentastome parasite Raillietiella orientalis, which has been associated with increased parasite burdens and mortality in native snakes. Despite their ecological significance and ancient evolutionary history, pentastomes remain poorly represented in genomic databases, limiting efforts to understand their evolutionary origins, host–parasite interactions, and mechanisms underlying parasite emergence. Household food insecurity is a multidimensional issue affecting millions of households in the United States. While research has established that low income and poor health or functional limitations can increase the risk of food insecurity, current approaches do not fully capture these dynamic processes to identify long-term vulnerability. Specifically, income-based measures rely on static thresholds that miss instability, potentially underestimating risk. Gaps also remain in understanding persistent food insecurity, including how functional limitations (difficulties with activities like bathing, shopping, or managing finances) are associated with insecurity over time. The resilience process model highlights an approach to how people mobilize their resources to cope and positively adapt in the face of adversity. Leveraging Conservation of Resources (COR) theory and the Work Home Resources Model (WHRM), I aim to uncover the extent to which resilience capacity (resources) and resilience enactment (coping) influence outcomes of resilience, offering one of the first empirical tests of the resilience process model. Specifically, COR theory posits that the more psychological resources someone has, the more successful they are in coping. The rapid expansion of the Internet of Things (IoT) has transformed modern computing by enabling seamless connectivity among heterogeneous devices across diverse application domains. However, this increased interconnectivity has significantly enlarged the attack surface of IoT networks, exposing them to a wide range of sophisticated cyber threats. Conventional security mechanisms often lack the capability to detect emerging attacks in real time, thereby necessitating the development of intelligent Intrusion Detection Systems (IDS) capable of accurately identifying malicious network activities. Despite extensive research documenting cultural and racialized trauma, no validated assessment tool had previously captured the intrafamilial mechanisms of trauma transmission specific to the BADEP population, whose trauma lineage is rooted in slavery. The present study addresses this gap through the development and initial validation of the Survivors of Historical Intergenerational and Familial Trauma Scale (SHIFTS), a culturally grounded instrument designed to measure the transmission of historical trauma (HT) and ongoing intergenerational trauma (IGT) among Black American Descendants of Enslaved People (BADEPs). Introduction: Bariatric surgery is the most effective treatment modality for individuals with morbid obesity, providing significant and durable weight loss and comorbidity resolution. Glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide receptor agonists have shown promise as weight loss drugs, in addition to their use in the treatment of metabolic disorders. While multimodal weight management is the standard of care for individuals with morbid obesity, the benefit of antecedent GLP-1 therapy prior to bariatric surgery has not been well-studied.
