背景

髓鞘少突膠質細胞糖蛋白抗體相關疾病(MOGAD)是一種罕見的自體免疫發炎性疾病,主要涉及中樞神經系統(CNS),其主要特徵是視神經、腦部和脊髓脫髓鞘。我們報告一例最初疑似復發性中風的病例,該病例在住院中風復健期間最終確診為MOGAD。

病例概要

49歲女性,左側大腦半球梗塞,在中風後復健期間出現多次右側肢體肌力下降伴隨失語症。這些神經功能波動偏離了典型卒中的預期復健軌跡。卒中發作兩個月後行腦部磁振造影(MRI)檢查,結果顯示右側內囊後肢、腦橋及C1脊髓出現多處新發病灶。後續血清學檢查顯示抗髓鞘少突膠質細胞糖蛋白(MOG)IgG抗體陽性,而水通道蛋白4(AQP4)抗體陰性,確診為MOGAD。患者接受了甲潑尼龍治療及充分的復健治療,追蹤期間肌力顯著改善,未見脫髓鞘病灶復發。

結論

本案例凸顯了復健科醫師在早期發現類似中風的神經發炎性疾病的關鍵作用。復健科醫師處於獨特的領先地位,能夠透過持續的功能監測識別「危險信號」;任何偏離預期復健路徑或無法解釋的神經功能波動都應促使醫生進行進一步檢查。早期識別此類非典型模式,並及時進行免疫治療和多學科康復,可以預防永久性功能衰退,並優化長期預後。

Language barriers significantly hinder academic performance, psychological well-being, and clinical communication skills of Arabic-speaking healthcare students educated through English-medium instruction. Although existing instruments focus on clinical communication—often among physicians—critical dimensions such as academic challenges and psychological impacts remain underassessed, and tools rarely address disciplines beyond medicine (e.g., nursing, pharmacy, radiology). The development of Artificial Intelligence (AI) in healthcare is largely dependent on the quality of medical datasets. However, these datasets often fail to accurately represent women (and those seen as women) due to historic, implicit and biological biases. This under-representation can lead to biased, inequitable and even harmful models. Building upon the findings of previously conducted qualitative semi-structured semantic interviews with clinicians on their perceptions of women’s health, this paper presents a framework for translating qualitative findings to dataset characteristics via operationalisation. The concept of neighborhood has been long studied with little agreement beyond the notion that neighborhoods are somewhat self-contained areas within a wider urban environment. Despite the lack of a finite definition, there is wide agreement of the importance of studying population dynamics at the neighborhood scale. Many attempts have been made to spatially define, measure, and quantify contemporary neighborhoods, but less attention has been given to methods to spatially define historical neighborhoods. BACKGROUND: Stroke leads to complex chronic structural and functional brain changes that specifically affect motor outcomes. The brain predicted age difference (PAD) has emerged as a sensitive biomarker of both sensorimotor and cognitive function after stroke. Our previous study showed a higher global brain PAD associated with poorer motor function after stroke. However, the association between local stroke lesion load, regional brain age, and motor impairment is unclear. This study aimed to investigate the associations between focal lesion damage, regional brain PAD in both hemispheres, and motor outcomes in chronic stroke, and to identify key predictors of motor impairment.

There are consumers who participate in political consumerism by avoiding, rejecting, or condemning products and services from countries perceived as aggressors through their military or economic action. This study examines how Filipino college students in Metro Manila, Philippines, who are frequently exposed to social media, will be affected as consumers amid the current tension in the West Philippine Sea. Using the theories of social identity, uses and gratifications, and echo chamber as frameworks, 770 college students from private and public universities in Metro Manila were surveyed. This study investigates the factors influencing brain drain, the flight of human capital from their home country. The study examines the drivers of brain drain by looking into the effects of the following: global peace index, CO2 emissions per capita, real minimum wage, and happiness index and its individual indicators. Using pooled OLS on an unbalanced panel, the results reveal a statistically significant yet negative effect of the happiness index and the real minimum wage on brain

This study explores the diffusion process of the internet in countries that were previously known as the U.S.S.R. and are currently divided into 15 countries. Utilizing the S-shaped logistic curves, this paper forecasts future trends in internet diffusion in these economies. Data varied in range, with each country having different time frames for similar variables. The logistic curves for each country determined the length of time of internet diffusion, maximum carrying capacity, and different stages of the diffusion process, namely, emerging, growth, maturity, and saturation. Ccb Release Impact On Indonesia’S Bank Lending: A Difference-In-Differences Approach, Sang Tyas Suci, Beta Yulianita Gitaharie

Jurnal Ekonomi dan Pembangunan Indonesia

This study investigates the impact of Indonesia’s Capital Conservation Buffer (CCB) release on loan growth during the COVID-19 recession. Using a Difference-in-Differences (DiD) approach and bank-level data from 2019:Q1 to 2022:Q2, it analyzes how regulatory capital relief affected lending. The results indicate that the regulation was an effective countercyclical tool to increase loan growth, particularly among less-capitalized banks. The research contributes to the limited empirical evidence on capital buffer policies in emerging markets by examining the effectiveness of temporary capital relief in crisis contexts. We suggest future research on the buffer reimposition and exploring its implications for post-crisis lending behavior.

Purpose: The purpose of this mixed-methods study was to examine the extent to which health professions graduate students' demographic, academic, and environmental factors influence time to graduation. Allied professionals comprise a significant portion of the law library workforce, yet their experiences and career pathways are often overlooked in discussions about the profession’s future. This article presents findings from a survey examining how allied professionals pursue professional development, upskilling, and career mobility. While respondents demonstrated a strong commitment to growth, opportunities varied widely and were often limited by institutional support. The authors discuss the survey findings, explore the implications for the profession, and offer recommendations for better supporting allied professionals and recognizing their essential contributions to law librarianship. Introduction and importance: Propeller perforator flaps provide a reliable reconstructive option for distal lower-limb soft-tissue defects, particularly in resource-limited settings where microsurgical free-flap reconstruction may not be readily available. The interior spaces are designed to reflect the identity and values of the mountannering clubs and the wider culture of climbing and mountain sports. Through both materiality and atmosphere, the project seeks to create an environment strongly connected to the experience of mountaineering and the community it supports. Elements such as tensioned ropes, climbing cords, carabiners, metal details, and topographic elements reference the equipment commonly associated with climbing and alpine activities. The overall aesthetic is clean and functional, with restrained use of colour inspired by the visibility of climbers against rocky mountain landscapes.
