Cultural competence remains essential in primary care settings serving diverse populations, yet staff at a small Southern Texas primary care clinic reported no previous formal cultural competency training in workplace orientation or school. The purpose of this Doctor of Nursing Practice quality improvement project was to evaluate whether a brief, evidence-based cultural competency intervention improved clinician cultural competency scores. Madeleine Leininger’s Culture Care Theory provided the theoretical framework, Lewin’s Change Theory guided the change process, and the Plan-Do-Study-Act cycle directed implementation and evaluation. Suicide remains a leading public health concern, and inconsistent application of the Columbia-Suicide Severity Rating Scale (C-SSRS) may contribute to variability in suicide risk assessment and delayed intervention within inpatient psychiatric settings. The purpose of this quality improvement (QI) project was to evaluate the effectiveness of a standardized C-SSRS-guided educational intervention on staff competency in suicide risk screening. Guided by Lewin’s Change Theory and implemented using the Plan-Do-Study-Act (PDSA) framework, this quantitative pretest-posttest QI project was conducted on an adult inpatient psychiatric unit. Purpose
The purpose of the quality improvement project was to evaluate the effectiveness of implementing a data driven debrief tool to improve the average compressions in target in adult cardio-pulmonary resuscitation (CPR) events.
Results
In FY23 prior to implementation the average compressions in target was only 1.65%. During the implementation phase of the project the average compressions in target reached 41% (n=3). In addition, the staff provided positive feedback regarding the tool and the integration in the workflow.

BACKGROUND: Dysphagia is a known complication of Parkinson's Disease (PD). Percutaneous endoscopic gastrostomy (PEG) tube placement is a therapy for select patients with dysphagia. There is a paucity of information on the role of PEG in PD management.
OBJECTIVES: To investigate the frequency and outcomes of PEG tube implantations in hospitalized patients with PD.
METHODS: We performed a retrospective review of the PINC AI™ Healthcare Database of hospitalized PD patients between 2017 and 2023. Pheochromocytoma typically presents with sustained or paroxysmal hypertension, but initial manifestation as acute non-ST-segment elevation myocardial infarction (NSTEMI) is rare. Peri-procedural hemodynamic fluctuations are often dramatic, making diagnosis and management challenging. We report a 51-year-old male admitted with chest tightness for 17 days. Elevated troponin and coronary angiography showing multivessel disease with total occlusion of the mid-left anterior descending artery (LAD) led to a preliminary diagnosis of NSTEMI. During percutaneous coronary intervention, blood pressure surged to 240/130 mmHg; intravenous nitroprusside caused an instantaneous drop to 95/55 mmHg, with rapid rebound to >200 mmHg upon withdrawal, showing no clear temporal relationship …

Airway smooth muscle (ASM) is a key determinant of airway caliber and a major contributor to structural remodeling in obstructive lung diseases, including asthma and COPD. ASM proliferation is regulated by transcriptional and post-transcriptional mechanisms, including ubiquitin-dependent protein turnover mediated by E3 ubiquitin ligases. Here, we investigated the role of the RING-type E3 ligase RNF145 in mitogenic signaling and ASM cell proliferation. Human ASM cells were transfected with RNF145 shRNA or treated with the E3 ligase inhibitor SMER3, followed by stimulation with fetal bovine serum (FBS) or platelet-derived growth factor (PDGF). Purpose: To evaluate whether implementing a staggered beam arrangement in volumetric modulated arc therapy (VMAT) improves treatment plan quality by enhancing normal tissue and organ-at-risk (OAR) sparing in patients with localized prostate cancer.
Methods: Ten patients meeting the study inclusion criteria were retrospectively replanned using both a standard arc technique and a staggered arc arrangement. Both plans were generated using identical optimization objectives and planning parameters. Dosimetric comparisons were performed after normalizing both plans to equivalent planning target volume (PTV) coverage. Dose-volume metrics were evaluated for the PTV, rectum, bladder, and penile bulb.

The ability to self-regulate is essential for young children's social, emotional, behavioral, and academic development. However, growing exposure to adverse childhood experiences (ACEs), toxic stress, family instability, and other social and environmental stressors has contributed to challenges in the development of these skills. Children with underdeveloped self-regulation skills are more likely to experience difficulties with executive functioning, peer relationships, classroom behavior, and school readiness, demonstrating the need for early intervention. This doctoral capstone project, completed during a 14-week Applied Leadership Experience (ALE) with Georgetown-Scott County EMS (GSCEMS), focused on the ACOTE target areas of Program Development and Advocacy by creating an evidence-based, trauma-informed, occupational therapy support group curriculum for motor vehicle accident (MVA) survivors. A literature review and needs assessment identified a significant gap in post-crash care, as many survivors experience long-term psychological challenges—including PTSD, anxiety, depression, fear of driving, and disruptions in daily roles—despite care often emphasizing physical rehabilitation over emotional recovery. The project aimed to achieve 80% compliance with daily electrode changes, improve alarm response times by 10%, reduce telemetry technician calls by 15%, and reduce alarm fatigue levels by 20%.

Background: Infectious disease outbreaks remain a persistent public health threat in Malawi. The 2022–2024 polio outbreak highlighted the central role of enhanced surveillance in timely detection and effective response. This abstract examines key strategies, outcomes, and lessons for strengthening surveillance and preparedness.
Methods: The response leveraged multiple surveillance approaches, including Acute Flaccid Paralysis (AFP) and environmental surveillance, improved specimen transport, digital tools for case investigation, and multiple supplementary immunization campaigns. Data from surveillance indicators and outbreak response activities were analyzed to assess performance and identify challenges.

Sexuality and intimacy are recognized as meaningful occupations within occupational therapy practice; however, they remain inconsistently addressed in clinical care. Previous research has identified limited educational preparation as a contributing factor to occupational therapists' discomfort and lack of confidence in addressing these occupations. Although classroom instruction provides foundational knowledge, Level II fieldwork serves as the primary setting in which students apply knowledge within authentic clinical environments. How do whole persons form for lives of character and flourishing within graduate schooling years? What steps could an institution take to move toward that end? This work describes the process of one institution’s use of a task force examining these questions using a combination of literature review and structured interviews with key stakeholders across campus. The context of the institution’s unique mission, vision, and virtue framework provided a foundation out of which four areas of focus emerged: 1) infrastructure and representation, 2) belonging and community, 3) communication, and 4) professional identity formation. Purpose: To evaluate dosimetric differences between breast boost plans generated on an initial prone whole-breast computed tomography (CT) simulation and those generated on a near-boost supine re-simulation CT acquired closer to boost delivery.
Methods: Ten patients receiving hypo-fractionated whole-breast irradiation followed by a sequential boost were retrospectively analyzed. Boost plans were generated using a three-field photon technique on both the initial prone CT and a near-boost supine CT. Target volumes, target coverage metrics, conformity index (CI), and organ-at-risk (OAR) doses were compared using paired statistical analyses.

BACKGROUND
Despite antiretroviral therapy (ART), people with HIV (PWH) are at heightened risk for insulin resistance (IR) and type 2 diabetes (T2D). Subcutaneous adipose tissue (SAT) fibrosis contributes to metabolic disease, but its role in IR among PWH is unknown. We investigated the relationship between SAT fibrosis and IR in PWH, along with transcriptional signatures to distinguish it from SAT fibrosis due to obesity.
METHODS
We analyzed body composition and SAT fibrosis (hydroxyproline) in 46 PWH and 74 people without HIV (PWoH), excluding individuals with T2D. We examined fibrosis-related gene transcription in the SAT using a targeted panel and measured …

Esophageal squamous cell carcinoma (ESCC) remains a major global health burden, particularly in East and Southeast Asia, where incidence rates are highest. Early-stage ESCC represents a critical therapeutic window in which curative treatment can be achieved using minimally invasive, organ-preserving approaches. Advances in endoscopic imaging, including narrow-band imaging, magnifying endoscopy, chromoendoscopy, and emerging modalities such as endocytoscopy and artificial intelligence–assisted detection, have substantially improved the accuracy of early diagnosis and invasion depth assessment. According to current staging systems, including the AJCC/UICC classification, endoscopic resection is an established curative treatment for carefully selected patients with mucosal ESCC. The July 2026 Employment Situation of Veterans report indicates that veteran unemployment increased slightly from 4.1% in June to 4.2% in July, while post-9/11 veteran unemployment improved marginally from 4.8% to 4.7%. Although overall veteran employment conditions remained relatively stable, notable differences emerged across demographic groups. Younger veterans (ages 18–24) experienced a significant decline in unemployment, while rates increased among veterans ages 45–54. Female veteran unemployment rose slightly but remained below that of nonveteran women. Sharp increases were reported among Black and Hispanic veterans, highlighting persistent disparities. Objectives: Breast cancer (BC) is the most common malignancy among women worldwide and represents a significant burden for Yemeni women. Early detection through breast self-examination (BSE) and knowledge of risk factors and symptoms are crucial. MALT1 protease is an intracellular signaling molecule that promotes tumor progression via cancer cell-intrinsic and cancer cell-extrinsic mechanisms. MALT1 has been mostly studied in lymphocytes, and little is known about its role in tumor-associated macrophages. We show that MALT1 is expressed in glioblastoma (GBM)-associated macrophages. Mechanistically, GBM tumor cells induce a MALT1-NF-κB signaling axis in macrophages, leading to enhanced macrophage migration and polarization toward an immunosuppressive ('M2-like') phenotype. Inactivation of MALT1 protease promotes transcriptional reprogramming that reduces migration and restores a macrophage anti-tumor 'M1-like' phenotype. Preclinical in vivo analysis shows that MALT1 inhibitor treatment results in immuno-reactivity of GBM-associated macrophages …

Mutations in the MECP2 gene cause the severe neurological disorder Rett syndrome. A cluster of frameshift-causing C-terminal deletions (CTDs) removes ~100 amino acids and accounts for approximately 10% of RTT-causing mutations. Their pathogenicity is unexpected because this C-terminal domain is dispensable in mice. Analysis of pathogenic and benign human MECP2 variants reveals that some individuals with apparently typical CTDs do not develop Rett syndrome, confirming that C-terminal truncations are not intrinsically pathogenic. Cigarette smoke promotes bladder tumor growth by enhancing cancer cell survival and proliferation through smoke mediated carcinogens. FASN, a key enzyme in fatty acid synthesis, is dysregulated in many cancers and correlates with aggressive phenotypes. In this study, we demonstrate elevated fatty acid levels and FASN specifically in smokers with bladder cancer. Elevated FASN under smoke exposure imparted epigenetic alterations, particularly histone acetylation, impacts DNA repair and DNA-binding transcription factors which regulate metabolic pathways. Japanese encephalitis virus (JEV), the prototype member of the JEV serogroup within the genus Orthoflavivirus (family Flaviviridae), is closely related to West Nile virus (WNV), Murray Valley encephalitis virus (MVEV), and St. Louis encephalitis virus (SLEV). To characterize antigenic cross-reactivity within this group, we evaluated 15 region-specific rabbit antisera, previously generated against nearly the entire JEV protein-coding region, by immunoblotting whole-cell lysates from BHK-21 cells infected with WNV, MVEV, or SLEV, with JEV included as a reference. Coastal marshes are among California’s most productive ecosystems and the most impacted, having been extensively modified by dredging, filling, and urban development. This thesis investigates over a century of change for a marsh in Northern Mission Bay, San Diego, California using ecological and spatial analyses.
Chapter 1 provides an overview of the environmental history and current land use of the Northern Mission Bay marsh, tracing changes from pre-contact Indigenous stewardship through colonial and modern urban development.
