This Doctor of Nursing Practice project was a staff education quality improvement initiative designed to optimize telehealth training for nurses to improve medication adherence in adults with posttraumatic stress disorder (PTSD). Telehealth has become an essential modality for delivering mental health care, yet its effectiveness depends heavily on nurses’ ability to communicate effectively, build trust, and engage patients in treatment discussions. Inaccurate and inconsistent administration and interpretation of standardized depression screening instruments can destabilize clinical accuracy and patient safety. Despite routine administration of the ZSDS during nursing intake, informal observation and documentation review indicated a gap in nurses' understanding of how to compute raw scores, convert them to index scores, and define the severity levels. The purpose of this doctoral project was to implement a quality improvement educational initiative to strengthen provider understanding of antidepressant side effects in an outpatient behavioral health setting. The project was conducted to address gaps in staff knowledge related to identifying and managing antidepressant adverse effects, which are essential skills for safe medication practices in mental health care. Addressing this gap was significant because unrecognized antidepressant side effects can contribute to poor adherence, worsening symptoms, preventable complications, and negative patient outcomes. The results of the project demonstrated meaningful improvements in provider knowledge. Six staff members completed pre- and posttest assessments. …

Buprenorphine (BUP) is widely used in the treatment of neonatal opioid withdrawal syndrome (NOWS). However, the most compounded formulation contains 30% ethanol, despite regulatory and clinical concerns regarding ethanol exposure in pediatric patients. Thus, this research aimed to develop an ethanol-free sublingual (SL) gel formulation of BUP that would be safe, stable, and suitable for NOWS. Multiple polymers were screened as gelling agents, with hydroxypropyl methylcellulose (HPMC) emerging as the ideal base polymer for the formulation due to its optimal pH, rheological characteristics, and stability. Military service provides a path toward the middle class by granting veterans benefits, including healthcare and compensation for disabilities acquired during service. Military characterization of service, assigned by the Department of Defense at separation, can qualify or disqualify veterans from benefits access. Black service members are 50% more likely than White service members to receive an Other Than Honorable rather than an Honorable discharge, resulting in disproportionate impediments to the benefits designed to facilitate re-entry into civilian life and prevent poverty. Yet, the Department of Defense provides little meaningful remedy for Black veterans to correct less than Honorable discharges, instead …

This case study describes the integration of a board-certified music therapist into two specialty dental clinics at the University of Rochester Medical Center’s Eastman Institute for Oral Health. Initiated in collaboration with the Eastman Performing Arts Medicine Center, the effort reflects a humanistic and biopsychosocial approach to dentistry that leverages music therapy to reduce anxiety, improve patient experience, and support better clinical outcomes.

Before implementation, monthly interdisciplinary planning meetings were held, bringing together dental faculty, research and administrative staff, and the music therapy team. Activation of cGAS-STING signaling in cancer cells requires cytosolic DNA produced by intrinsic or treatment-induced DNA damage. However, clinical efforts to exploit this pathway to improve immunotherapy have yielded limited success, highlighting gaps in understanding the link between DNA damage and immunotherapy. Here, we identify ubiquitination-directed cytosolic DNA degradation as a critical determinant for cGAS-STING activation following DNA damage. Mechanistically, the cytosolic DNA exonuclease TREX1 is degraded by the E3 ubiquitin ligase SPOP but is reversely stabilized by the deubiquitinase USP7. Cancer-associated SPOP mutations or USP7 overexpression elevate TREX1 levels, promoting cytosolic DNA degradation and impairing cGAS-STING-mediated immune activation. Notably, …

The co-evolution of different cell subsets in the progression of precursor lesions to lung adenocarcinoma (LUAD) is incompletely understood. We generated spatial transcriptomic maps of 56 human precursor lesions and LUADs from 25 patients and of an independent cohort of 36 lesions from 19 patients, analyzing a total of 486,519 spots and 5.4 million cells. We identify region-specific programs that distinguish precursors from LUADs. Spatially resolved clonal architectures reveal patient-specific heterogeneity in evolution of precursors to LUADs. We find epithelial alveolar progenitors expressing tumor-associated meta-programs and residing in niches enriched with proinflammatory subsets including IL1B high macrophages. Proper regulation of ribosome biogenesis is essential for stem cell function and tissue homeostasis, yet its upstream control in adult intestinal stem cells (ISCs) remains unclear. Here, we identify the WD repeat protein Wdr4 as a key regulator of ISC homeostasis in the Drosophila midgut. Wdr4 cooperates with the methyltransferase Mettl1 to catalyze N⁷-methylguanosine (m⁷G) modification of let-7 miRNA. Wdr4 or Mettl1 depletion disrupts this modification, reducing let-7 levels and aberrantly activating TOR-JNK-dMyc signaling. This drives elevated ribosome biogenesis, ISC overproliferation, misdifferentiation, and intestinal dysplasia. Overexpression of let-7, inhibition of TOR, or suppression of JNK rescues these defects. HER2-low breast cancer (IHC 1+ or 2+ without HER2 gene amplification) is a distinct and understudied subtype. In a cohort of 14,593 early-stage breast cancer patients treated at MD Anderson from 2006 to 2019, 60.4% were HER2-low. Multivariable analysis showed HER2-low status was independently associated with race, histologic subtype, higher nuclear grade and stage, and estrogen receptor (ER) positivity. Among 2464 patients receiving neoadjuvant chemotherapy, HER2-low status was not linked to pathologic complete response (pCR), overall survival (OS), or disease-free survival (DFS) compared to HER2-0 tumors. Rapid and comprehensive analysis of complex proteomes across large sample sets is vital for unlocking the potential of systems biology. We present a high-throughput mass spectrometry (MS) proteomics method that integrates narrow-window data-independent acquisition (nDIA) with short-gradient micro-flow chromatography, enabling profiling of >240 samples per day. This optimized MS approach identifies 6,201 and 7,466 human proteins with 1- and 2-min gradients, respectively. As a practical application, we analyzed 507 samples composed of 13 different tissues from mice treated with the enzyme-drug L-asparaginase (ASNase) or its glutaminase-free Q59L mutant, generating a quantitative profile of 11,472 proteins following drug treatment. This study probes the intricate relationship between economic complexity and inflation dynamics, an area that has been relatively under-researched despite the potential for economic complexity to predict inflation. We focus on selected OECD countries and use panel data from 1998 to 2021. Utilizing a Panel Autoregressive Distributed Lag (PARDL) model, we analyze both short- and long-term dynamics across various components of inflation. Our analysis encompasses both aggregate and disaggregated indicators of complexity, including ECI-Trade, ECI-Technology, and ECI-Research. The results indicate that the impact of economic complexity on inflation differs depending on the type of inflation and the time frame considered. …

This study presents an optimized design and operation strategy for a stand-alone solar photovoltaic power system to drive a 4.5 kW elevator at Mansoura University Hospital, Egypt. The research aims to reduce the battery bank capacity and overall system cost while maximizing the utilization of solar panels. A novel "Optimum Operator System" is proposed, incorporating three power switches controlled by an Arduino microcontroller. The system's performance is analyzed through extensive modeling, simulation, and experimental validation. The Head Start program, launched in 1965 and targeted to children from disadvantaged backgrounds, remains the largest early childhood care and education (ECE) program in the United States and the only one deployed at the federal level. As such, the Head Start literature spans several decades and now allows for synthesis of findings from different contexts, time periods, and research designs. For a long time, from the nineteenth century to most of the twentieth century, the cerebellum was thought to be an organ that regulates movement. Towards the end of the twentieth century, the brain functions associated with the cerebellum began to extend beyond motor control. Now, there is a consensus that the cerebellum is involved not only in motor functions but also in the most basic autonomic functions and the most complex cognitive and emotional functions, with a focus on predictions and internal models. Purpose

The role of adjuvant fractionated radiotherapy (aFRT) after gross total resection (GTR) of WHO-2 meningiomas remains unclear. We aimed to estimate the effect of aFRT on recurrence risk and survival following GTR and subtotal resection (STR).

Methods

We analyzed 1452 patients with WHO-2 from our international, multicenter database (followed between 1989 and 2019). Outcomes were recurrence (10-year follow-up) and death (5-year follow-up). Risk estimates were obtained using competing risks and survival analysis. Electroencephalography (EEG) eye state classification is crucial as it enables noninvasive monitoring and identification of different eye states. This importance has been underscored in studies exploring the use of machine learning techniques for accurate classification. Previous research has primarily applied supervised learning methods to EEG signal analysis for eye state classification using real-world datasets, focusing on improving accuracy through novel algorithms. Purpose: The global food system is undermining human and planetary health. Urgenttransdisciplinary action is required among food system stakeholders equipped to address complexchallenges. This study identified and prioritised curriculum concepts for tertiary education, with theaim of preparing entry-level nutritionists and dietitians to contribute to sustainable food systems andplanetary health within their practice.Methodology: A mixed-methods Q study was conducted. Mechanisms responsible for skeletal muscle kidney crosstalk have not been defined. We have determined that a circulating mediator, signal regulatory protein α (SIRPα), impairs intracellular insulin-mediated functions. To elucidate the effect of myokine SIRPα on diabetic kidney disease (DKD), flox mice and muscle-specific (m-specific) SIRPα-KO mice were subjected to an obesity-induced model of diabetes, high-fat diet (HFD; 60%) or insulin-deficient hyperglycemia model, streptozotocin (STZ), and were subsequently exposed to anti-SIRPα monoclonal antibodies. In the obesity-induced diabetic mice, serum SIRPα increased. Purpose: Familial history of aortic disease has been associated with increased risk of aneurysm-related complications after standard endovascular aortic repair. The aim of this study was to evaluate outcomes of fenestrated-branched endovascular aortic repair (FB-EVAR) for complex aortic aneurysms in patients with or without familial aortic aneurysms (FAAs).

Whilst youth offending has been declining, there have been increases in serious youth violence in the last decade. Therefore, there is a pressing need to prevent youth crime and violence owing to its prevalence, harms and cost to society. Part of the effort to address this involves identifying and disseminating evidence-based practice. We explore key challenges in this endeavour and offer ideas for how to address them. These fall into two categories. The first concerns the focus and nature of interventions and the imperative to increase the effectiveness of our collective efforts.
