Purpose: Novel methods have been developed for functional avoidance that proposes to use 4DCT to derive lung ventilation and perfusion images. Previous methods for quantifying lung perfusion on non-contrast 4DCT rely either on HU-based physical models or black-box deep learning models. While deep learning typically achieves higher accuracies in image processing tasks, physical models provide a rationale for model predictions. The purpose of this study is to introduce a biophysics-informed machine learning method for estimating pulmonary perfusion from noncontrast 4DCT. Our approach is designed to combine the predictive power of neural networks with the interpretability of physical modeling, with the …

Purpose: To improve comprehension in traditionally difficult subjects for a senior-level undergraduate medical physics course, we designed and implemented a Team Learning technique. This work evaluates the translation of Team Learning to medical physics. Methods: At the beginning of the term, students were sorted into 3 even teams (students/year = 14.5). The class met twice weekly: a traditional lecture was taught first, and each team was assigned a specific 1/3 of the lecture material in which to specialize. Each team was assigned to create a PowerPoint presentation on their material using outside references and without copying the original figures or …

Purpose: CT-derived perfusion (CTP) is a novel image processing modality that quantifies pulmonary perfusion from non-contrast inhale/exhale CT image pairs. While existing imaging markers for COPD primarily depend on morphological features or HU thresholding, CTP provides a quantitative marker of blood flow and has been shown to identify disease progression prior to the appearance structural abnormalities. In this study, we assess the potential utility of CTP as a disease severity marker for COPD. Methods: CTP was computed from high-resolution inhale-exhale CT scans for 785 patients with a Global Initiative for Obstructive Lung Disease (GOLD) score ranging from 0 to 4 …

Purpose: ExacTrac is clinically used for inter- and intra- fraction shift corrections for hypofractionated prostate patients by registering to implanted fiducial markers. ExacTrac corrections for thirty-three prostate patients were collated and analyzed. Methods: ExacTrac shifts were exported for prostate patients (140 total fractions) who were treated with two 180° partial arcs with beam-on time less than 60seconds/arc. Prior to each treatment fraction, a cone beam computed tomography (CBCT) image was acquired to verify bladder and rectal filling against simulation. Following CBCT, ExacTrac was used for initial prostate localization and shifts were applied using a 6 degree-of-freedom (6D) couch. Following initial …

Purpose: Measurement-based QA identifies large deviations between planned and delivered dose, but subtle—yet clinically unacceptable—deviations can pass through the conventional QA process unnoticed. Incorporating logfilebased information into pre-treatment QA should result in more robust QA overall, provided such information is stable across the entire treatment course. We present here a case-study of logfile stability to investigate the potential of logfiles as useful adjuncts in the QA process. Methods: We analyzed logfiles from six fractions of a VMAT plan delivered on an Elekta Infinity linac with an Agility MLC. To simplify the analysis, we considered a single 52 control point VMAT …

Purpose: Proton arc is a new treatment modality that delivers proton beams while continuously rotating the gantry. This study proposed a regularized l0-minimization primal dual active set with continuation (PDASC) algorithm for proton arc spot sparsity optimization to simultaneously optimize the plan quality and the beam delivery time (BDT). Methods: Based on the previously published beam delivery sequence model of IBA ProteusONE®, proton treatment delivery time is actually dominated by spot switching time (SSWT). SSWT is approximately linearly dependent on spot numbers. So we used a non-convex l0-norm to control the sparsity level of the regularized solution. The clinical objective …

Pilot Feasibility Study Of A Simple Regenerative Peripheral Nerve Interface Designed To Diminish Cutaneous Dysesthesia After Supraclavicular Operations, Alexander Yang, Robert W Thompson
2020-Current year OA Pubs
Supraclavicular operations can be associated with postoperative cutaneous dysesthesia and hypersensitivity. Regenerative peripheral nerve interfaces, created by attaching the proximal end of a divided peripheral nerve into a viable muscle target, can promote neurite regrowth and neuromuscular connections to help suppress painful nerve hyperactivity. During 40 consecutive operations for neurogenic thoracic outlet syndrome, we demonstrated that division of at least one of the superficial supraclavicular cutaneous sensory nerve branches was necessary in 98% of cases. We subsequently developed a novel regenerative peripheral nerve interface for supraclavicular operations using the adjacent omohyoid muscle and have described the technical steps involved in …

T1- and T2-weighted (T1w and T2w) images are essential for tissue classification and anatomical localization in Magnetic Resonance Imaging (MRI) analyses. However, these anatomical data can be challenging to acquire in non-sedated neonatal cohorts, which are prone to high amplitude movement and display lower tissue contrast than adults. As a result, one of these modalities may be missing or of such poor quality that they cannot be used for accurate image processing, resulting in subject loss. While recent literature attempts to overcome these issues in adult populations using synthetic imaging approaches, evaluation of the efficacy of these methods in pediatric …

Low levels of high density lipoprotein-cholesterol (HDL-C) are associated with an elevated risk of arteriosclerotic coronary heart disease. Heritability of HDL-C levels is high. In this research discovery study, we used whole-exome sequencing to identify damaging gene variants that may play significant roles in determining HDL-C levels. We studied 204 individuals with a mean HDL-C level of 27.8 ± 6.4 mg/dl (range: 4-36 mg/dl). Data were analyzed by statistical gene burden testing and by filtering against candidate gene lists. We found 120 occurrences of probably damaging variants (116 heterozygous; four homozygous) among 45 of 104 recognized HDL candidate genes. Those …

The receptor for activated C-kinase 1 (RACK1), a highly conserved eukaryotic protein, is known to have many varying biological roles and functions. Previous work has established RACK1 as a ribosomal protein, with defined regions important for ribosome binding in eukaryotic cells. In Plasmodium falciparum, RACK1 has been shown to be required for parasite growth, however, conflicting evidence has been presented about RACK1 ribosome binding and its role in mRNA translation. Given the importance of RACK1 as a regulatory component of mRNA translation and ribosome quality control, the case could be made in parasites that RACK1 either binds or does not …

Myelodysplastic syndromes (MDS) are hematopoietic stem cell disorders, the pathogenesis of which involves enhanced immune signaling that promotes or selects for mutant hematopoietic stem and progenitor cells (HSPCs). In particular, toll-like receptor (TLR) expression and signaling are enhanced in MDS, and their inhibition is an attractive therapeutic strategy. Although prior studies have reported increased expression of TLR2 and its binding partners TLR1 and TLR6 in the CD34

Imaging the infant brain with MRI has improved our understanding of early neurodevelopment. However, head motion during MRI acquisition is detrimental to both functional and structural MRI scan quality. Though infants are typically scanned while asleep, they commonly exhibit motion during scanning causing data loss. Our group has shown that providing MRI technicians with real-time motion estimates via Framewise Integrated Real-Time MRI Monitoring (FIRMM) software helps obtain high-quality, low motion fMRI data. By estimating head motion in real time and displaying motion metrics to the MR technician during an fMRI scan, FIRMM can improve scanning efficiency. Here, we compared average …

Aberrant activation or suppression of WNT/β-catenin signaling contributes to cancer initiation and progression, neurodegeneration, and bone disease. However, despite great need and more than 40 years of research, targeted therapies for the WNT pathway have yet to be fully realized. Kinases are considered exceptionally druggable and occupy key nodes within the WNT signaling network, but several pathway-relevant kinases remain understudied and "dark." Here, we studied the function of the casein kinase 1γ (CSNK1γ) subfamily of human kinases and their roles in WNT signaling. miniTurbo-based proximity biotinylation and mass spectrometry analysis of CSNK1γ1, CSNK1γ2, and CSNK1γ3 revealed numerous components of the …

BACKGROUND: Up to 20% of patients with cerebellar infarcts will develop malignant edema and deteriorate clinically. Radiologic measures, such as initial infarct size, aid in identifying individuals at risk. Studies of anterior circulation stroke suggest that mapping early edema formation improves the ability to predict deterioration; however, the kinetics of edema in the posterior fossa have not been well characterized. We hypothesized that faster edema growth within the first hours after acute cerebellar stroke would be an indicator for individuals requiring surgical intervention and those with worse neurological outcomes.
METHODS: Consecutive patients admitted to the neurological intensive care unit with …

Most Favored Racial Hierarchy: The Ever-Evolving Ways Of The Supreme Court’S Superordination Of Whiteness, David Simson
Michigan Law Review
This Article engages in a critical comparative analysis of the recent history and likely future trajectory of the Supreme Court’s constitutional jurisprudence in matters of race and religion to uncover new aspects of the racial project that Reggie Oh has recently called the “racial superordination” of whiteness—the reinforcing of the superior status of whites in American society by, among other things, prioritizing their interests in structuring constitutional doctrine. This analysis shows that the Court is increasingly widening the gap between conceptions of, and levels of protection provided for, equality in the contexts of race and religion in ways that prioritize …

Recipe generation from food images and ingredients is a challenging task, which requires the interpretation of the information from another modality. Different from the image captioning task, where the captions usually have one sentence, cooking instructions contain multiple sentences and have obvious structures. To help the model capture the recipe structure and avoid missing some cooking details, we propose a novel framework: Decomposing Generation Networks (DGN) with structure prediction, to get more structured and complete recipe generation outputs. Specifically, we split each cooking instruction into several phases, and assign different sub-generators to each phase. Our approach includes two novel ideas: …

In this research, a taxi travel time based Geographically Weighted Regression model (GWR) is proposed and utilized to model the public housing price in the case study of Singapore. In addition, a comparison between the proposed taxi data driven GWR and other models, such as ordinary least squares model (OLS), GWR model based on Euclidean distance and GWR model based on public transport travel time, have also been carried out. Results indicates that taxi travel time based GWR model has better fitting performance than the OLS model, and slightly better than the Euclidean distance-based GWR model, however, it is not …
Getting Into Equity, Samuel L. Bray, Paul B. Miller
Getting Into Equity, Samuel L. Bray, Paul B. Miller
Notre Dame Law Review
For two centuries, common lawyers have talked about a “cause of action.” But “cause of action” is not an organizing principle for equity. This Article shows how a plaintiff gets into equity, and it explains that equity is shaped by the interplay of its remedial, procedural, and substantive law. Equity is adjectival, that is, it modifies law rather than the other way around. Its power comes from remedies, not rights. And for getting into equity, what is central is a grievance. To insist on an equitable cause of action is to work a fundamental change in how a plaintiff gets …

This contribution to the Notre Dame Law Review’s annual Federal Courts Symposium on “The Nature of the Federal Equity Power” asks what the traditions of equity can tell us about Article III standing. I take as my point of departure the observation by Professors Sam Bray and Paul Miller, in their contribution to the Symposium, that equity does not have causes of action as such—or at least not in the same way as actions at law. This is potentially important for standing, as many academic critiques of the Supreme Court's standing jurisprudence have argued that standing should turn on …
