Case:
Background: Cardiac B Myosin heavy chain (MYH7) gene mutation variant c.5326A>G (p.Ser1776Gly) is found in some hypertrophic cardiomyopathy (HCM) cases. We present two Filipino ethnic HCM patients with this variant to indicate strong association between this variant and this ethnicity.
Case: 1) 57-year-old adopted man had echo for dyspnea. It showed left ventricular (LV) hypertrophy and LV ejection fraction of 38%. Coronary angiogram was normal. Cardiovascular magnetic resonance (CMR) suggested cardiac amyloidosis (CA) vs burned-out HCM. CA was ruled out by endomyocardial biopsy, hence the HCM diagnosis. He had the above variant.

Background: Lack of US-approved devices kept transcatheter aortic valve implantation (TAVI) for aortic insufficiency (AI) an area of unmet need. We present pure AI case managed with current TAVI devices.
Case: 90-year-old man with severe noncalcified trileaflet regurgitant aortic valve, dilated aortic root (4.1 cm), and hypokinetic left ventricle (LV), was deemed TAVI candidate and high surgical risk. SAPIEN Ultra Resilia 29mm valve +8 cc was deployed under pacing. Before femoral access site closure with pre-Perclose, we noted central AI jet on echo. Angiogram showed LV valve migration. We swiftly reinserted the 16-Fr sheath we just pulled. We deployed a …

Covenants Not To Compete Are Again Being Scrutinized By Federal Regulators, Daniel Schulte Jd

This column examines the increasing federal scrutiny of non-compete agreements by the Federal Trade Commission (FTC). It notes that both the Trump and Biden administrations have prioritized curtailing the misuse of these agreements, particularly in healthcare. The article provides practical steps for employers to audit existing agreements for reasonableness and individualization. It encourages exploring alternatives such as non-solicitation and confidentiality clauses and stresses the importance of documenting the legitimate business interests that justify any remaining non-compete requirements to withstand future regulatory scrutiny.

BACKGROUND: The Hokusai Venous Thromboembolism (VTE) Cancer trial demonstrated that edoxaban was noninferior to dalteparin for the treatment of cancer-associated venous VTE.
OBJECTIVES: We reanalyzed the trial using the win ratio, an approach that evaluates a composite of outcomes in a hierarchical order.
METHODS: Forty-nine thrombosis experts ranked 10 outcomes in order of clinical importance from all-cause death (most important) to clinically relevant nonmajor bleeding (least important). We performed unmatched pairwise comparisons between participants on edoxaban and those on dalteparin at 6- and 12-month follow-up. BACKGROUND: Rotator cuff repair is a common surgical procedure, but postoperative outcomes can be highly variable, and postoperative repair tissue healing remains a significant clinical challenge. Furthermore, the biomechanical effects of rotator cuff repair are not fully understood.
METHODS: Twenty-two participants scheduled for arthroscopic rotator cuff repair were evaluated preoperatively and at 3, 12, and 24 months postsurgery. The following data were recorded at each time point: glenohumeral and scapulothoracic kinematics, shoulder strength, and patient-reported measures of pain and function. Postoperative repair integrity was assessed via magnetic resonance imaging at 3 months and 24 months postsurgery.

Age-associated reduced motivation is a hallmark of neuropsychiatric disorders in the elderly. In our rapidly aging societies, it is critical to keep motivation levels high enough to promote healthspan and lifespan. However, how motivation is reduced during aging remains unknown. Here, we used multiple mouse models to evaluate motivation and related affective states in young and old mice. We also compared the effect of social isolation, a common stressor in aged populations, to those of aging. For decades, discourse has been constructed for and about Nigerian women, rather than in dialogue with them, despite their rich worldview rooted in Indigenous knowledge systems. Empirical studies have consistently focused on Europe and America as sources of knowledge production, while treating the Global South as the object of knowledge. As a result, little is known about how work and organising/organisation are done, particularly in the informal economy. This study, Gendered Organising: Decolonial Feminist Epistemologies from marginalised women, introduced us to the lived experiences of women textile traders in Southwest Nigeria.
