This paper examines the evolving landscape of healthcare decision-making in Japan’s rapidly ageing society, with a focus on the legal, policy, and ethical challenges it entails. It analyzes the limitations of the adult guardianship system, the risks associated with family-dependent decision-making, and the underutilization of advance care planning (ACP). The study further considers how demographic shifts, advances in medical technology, and socio-legal inertia affect end-of-life care, particularly for individuals with diminished capacity. A central theme is the inherent ethical dilemma in balancing individual autonomy with Japan's cultural context, where explicit self-assertion can be challenging, especially for older adults. This paper investigates the institutional transition in Japan’s acceptance of foreign workers in the agriculture and food manufacturing sectors, focusing on the restructuring of entry-stage training schemes and their linkage to skill-based employment. In particular, it traces the shift from the Technical Intern Training Programme (TITP) to the Employment for Skill Development (ESD) Programme as the primary entry route, and examines how these institutional changes are embedded within the Specified Skilled Worker (SSW) framework.

As telework (e.g., working from home) has become widespread, understanding its organizational impact from managers’ perspectives and identifying supervisor behaviors that influence this impact are critical. Drawing on work disruption theory, we examined the relationships between four supervisor behaviors in the Health and Safety Executive (HSE) management competencies—respectful and responsible (RR), managing and communicating work (MCW), reasoning and managing difficult situations (RDS), and managing the individual within the team (MIT)—and the perceived organizational impact of telework. This staff education project focuses on improving depression detection in a primary care settings using Patient Health Questionnaire (PHQ-9) screening tool. Depression is a leading cause of disability worldwide, requiring reliable and adaptable screening tools. The diagnosis of depression is one of the most common mental health disorders in primary care but is poorly identified and frequently missed. The lack of consistent screening for depression among patients often results in delayed identification of symptoms, which postpones early treatment, increasing the risk of complications associated with untreated depression. Syphilis remains a significant transfusion-transmissible infection, yet regional epidemiological data from Northern Sudan are lacking. This study aimed to establish preliminary baseline syphilis seroprevalence among asymptomatic blood donors in Merowe, Northern Sudan. A cross-sectional study was conducted in January 2022 at the Merowe blood donation center. Eighty-nine consecutive asymptomatic male blood donors were screened for anti-Treponema pallidum antibodies using a commercial Enzyme-Linked Immunosorbent Assay (ELISA) kit (Fortress Diagnostics, UK). Demographic data were collected via structured questionnaire. The overall seroprevalence was 2.25% (2/89; 95% CI: 0.6–7.9%). The donor population median age was 32 years (range: 18–64). Glucagon-like peptide 1 receptor agonists (GLP-1 RAs) are widely used for the management of type 2 diabetes mellitus (T2DM) and obesity, with growing evidence of their anti-inflammatory effects. We report the case of a 54-year-old woman with obesity, T2DM, and biopsy-confirmed plaque psoriasis who developed joint pain and stiffness consistent with psoriatic arthritis (PsA). Her symptoms persisted despite appropriate trials of topical corticosteroids, calcipotriene, and oral apremilast. She also declined methotrexate due to concerns about side effects. Imaging of her hands revealed distal interphalangeal joint space narrowing, marginal erosions, and periosteal reaction, supporting the diagnosis of PsA. The patient was …

Fragile X Syndrome (FXS), the most common inherited cause of intellectual disability and a leading monogenic cause of autism, arises from the loss of Fragile X Messenger Ribonucleoprotein (Fmr protein), leading to synaptic and cognitive dysfunction. Here, we demonstrate that the frequency-specific electrical stimulation of the medial septum/diagonal band of Broca (MSDB) can restore cognitive and synaptic impairments in Fmr1 knockout mice, a validated model of FXS. Thirty minutes of 35 Hz MSDB stimulation significantly improved subsequent recognition memory, sociability, spatial learning, and fear memory, with recognition memory enhancements persisting for up to 7 days after daily 30 min …

BACKGROUND: Generative artificial intelligence tools such as ChatGPT are increasingly used by medical students for self-directed learning. Although these models demonstrate linguistic fluency, their reliability as supplementary resources for preclinical education remains uncertain. In particular, comparisons with evidence-based references such as UpToDate are lacking.

‘Animal’ exploitation is not a new phenomenon. Whilst categorising ‘animals’ by broader groups rather than species helps us understand their experiences, our language still limits our ability to capture the nuances of their treatment. Dogs appointed as domesticated companions are subjected to a variety of experiences. Some are fed gourmet meals and taken to privately rented parks; and others suffer from the consequences of impulsive purchasing and physical abuse. This paper examines the quantitative scale, field composition, and financial structure of nonprofit organizations (NPOs) in rural regions experiencing population decline and aging at three levels—the nonprofit sector as a whole, policy fields, and individual organizations—and explores their adaptation and sustainability under demographic change. The empirical setting is certified NPO corporations located in Tottori Prefecture. Drawing on publicly available financial data for fiscal years 2014–2023, the study constructs ratio indicators and per-capita indicators and, in particular, depicts recent trends through a two-point comparison between FY2014 and FY2023 together with a description of year-by-year changes. This paper quantitatively analyzes trends in the working hours of schoolteachers in Japan using Teaching and Learning International Survey (TALIS) data from three survey waves. The analysis focuses on differences between fixed-term and permanent employment contracts and on school environment variables, and their relationships with working hours and teacher well-being. Teachers on permanent contracts tend, on average, to work longer hours than those on fixed-term contracts, largely because of greater time spent on administrative duties and extracurricular activities. School environmental characteristics and principal leadership show only weak statistical associations with total working hours. Subjective job satisfaction is lower, and workload …

Existing literature has well-documented the disproportionate effects of abortion restrictions upon socioeconomically disadvantaged women and women of color. This thesis builds on that work by exploring the relationship between state-level abortion restrictions and women’s self-reported health, as well as state-level infant mortality rates and maternal mortality rates. This project breaks these relationships down into three separate studies: Study 1 investigates the effect that abortion restrictions have on women’s self perceptions of health. Targeted drug delivery (TDD), specifically through targeting ligand–drug conjugates, has reshaped oncology by enabling selective delivery of cytotoxic payloads to cancer cells while minimizing uptake by normal tissues. A key approach relies on exploiting overexpressed cell surface receptors (CSRs) to enable selective uptake of drug conjugates via receptor-mediated endocytosis. This review delineates four clinically validated CSRs (HER2, Trop-2, Nectin-4, and SSTR2) with several FDA-approved drug conjugates. Furthermore, emerging CSRs (EGFR, DLL3, and keratin 1) that may support next-generation TDD platforms for cancer treatment are also highlighted. Currently, Oman Vision 2040 articulates commitments to social cohesion, global citizenship, and holistic development. Existing empirical research indicates constant challenges related to bullying, student well-being, emotional distress, and school-related harm. This study conceptualizes peace education as a structural and relational mechanism embedded within institutional routines, leadership practices, and conflict-management responses. Pseudomonas aeruginosa poses a major clinical challenge due to its high levels of antibiotic resistance. This study evaluated the antibacterial activity of three medicinal plant extracts Allophylus africanus (leaves), Securidaca longepedunculata (root bark), and Sapium ellipticum (stem bark), against 30 drug-resistant P. aeruginosa isolates. Antibacterial screening was conducted using the agar well diffusion method, followed by determination of minimum inhibitory concentrations (MICs) via a 96-well microtiter plate assay. All three plant extracts demonstrated inhibitory activity against three resistant P. aeruginosa strains (gentamicin-resistant strains F4 and GR4RAU-AI, and ceftazidime-resistant strain 19-10), with inhibition zones of varying diameters. BACKGROUND: While cross-sectional assessments offer snapshots of care cascade progress, longitudinal analyses are essential to capture the dynamic engagement in care of people living with HIV (PLHIV). We assessed the advancement of the HIV cascade, engagement in care and factors associated with different transitions among PLHIV at the Chronic Disease Clinic of Ifakara, Ifakara, Tanzania, using both cross-sectional and longitudinal analyses. This study examines the spread and ecological impact of invasive Flowerhorn cichlids (Amphilophus spp.) in Lake Sampaloc, Philippines, driven by both accidental release and intentional aquaculture practices. Field observations and literature reveal that these highly adaptable and aggressive fish outcompete native species, contributing to biodiversity decline amid increasing pollution, eutrophication, and microplastic contamination. Their phenotypic plasticity allows them to thrive in degraded environments, further exacerbating ecological imbalance. While local communities have begun utilizing Flowerhorns for ornamental trade and potential aquafeed production, such practices risk reinforcing their proliferation. BACKGROUND AND OBJECTIVES: Quantitative flow ratio (QFR) offers a non-interventional alternative for evaluating ischemia, yet its concordance with real-world treatment and clinical outcomes remains unclear. This study aimed to assess the alignment between Murray law-based QFR (μQFR) physiological assessment and treatment strategies, evaluate the association between treatment concordance and clinical outcomes, and explore factors influencing concordance.
