Competency-based medical education (CBME) has reshaped physician training, yet system-level implementation in allied health professions is rarely described. In this Perspective, we examine Taiwan's decade-long national reform of medical radiation technologist (MRT) education led by the Taiwan Association of Medical Radiation Technologists (TAMRT) and distill lessons for system-level CBME. Beginning in 2012, TAMRT adapted the Accreditation Council for Graduate Medical Education (ACGME) core competencies and introduced workplace-based assessments across diagnostic radiography, radiation therapy, and nuclear medicine. Early national accreditation cycles revealed persistent variability in assessment quality and highlighted the limitations of time-based advancement. Electronic nicotine delivery system manufacturers, such as Charlie's Holdings Inc., ECBlend, Outlaw, and NicRiver, have recently introduced nicotine analogs, such as 6-methyl nicotine, 6-MN ("Metatine"), and nicotinamide, NA ("Nixamide," "Nixodine," or "Nixotin-free base and salt") in products to circumvent the U.S. FDA's premarket tobacco product application (PMTA) requirements. Marketed as "tobacco-free," "PMTA-exempt," or "FDA-approved," these compounds now appear in oral nicotine pouches and disposable bars/vapes from brands such as Outlaw Dip, Kumi-Six, SBX, Katchmi, and Spree Bar under proprietary labels including "NoNic6," "Metatine," or "NIC-SAFE." BACKGROUND: Previous studies on intraventricular hemorrhage (IVH), a common and severe complication of preterm birth, and subsequent post-hemorrhagic hydrocephalus (PHH), have predominantly concentrated on the secretory function of the choroid plexus (ChP), with considerably less emphasis on its barrier function. We hypothesized that PHH is associated with immune-related alterations in the junction biology of ChP. METHODS: We examined differences in tight junctions and macrophages using a neonatal mouse model of PHH (n = 40) and in vitro ChP explants (n = 22), as well as human post-mortem samples (n = 6). PURPOSE: This study qualitatively explored barriers and facilitators to accessing supportive vision services for childhood cancer survivors with vision deficits and identified current service-delivery contexts from the perspectives of community professionals and caregivers. METHODS: Participants included caregivers and professionals who completed semi-structured interviews. Provider questions focused on patient identification and assessment, follow-up protocols, and available resources, while caregiver questions addressed navigating survivorship care, academic needs, and goals for their children. Transcripts were thematically analyzed following inductive and deductive coding. RESULTS: Six caregivers and 10 professionals completed interviews. In the context of ongoing global climate change, the intensification of the hydrological cycle is expected to modify the frequency, magnitude, and spatial distribution of extreme precipitation events. Understanding how precipitation extremes will evolve under climate change is critical for East Africa (EA), a region highly vulnerable to hydroclimatic hazards and strongly dependent on rainfall-driven socioeconomic systems. The region experiences two main rainfall seasons, namely the long rains (March-May, MAM) and the short rains (October-December, OND). This study investigates future projections of the characteristics of extreme precipitation across the EA using an ensemble of biascorrected CMIP6 global climate models. Extreme …

Clinical learning is an important component in the formation of nursing students’ professional competence. The clinical environment, student motivation, and the role of clinical instructors are factors that influence the achievement of learning outcomes in nursing clinical practice. However, the relationship between these factors is still not fully understood. This study aims to examine the relationship between the clinical environment, student motivation, and the role of clinical instructors, and to determine how these three factors contribute to learning outcomes in nursing clinical practice. Biological threats present unique challenges to traditional deterrence models due to their dual-use nature, difficulties in attribution, and rapidly evolving scientific landscape. In this commentary, the authors argue that deterrence by denial, while essential, is insufficient on its own to address contemporary biothreats. They propose a broader framework that integrates deterrence by disruption and capability diminishment to interfere with and reduce adversaries’ ability to develop and employ biological weapons. This approach emphasizes coordinated efforts across scientific, technological, economic, intelligence, and policy domains, as well as collaboration with public health and international partners. Taiwan's declining fertility rate is reshaping the pediatric care landscape. The demands of child health care encompass acute and critical care, chronic disease management, complex diagnostic conditions, as well as developmental and mental health support. In response, the Ministry of Health and Welfare's Program for Excellence in Child Health Care and the Project of Holistic Physicians for Young Children represent a strategic transition toward proactive, community-anchored child health stewardship. By integrating preventive services, developmental surveillance, case management, and cross-sector coordination, these initiatives strengthen early detection and timely intervention while redefining pediatricians as key actors within the social safety net. Background: Despite increasing emphasis in medical education, LGBTQ clinical sensitivity and care competencies remain insufficient. This study developed a structured Standardized Patient (SP) training model replacing traditional lecturing. It aims to alleviate role boundary blurring and psychological stress SPs face with highly sensitive topics while enhancing their professional comprehension.
Objectives: To develop and evaluate a structured SP training model that integrates psychological safety mechanisms to mitigate role confusion and stress in handling gender diversity topics, while enhancing SPs' professional role interpretation and cultural sensitivity.

In the 13th century, under the leadership of Genghis Khan, the Mongol Empire emerged where alcohol was not merely a consumable item but a multifaceted and functional cultural practice integral to the construction of political legitimacy, the establishment of social order, and the performance of ritual representations. For the nomadic Mongols, alcohol consumption transcended personal habit, playing a central role in regulating social relations, strengthening leadership, and consolidating political alliances. Alcohol presentations during ceremonial events such as kurultais, enthronement ceremonies, diplomatic meetings, military victory celebrations, and festivals were regarded as symbolic indicators of generosity, loyalty, and hierarchical order. Hypertension is one of the non-communicable diseases in Indonesia that is closely related to obesity. Therefore, this study aimed to determine the association between central obesity and hypertension in a cohort study group in Bogor City, West Java from 2011 to 2021. A retrospective cohort study design was employed, with multivariate Cox regression analysis performed using secondary data from the Risk Factors for Non-Communicable Diseases (FRPTM) Cohort Study. Among 3,586 samples, 879 incident cases of hypertension were observed by the end of follow-up, accounting for 24.5% of the at-risk population. Mitchell and colleagues (Reference Mitchell, Haslam, Burke and Steffense2026) provide a well-reasoned, sound, and thought-provoking case for why HR practices can promote or discourage narcissistic tendencies in leaders. Acknowledging their contribution, we offer two points of pushback and one point of extension based on those areas of pushback. The first point of pushback is that counter to the authors, we contend that hypercompetitive selection methods that seek to identify the best leadership candidate, although not ideal, are necessary. BACKGROUND: There is consensus that research participants should be informed about plans for return of genetic research results. However, best practices for return of results in large biobank and cohort studies do not exist currently, and how best to communicate actionable genetic research results remains unclear. While having genetic counselors disclose these results may be ideal to ensure understanding, minimize distress, and optimize medical follow-up, genetic counselor (GC) workforce shortages and costs are barriers. Background: Emerging evidence suggests an association between vascular risk factors and changes in brain cortical structure and function. However, the relationship between venous thromboembolism (VTE) and brain cortical remodeling remains unclear, impeding further research on brain cortical dysfunction. Falls are a leading cause of injury among older adults, often resulting in significant physical, psychological, and social consequences. This study aims to determine the factors that contribute significantly to fall risks among the older adults in rural Sabah. A cross-sectional community study was conducted in the Kudat Division, Sabah, Malaysia from January to June 2021. Using multistage random sampling, 700 adults aged ≥60 years were recruited, excluding those with severe disability, mental illness, or institutionalized. During the COVID-19 pandemic, the number of early marriages increased. Early marriage causes biological and psychological problems in the mother and her baby. The combination of psychoeducation and mind mapping emerged as a preventive effort against early marriage. Mechanisms underlying breast cancer brain metastasis (BCBM) are still not well understood. Here, we identified that BCBM patient serum contained extracellular vesicles (EVs) with high levels of microRNAs (miRNAs)-107 and -425. Levels of miR-107 and miR-425 were elevated in brain metastases, and the elevation was associated with poor patient prognoses. Ectopic expression of miR-107 and miR-425 promoted mammospheres; however, the inhibition of miR-425, but not miR-107, suppressed breast cancer mammosphere formation. We further observed that EVs from miR-425-overexpressing breast cancer cells strongly activated astrocytes whereas their inhibitors abrogated the effect. This descriptive-phenomenological study examined the role of mentorship in shaping identity. There is limited literature that examines mentorship as an identity-shaping process among Black queer men (BQM). Intersectional and Black queer theory provided the theoretical framework for thematic analysis.
Within the workplace, BQM must negotiate the effects of double marginalization resulting from racism and anti-queer biases. The effects of racism and anti-queer biases can lead to adverse psychological and physical health outcomes, negatively impacting the careers of Black and queer individuals. Tetralogy of Fallot (TOF) is a congenital heart defect that causes chronic low oxygen levels, increased right ventricular workload, and a higher risk of complications. Weaning mechanical ventilation in patients with TOF is particularly challenging due to right ventricular dysfunction, muscle weakness, and the risk of heart failure. This study evaluates the use of a spontaneous breathing trial (SBT) strategy with pressure support ventilation (PSV) mode for mechanical ventilation weaning in pediatric patients with TOF.
