Introduction: The sociodemographic and clinical factors play a role in type 2 Diabetes Mellitus (T2DM) glycemic control.
Objectives: To assess glycemic control among T2DM patients and its association with sociodemographic and clinical factors.
Materials and methods: A prospective observational study of 210 patients with T2DM at a tertiary diabetes and endocrinology center in Sulaymania city/ Iraq, from April to November 2024. High-frequency stimulation (HFS) is a human surrogate model of secondary hyperalgesia and a key experimental tool for understanding the mechanisms and modulation of central nociceptive pathways. An emerging area of research focuses on the role of top-down endogenous analgesic systems during secondary hyperalgesia development. However, the test-retest reliability of the early temporal changes in sensitivity are poorly understood. BACKGROUND: Sickle cell disease (SCD) encompasses a spectrum of genetic hematologic disorders characterized by erythrocyte sickling. Patients may experience musculoskeletal manifestations, such as osteonecrosis, which may warrant consideration for total shoulder arthroplasty (TSA). While prior literature suggests patients with SCD more frequently suffer complications after orthopaedic procedures, this is not well characterized for TSA.
METHODS: Adult TSA patients (older than 18 years) were identified in the 2010 to 2023 PearlDiver M170 database. Patients with SCD were matched 1:4 to patients without SCD based on age, sex, and Elixhauser Comorbidity Index (ECI). INTRODUCTION: Isotretinoin (ITN) is widely prescribed for severe acne in adolescents and young adults, a population where anterior cruciate ligament reconstruction (ACLR) is commonly performed. Long-standing concerns about impaired wound healing have prompted many surgeons to delay procedures, despite limited orthopaedic-specific evidence. This retrospective case series evaluates early postoperative clinical complications in patients with documented ITN exposure undergoing ACLR.

This period of time was marked by elevated uncertainty and fear among individuals. COVID-19 contributed to deterioration in not only physical health but also psychological well-being. One mental health condition frequently under-examined and saturated with misconceptions is obsessive-compulsive disorder. 4-H Builds Work Readiness Skills In Youth, Mike Knutz

4-H Programs provide work readiness skills like teamwork, respect, leadership, critical thinking, work ethics, and communication. Supported studies are highlighted as well as results from an evaluation of youth participants from a Know Your Government statewide program. Data was collected from middle and high school age youth to measure their self-reported increase in work readiness skills as well as knowledge of government. Results are discussed in terms of their relevance to life skill theory and measurement. The study supports the value of 4-H Youth Development Programs as a means for youth to acquire work readiness skills.

The Teaching Personal and Social Responsibility (TPSR) model has been identified as an evidence-based structure that can support social-emotional learning (SEL) competencies in after-school settings. However, much of the research includes general physical fitness and skill activity-based programs, and not activities students identify with as meaningful in their lives. Evaluation of personality and psychological disorders is accomplished through the use of psychometric measures. However, tests require interpretation of test responses using a normative criterion based on a particular population that may not fully consider the effect of cultural influences on an individual’s level of willingness to report psychological difficulties. For example, individuals in collectivistic cultures may not be as willing to self-report problems because mental illness is often perceived as a source of embarrassment and shame. INTRODUCTION: Disabled occupational therapy students face systemic barriers in clinical education. Although legal mandates require reasonable accommodations, implementation is often inconsistent, and few standardized guidelines exist for fieldwork education. This study is aimed at establishing best practices for supporting occupational therapy and occupational therapy assistant students with disabilities during fieldwork.
MATERIALS AND METHODS: A three-round Delphi study was conducted with a panel of 30 experts, including disabled occupational therapy students, disabled occupational therapy practitioners, academic fieldwork coordinators (AFWCs), fieldwork educators (FWEs), disability services staff, and researchers. This report analyzed firearm-related deaths rates in the United States at the state level over a two decade period between 2000 to 2019. The study used a state-by-year analytical framework created from the U.S. Gun Deaths by County, 1999–2019 dataset (Eltom, 2019) in order to examine national trends, state-level differences, changes over time, geographic concentration, and regional variation. The analysis focused on firearm-related deaths by adjusting for population size differences (i.e., the crude firearm-related death rate) across U.S. states from 2000 to 2019. Major depressive disorder is prevalent, chronic, and heterogeneous, including cognitive- affective (e.g., maladaptive beliefs or thinking patterns and sad mood) and somatic (e.g., fatigue, appetite, or sleep changes) symptom domains. While depression domains have been long theorized, research examining psychological mechanisms underlying this distinction is limited. Preliminary research suggests cognitive-affective symptoms relate more strongly to cognitive mechanisms (cognitive bias, perseverative thought), somatic symptoms relate more strongly to behavioral mechanisms (behavioral inhibition/activation), and gender, race, and ethnicity moderate these relationships. Political rhetoric surrounding key events has seen sharp increases in emotionally affective speech, intergroup sentiment, and rhetoric unbecoming of democratic societies in recent years. Similarly, semi-democratic and ideologically driven speech has become increasingly normalized within an era of political polarization and hardline beliefs in modern media. Within this economy of outrage, sensational and largely unmoderated media formats, particularly podcasts, have become a mainstay of political information and discourse. Cranial nerves provide afferent pathways that enable bottom-up modulation of central autonomic and cognitive neural pathways. Because these pathways project to brainstem nuclei such as the nucleus tractus solitarius and engage neuromodulatory systems, including the locus coeruleus, electrical stimulation offers a mechanistically grounded approach to influencing brain function. This thesis tested whether active transcutaneous auricular vagus nerve stimulation (taVNS) produces physiological responses indicative of parasympathetic engagement, defined as decreases in heart rate, increases in heart rate variability, and decreases in respiration relative to a dose-matched earlobe control condition. This paper analyzes the influence of financial development on the implementation of renewable energy in BRICS+ nations, addressing both short-term and long-term implications. The empirical research utilizes Autoregressive Distributed Lag (ARDL) and Nonlinear ARDL models, analyzing data from 1992 to 2021 for ten BRICS+ nations, while accounting for essential economic variables such as Foreign Direct Investment and inflation rate. β-Lactam antibiotics constitute one of the most extensively prescribed classes of antimicrobial agents in human and veterinary medicine. However, their widespread use has raised growing concerns regarding antibiotic resistance, drug safety, and the presence of residual antibiotics in pharmaceutical products, biological fluids, foodstuffs, and environmental samples. This memorandum discusses the possible collateral damage that Propositions 41 and 42 (on the ballot in California on November 3, 2026) might cause. The two propositions were concocted very quickly by billionaires in order to trick voters that will vote for Proposition 40 (the billionaire tax) also to vote for these propositions that are designed to undermine the billionaire tax.
The basic trick is that these propositions have plausible seeming veneers but hide a poison pill that aims to frustrate the functioning of the billionaire tax. The veneers are “plausible seeming” because they address problems that are not problems. For …

Estrogen receptor-positive (ER+) breast cancer exhibits a marked propensity for skeletal metastasis; however, the molecular drivers of bone colonization remain incompletely defined. We investigated the tumor suppressor neurofibromin (NF1), a dual repressor for RAS and ER signaling, whose inactivation promotes endocrine therapy (ET) resistance and is associated with inferior relapse-free survival. NF1 copy number loss was detected in 62% of ER+ patients who subsequently developed metastases and was associated with an increased likelihood of bone metastases at initial diagnosis. In mouse xenograft models, NF1-depleted ER+ breast cancer cells demonstrated enhanced dissemination to skeletal sites following surgical resection of primary tumors. …

To compare hypertension prevalence using 'initial' blood pressure (BP), 'subsequent' BP (averaged second and third readings) and 'total' BP (averaged three readings), we included 8-17-year-old youth with three BP readings from NHANES 2013-2020. From 2013-2016, BP was obtained using auscultation (manual protocol, MP) and subsequently, using oscillometry (automated protocol, AP). We excluded subjects with 'zero' diastolic blood pressure (DBP). In 5656 subjects, clinical hypertension (high systolic blood pressure [SBP] and/or DBP) prevalence was highest with 'initial' BP (350/5 642, 6.2% [95% CI 5.6-6.9%]), lower with 'subsequent' BP (289/5 642, 5.1% [4.6-5.7%], p < 0.001), and lowest with 'total' BP (251/5 642, 4.4% [3.9-5.0%], p < 0.001). Both MP and AP groups maintained this trend with a significantly lower hypertension prevalence with 'total' vs. 'subsequent' BP. An exception to the trend occurred in the AP group for three conditions: diastolic hypertension, hypertension in high BMI and Hispanic subjects. Diastolic hypertension prevalence with 'subsequent' DBP (86/2 285, 3.8% [3.1-4.6%]) exceeded that with 'initial' DBP (83/2 285, 3.6% [2.9-4.5%]) and was lowest with 'total' DBP (59/2 285, 2.6% [2.0-3.3%]). Diastolic hypertension prevalence was higher (126/5 642, 2.2% [1.9 - 2.7%]) with the 2016 European guidelines (same as 'subsequent' DBP) than with 'total' DBP (94/5 642, 1.7% [1.4 - 2.0%]). Our findings support the inclusion of the first BP reading in BP categorization, as 'total BP' leads to lower hypertension classification than 'subsequent' BP.

The spinal cord plays a central role in the pathophysiology and clinical manifestations of multiple sclerosis (MS), yet remains under-studied compared with the brain. This review summarizes key insights from the 2025 North American Imaging in MS Spinal Cord Imaging Workshop, highlighting recent advances, ongoing challenges, and future opportunities in MS spinal cord imaging. We review pathological studies and outline the clinical relevance of spinal cord lesions and atrophy for diagnosis, prognosis, and disease monitoring, highlighting emerging biomarkers of progression independent of relapse activity. To develop a mutual information (MI)–based mechanism for quantifying representation distance, and to introduce a constrained multiview learning paradigm that dynamically re-ranks and selects sample views, thereby improving contrastive representation learning for lung lesion segmentation on CT images—addressing the difficulty of measuring distances in high-dimensional feature spaces and the impracticality of constructing large positive–negative sample banks in the medical domain.Materials and Methods

Introduction: Midlife cardiovascular burden contributes to cognitive decline and dementia. However, research in diverse populations remains scarce.
Methods: We included 402 San Antonio Heart Study participants (1979–1988; midlife mean age 57.9 ± 3.8, 58.5% women, 52% Mexican American) later enrolled in the San Antonio Longitudinal Study of Aging (1992–2004). Midlife cardiovascular health was assessed using Life's Simple 7. Cognition was measured up to four times with the Mini-Mental State Examination. Generalized estimating equation models adjusted for age, sex, income, and education were used to relate cardiovascular factors to cognitive change. Sensitivity analyses evaluated attrition bias.

Women’s entrepreneurship represents an important source of economic participation, job creation, and local development in Kosovo. Despite gradual progress, women entrepreneurs continue to face structural constraints linked to family responsibilities, unequal care burdens, and uneven institutional support. This study examines how family-related responsibilities and institutional support mechanisms interact to shape the entrepreneurial experiences of women entrepreneurs in Kosovo. The analysis is guided by the 5M framework of women’s entrepreneurship developed by Brush et al. (2009) and applies a mixed-methods research design combining survey data from 145 women-owned businesses, semi-structured interviews, and secondary evidence.
