BACKGROUND: Altered mental status is a common chief complaint in the emergency setting, and the initial evaluation often fails to find an etiology. Subclinical (electrographic) seizures and nonconvulsive status epilepticus (NCSE) are potential causes of delirium and altered mental status, but require electroencephalogram (EEG) for diagnosis. It is not clear how often subclinical seizure or NCSE is present in those with undifferentiated altered mental status, nor which patients are most likely to benefit from emergent EEG. Introduction: Geographic disparities in Alzheimer’s disease and related dementias (ADRDs) remain insufficiently understood at the national scale, limiting efforts to address inequities in dementia burden across USA communities. This study examined county-level variation in ADRD prevalence and mortality and evaluates the association between social vulnerability and these outcomes.
Methods: This cross-sectional ecological study analyzes 2020 county-level ADRD prevalence and all-cause mortality among Medicare beneficiaries from the NORC Dementia DataHub. The analytic sample included 3,108 USA counties in the contiguous USA and the District of Columbia. This study investigated factors influencing Intercollegiate Horse Show Association (IHSA) LGBTQ+ riders’ perceptions of inclusion, safety, and belonging, with the hypothesis that teams with diversity and inclusion policies would be perceived as more accepting. An online survey was developed to gather information from self-identified LGBTQ+ IHSA students. Questions covered demographics, team environment, and perceived inclusivity within their equestrian programs. Question types included multiple choice, Likert-type, short answer, and open-ended. IHSA coaches were asked to share the survey link with team members. Students were able to respond anonymously. The survey was open for 8 weeks. Reminders were sent out at 3 …

Purpose: Although some studies suggest articulatory cues may help enhance early literacy outcomes, research has yet to isolate their specific contribution to the gains. Framed by the motor theory of speech perception (Liberman & Mattingly, 1985), this study seeks to fill this gap by providing phonemic segmentation training using articulatory cues and comparing reading/spelling outcomes to two different comparison groups to attempt to control for the motivation and attention the articulatory cues may provide.

The persistent nursing shortage across the United States, particularly in Long-Term Care (LTC) settings, has complicated the healthcare system’s ability to meet the needs of an aging population. While existing research often examines progression from Licensed Practical Nurse (LPN) to Registered Nurse (RN), limited research explores factors influencing progression from entry-level LTC healthcare workers to Practical Nursing (PN) licensure. This quantitative descriptive study examined the barriers and supporting factors influencing advancement toward PN licensure for Medication Assistants-Certified (MA-Cs) in Arkansas LTC facilities. Chronic aphasia following a left middle cerebral artery (L-MCA) stroke is often associated with limited recovery beyond the acute phase, necessitating interventions that target neuroplastic mechanisms. This study examined the effects of somatosensory pneumotactile stimulation paired with cognitive-linguistic tasks on functional language performance in an individual with chronic aphasia. Using a single-subject pre-post design, the participant completed a 10-week intervention protocol involving repeated pneumotactile stimulation via the GALILEO system combined with language and motor tasks. Outcomes were assessed using standardized measures, discourse-based language samples, oral reading, repetition, and phonemic verbal fluency. The focus of this dissertation involves the effects of static ankle stretching on balance and gait in adults of different ages and with Parkinson’s disease (PD). A brief introduction in Chapter 1 precedes a detailed literature review in Chapter 2, in which we identify gaps in research on balance and gait following ankle stretching and explain why ankle stretching may serve as a predominant strategy for improving motor performance in aging and PD populations. The Supreme Court in New York State Rifle & Pistol Association v. Bruen introduced a history-and-tradition test for Second Amendment challenges, directing courts to compare modern firearm regulations to past ones. To conduct this historical inquiry, litigants and judges have increasingly turned to professional historians as expert witnesses. This Note qualitatively examines how historian experts are used and received in post-Bruen federal litigation. It finds that a small group of repeat players—sixteen historians—make up this emerging field, typically appearing exclusively for either challengers or governments. These historians serve primarily in civil litigation, leaving a relative expertise gap in criminal cases, …

BACKGROUND: Advanced machine perfusion techniques have increased the use of hard-to-place organs, effectively changing the liver transplantation landscape. Liver transplantation machine perfusion has been driven largely by industry or individual transplant centers. We describe the implementation of a portable, organ procurement organization-initiated hypothermic oxygenated machine perfusion strategy for liver transplantation as a proof of concept.
METHODS: Surgeons and staff at a regional organ procurement organization (Indiana Donor Network) were trained to use and cannulate liver allografts using a portable hypothermic oxygenated machine perfusion platform. Urban heat risk is increasing, while fixed monitoring networks remain too sparse and coarse to resolve the pedestrian-scale variability, especially radiative loads, that governs outdoor thermal stress. This short communication advances the concept of climate walks, defined as route-based, human-centred field campaigns that build on earlier work on “thermal walks”, and presents them as a practice-ready methodology for design-relevant evidence. We define climate walks as structured, route-based, georeferenced assessments that pair high-resolution mobile microclimate measurements with synchronous in-situ human responses to capture transient, spatially heterogeneous conditions along actual walks. Goal-Directed Perfusion Strategies And Postoperative Acute Kidney Injury: A Scoping Review, Austin Davis

Background: Acute kidney injury (AKI) remains a frequent and clinically important complication following cardiopulmonary bypass (CPB). Goal-directed perfusion (GDP) strategies—most commonly targeting indexed oxygen delivery (DO₂i) and/or other hemodynamic and metabolic parameters—have been proposed to reduce renal injury by improving renal oxygenation and perfusion during CPB.
Methods: A Preferred Reporting Items for Systematic Reviews and Meta-Analyses – Scoping Review (PRISMA-ScR) compliant scoping review identified 28 adult CPB studies evaluating GDP strategies and postoperative AKI outcomes.
Results: Maintenance of indexed oxygen delivery (DO2i) above critical thresholds and minimizing hemodilution were consistently associated with reduced AKI.

The problem that was addressed through this study is that preschool special educators are not consistently integrating role-play strategies when teaching language development to children with autism spectrum disorder (ASD) at ABC district (a pseudonym), a district in the southeastern United States. The absence of formalized governance, measurement systems, and standardized processes in nonprofit organizations can limit performance, accountability, and long-term sustainability. Nonprofit executive leaders and board members are concerned about fulfilling their responsibilities for organizational effectiveness and community social effectiveness. Primary caregivers of teenage violent offenders play a vital role in identifying and addressing safety concerns, providing emotional and financial support, and meeting individual developmental and psychosocial needs. As more grandparents assume the role of primary caregiver, the need for targeted interventions to support their emotional and psychosocial health continues to grow, emphasizing the increasing importance of population-specific support for grandparents caring for their grandchildren. Problems arise when grandparents take on this role without access to interventions or resources that are designed for caring for adjudicated teen violent offenders.
