Journal of Patient-Centered Research and Reviews
At the peak of the 2021 wave of the SARS-CoV-2 alpha variant in North America, there was concern for a superimposed wave of viral respiratory infections. There was, however, an apparent shift in the usual epidemiology of these pathogens, especially during the traditional influenza season from approximately October 2020 to March 2021. This article seeks to briefly describe the epidemiology of notable respiratory pathogens during the first wave of the COVID-19 pandemic and to focus on one possible factor for the trends observed. There are many contributory elements to the observed viral trends, but in particular, we present a synopsis …

The History Of Geriatric Emergency Medicine, Teresita M. Hogan Md, Lowell Gerson Phd, Aurthur B. Sanders Md
Journal of Geriatric Emergency Medicine
Abstract: Excellent emergency care does not happen by chance. The standard emergency approach that excels in the young, fails in older patients. Older adults experience unnecessary morbidity and excess mortality in our emergency departments. This article describes the pursuit of excellent emergency care in the historically challenging older adult population. A pivotal point occurred once emergency physicians recognized older patients as a distinct population in need of unique evaluation and treatment.
In the early 1990s a group of geriatricians, philanthropists, and emergency physicians joined forces to improve older patient care. Geriatric Emergency Medicine (GEM) emerged as a subspecialty as these …

An Industry Study Of Best Practices In Life Insurance Automated And Accelerated Underwriting Models, Kartik Sakthivel
Theses and Dissertations
Having life insurance is critical for hundreds of millions of people around the world as a key part of sound financial protection for individuals and families, but there are unacceptably high populations that lack coverage. 106 million Americans are uninsured or underinsured, the protection gap being pronounced in communities of color. Making the rigorous process of purchasing insurance easier is crucial in helping protect the financial security of millions. Automated and accelerated underwriting, a relatively nascent practice in the mature life insurance industry, is intended to streamline this process and narrow this coverage gap by making it easier to obtain …

Proinflammatory agonists provoke the expression of cell surface adhesion molecules on endothelium in order to facilitate leukocyte infiltration into tissues. Rigorous control over this process is important to prevent unwanted inflammation and organ damage. Protein L-isoaspartyl O-methyltransferase (PIMT) converts isoaspartyl residues to conventional methylated forms in cells undergoing stress-induced protein damage. The purpose of this study was to determine the role of PIMT in vascular homeostasis. PIMT is abundantly expressed in mouse lung endothelium and PIMT deficiency in mice exacerbated pulmonary inflammation and vascular leakage to LPS(lipopolysaccharide). Furthermore, we found that PIMT inhibited LPS-induced toll-like receptor signaling through its interaction …

BACKGROUND: While immune-cell infiltrated tumors, such as human papillomavirus positive (HPV+) ororpharyngeal squamous cell carcinomas (OPSCC) have been associated with an improved clinical prognosis, there is evidence to suggest that OPSCCs are also subjected to increased immunoregulatory influence. The objective of this study was to assess whether patients with clinically aggressive OPSCC have a distinct immunosuppressive immune signature in the primary tumor.
METHODS: This retrospective case-control study analyzed 37 pre-treatment tissue samples from HPV+ and HPV-negative OPSCC patients treated at a single institution. The cases were patients with known disease recurrence and the controls were patients without disease recurrence. An …

Bromo- and extra-terminal domain inhibitors (BETi) have exhibited therapeutic activities in many cancers. However, the mechanisms controlling BETi response and resistance are not well understood. We conducted genome-wide loss-of-function CRISPR screens using BETi-treated KMT2A-rearranged (KMT2A-r) cell lines. We revealed that Speckle-type POZ protein (SPOP) gene (Speckle Type BTB/POZ Protein) deficiency caused significant BETi resistance, which was further validated in cell lines and xenograft models. Proteomics analysis and a kinase-vulnerability CRISPR screen indicated that cells treated with BETi are sensitive to GSK3 perturbation. Pharmaceutical inhibition of GSK3 reversed the BETi-resistance phenotype. Based on this observation, a combination therapy regimen inhibiting both …

The Great Debate session at the 2022 Melanoma Bridge congress (December 1-3) featured counterpoint views from leading experts on five contemporary topics of debate in the management of melanoma. The debates considered the choice of anti-lymphocyte-activation gene (LAG)-3 therapy or ipilimumab in combination with anti-programmed death (PD)-1 therapy, whether anti-PD-1 monotherapy is still acceptable as a comparator arm in clinical trials, whether adjuvant treatment of melanoma is still a useful treatment option, the role of adjuvant therapy in stage II melanoma, what role surgery will continue to have in the treatment of melanoma. As is customary in the Melanoma Bridge …

(1) Background: The correlations between brain connectivity abnormality and psychiatric disorders have been continuously investigated and progressively recognized. Brain connectivity signatures are becoming exceedingly useful for identifying patients, monitoring mental health disorders, and treatment. By using electroencephalography (EEG)-based cortical source localization along with energy landscape analysis techniques, we can statistically analyze transcranial magnetic stimulation (TMS)-invoked EEG signals, for obtaining connectivity among different brain regions at a high spatiotemporal resolution. (2) Methods: In this study, we analyze EEG-based source localized alpha wave activity in response to TMS administered to three locations, namely, the left motor cortex (49 subjects), left prefrontal cortex …

BACKGROUND: Radiocarpal dislocations are rare but potentially devastating injuries. Poorer outcomes are associated with inadequate or lost reduction, such as ulnar translocation, but no consensus exists on the ideal fixation technique. Dorsal bridge plate fixation has been described for various settings in the treatment of complex distal radius fractures and can be fixed distally to the second or third metacarpal, but its application for radiocarpal dislocations has not been established.
AIM: To determine whether distal fixation to the second or third metacarpal matters.
METHODS: Using a cadaveric radiocarpal dislocation model, the effect of distal fixation was studied in two stages: …

Herpes zoster (HZ; shingles) caused by varicella zoster virus reactivation increases stroke risk for up to 1 year after HZ. The underlying mechanisms are unclear, however, the development of stroke distant from the site of zoster (eg, thoracic, lumbar, sacral) that can occur months after resolution of rash points to a long-lasting, virus-induced soluble factor (or factors) that can trigger thrombosis and/or vasculitis. Herein, we investigated the content and contributions of circulating plasma exosomes from HZ and non-HZ patient samples. Compared with non-HZ exosomes, HZ exosomes (1) contained proteins conferring a prothrombotic state to recipient cells and (2) activated platelets …

Climate change is expected to extend the growing season for trees in northern latitudes, potentially leading to a northern migration of species. However, the evidence of such migration is largely limited to indirect modeling methods, due to the lack of empirical data. FIA data of USDA provide an opportunity to empirically examine the signatures of species migration across United States (US). If the northern migration proves to be true, it is expected that sapling population will be further north than adult populations in both leading and trailing edge of the species range. This study aims to assess tree species migration …

As the Internet has increasingly transformed patients’ engagement with health information, scholars in the rhetoric of health and medicine have shifted their attention to understanding how everyday health practices rhetorically shape the complex identities of patients, as well as the decisions they make in healthcare settings (Arduser, 2011; Bivens et al., 2018; Bellwoar, 2012; Hensley Owens, 2011; DeHertogh, 2015; Segal, 2009). This scholarship complicates traditional notions of agency, expertise, and empowerment in writing studies by effectively demonstrating how patients develop health practices that are embodied, technological, and rhetorical; individuals often navigate varying levels of complex information from both institutional and …
Accurate Pulse Assessment By Radial Artery Palpation: A Pilot Study, Abigail Mathis
Accurate Pulse Assessment By Radial Artery Palpation: A Pilot Study, Abigail Mathis
Honors Projects
Pulse assessment is an essential component of an individual’s health state. Assessment of a pulse by palpation is the most commonly used technique. A patient’s pulse rate informs healthcare workers of the status of a variety of body systems, including cardiac, neurologic, and endocrine functions. Despite the widespread and frequent use of pulse assessment, current methods for counting a pulse lack strong supporting evidence. Additionally, published research on accuracy of pulse palpation used only young, healthy participants without arrhythmias or cardiac problems, limiting generalizability to the broader population who contain those variables. The purpose of this study is to determine …

A Canadian Perspective On Fifty Years Of International Economic Law, J. Anthony Van Duzer
Dalhousie Law Journal
In 1970, “international economic law” (IEL) was not a distinct academic subject. Fifty years later, IEL has become an important and well-recognized field of legal enquiry, though its boundaries remain unclear. Globalization of trade and investment activity and the concomitant proliferation of trade and investment treaties over the last 50 years have been key drivers of academic interest in IEL and its transformation. The impacts of trade and investment on the protection of the environment and health, Indigenous, labour, and human rights, development, and other policy priorities have become significant subjects of academic discourse and are increasingly addressed in trade …

Police-Generated Evidence In Bail Hearings: Generating Criminality And Mass Pretrial Incarceration In Canada, Jillian Rogin
Dalhousie Law Journal
Systemic racism in policing impacts many aspects of the criminal legal system including the system of judicial interim release. This paper traces the ways in which reliance on police-created evidence at bail hearings might contribute to mass pretrial incarceration in Canada which is disproportionately felt by Indigenous, Black, and marginalized people. The police synopsis and police-created criminal records are state knowledge created for state purposes. This state-created evidence in fact generates race and racialization; all of the structural inequalities built into the system of policing become relied on at bail hearings through police-created evidence which contributes to mass pretrial incarceration …

To explore the process through which people develop knowledge about socioecological change, this article describes a mixed-methods toolkit containing a technique for making maps in real time while moving through landscapes. The quantitative component of the toolkit is grounded in ethnobiologists’ embeddedness in place-based communities and harnesses the power of global positioning systems (GPS). As GPS-wielding ethnobiologists engage in participatory mapping by moving through landscapes with their research collaborators, we can use handheld devices and simultaneously communicate with satellites in outer space to produce maps in real time. Within the existing, large inventory of ethnobiological methods, using handheld GPS devices …

Taxidermy, despite its association with the bizarre and outlandish, has a rich history and culture that helped to define post-Civil War America and its pursuit of knowledge and reconnection with nature. With the widespread publication and availability of how-to guides, natural history collecting and taxidermy became accessible to any individual regardless of age, gender, or class. The hobby required physicality and courage to collect unique and interesting specimens, and intellect and creativity to conserve and display them, all of which inherently connected the avocation with respect. With varying levels of success, hobbyists experimented with different chemicals, such as arsenic, in …

Group B Streptococcus (GBS) is a pervasive neonatal pathogen accounting for a combined half a million deaths and stillbirths annually. The most common source of fetal or neonatal GBS exposure is the maternal microbiota. GBS asymptomatically colonizes the gastrointestinal and vaginal mucosa of 1 in 5 individuals globally, although its precise role in these niches is not well understood. To prevent vertical transmission, broad-spectrum antibiotics are administered to GBS-positive mothers during labor in many countries. Although antibiotics have significantly reduced GBS early-onset neonatal disease, there are several unintended consequences, including an altered neonatal microbiota and increased risk for other microbial …

We present PepQuery2, which leverages a new tandem mass spectrometry (MS/MS) data indexing approach to enable ultrafast, targeted identification of novel and known peptides in any local or publicly available MS proteomics datasets. The stand-alone version of PepQuery2 allows directly searching more than one billion indexed MS/MS spectra in the PepQueryDB or any public datasets from PRIDE, MassIVE, iProX, or jPOSTrepo, whereas the web version enables users to search datasets in PepQueryDB with a user-friendly interface. We demonstrate the utilities of PepQuery2 in a wide range of applications including detecting proteomic evidence for genomically predicted novel peptides, validating novel and …

The Reality Of Teaching English Virtually: Esl Teachers' Perspectives And Experiences During The Covid-19 National Pandemic, Natalia Guerrero
LSU Doctoral Dissertations
This study examined the dilemma ESL teachers experienced as the educational system shifted from the usual modus operandi of in-person lessons to the uncharted virtual learning environment (VLE). ESL teachers, in one of the largest urban districts in Louisiana, accumulated additional roles and responsibilities that were unique to the teachers of the English learner (EL) population enrolled at their schools.
Data collected to answer the research questions were the product of single and focus group’s interviews with five ESL elementary and middle school teachers in Freedom District. State and district emergency response to COVID-19 guidelines, along with instructional artifacts, were …
