Abstract
Metabolic and bariatric surgery (MBS) is an increasingly common treatment option for individuals with severe obesity, but utilization disparities remain with race and ethnic minority groups completing the procedure less frequently than non-Hispanic Whites. We examined the trends in MBS procedure types and prevalence of utilization by race and ethnicity among Florida inpatients.
Discharge records with any MBS using the International Classification of Diseases 9th or 10th edition Procedure Coding System (ICD-PCS), were extracted from the Florida Agency for Health Care Administration (AHCA) inpatients’ data (2006 to 2017). INTRODUCTION: Bleeding, especially non-variceal upper gastrointestinal bleeding (NVUGIB), remains the most common cause of readmission in left ventricular assist device (LVAD) patients. Any readmission after NVUGIB carries a worse prognosis.
AIM: To compare readmission outcomes in NVUGIB patients with and without LVAD.
MATERIAL AND METHODS: We identified adult NVUGIB patients using the National Readmission Database 2018 employing International Classification of Diseases, Tenth Revision (ICD-10) codes. To facilitate decision-makers to grasp the technical level and environmental influence of different in-situ thermal recycling units during construction, this paper proposed an evaluation method for in-situ thermal recycling and clean construction of asphalt pavement based on the game theory combination weighting and matter-element extension model. Firstly, based on the construction technology level and environment, a comprehensive analysis of in-situ thermal recycling construction was conducted, with the clean construction concept proposed. To efficiently and accurately identify the moving load parameters on bridge structures, this paper proposed a layered identification method for moving loads on bridges using BP neural networks based on the whale optimization algorithm (WOA), and built a moving load parameter identification model based on WOA-BP neural networks. By adopting this moving load identification model, step-by-step coupling identification was conducted on the action lane, speed, and load capacity of vehicles on the bridge. To detect structural abnormalities timely and ensure the safe operation of bridges, this paper took the health monitoring system of Xinguang Bridge in Guangzhou as the engineering background, and conducted research on a comprehensive multi-indicator early warning system for large-span steel arch bridges. Meanwhile, it selected multiple warning indicators from the categories of environmental effects and structural responses, set early warning indicator thresholds based on the distribution pattern of measured data, and formulated scoring standards for early warning indicators to score each early warning indicator. To study the quality disease and residual mechanical performance of bearings under the replacement of in-service bridges, this paper took the pot bearings under the replacement of a real bridge as an example to conduct appearance, mechanical perfornance, and raw material testing. Meanwhile, it determined the main causes of the disease led to by the replacement, analyzed the influence of bearing disease on the restrained concrete, and studied the performance improvement technology of in-service old bearings, with mechanical performance verification carried out. This study introduces a pioneering structural equation modeling (SEM)-based framework to assess BIM-DT adoption readiness in sustainable construction. The research’s approach delves into the intricate correlation between key success factors (KSFs) and sustainable parameters (SPs), fostering BIM-DT deployment. This interdisciplinary perspective provides a holistic understanding of the impact of KSFs on BIM-DT adoption. The research aims to identify KSFs and parameters, prioritize their relative weights for BIM-DT implementation, analyze KSFs and SPs correlations, and offer practical insights into the findings. The results revealed the importance of sustainability considerations in BIM-DT adoption, with cost optimization and resource management playing pivotal roles. …

Tobacco use is a major risk factor for many diseases and is heavily influenced by environmental factors with significant underlying genetic contributions. Here, we evaluated the predictive performance, risk stratification, and potential systemic health effects of tobacco use disorder (TUD) predisposing germline variants using a European- ancestry-derived polygenic score (PGS) in 24,202 participants from the multi-ancestry, hospital-based UCLA ATLAS biobank. Among genetically inferred ancestry groups (GIAs), TUD-PGS was significantly associated with TUD in European American (EA) (OR: 1.20, CI: [1.16, 1.24]), Hispanic/Latin American (HL) (OR:1.19, CI: [1.11, 1.28]), and East Asian American (EAA) (OR: 1.18, CI: [1.06, 1.31]) GIAs but …

In a 3-arm presurgical trial, four-six weeks exemestane 25 mg three times/week (TIW) was non-inferior to 25 mg/day (QD) in suppressing circulating estradiol in postmenopausal women with ER-positive breast cancer. Since obesity may decrease exemestane efficacy, we analyzed changes in sex steroids, adipokines, Ki-67, and drug levels in relation to obesity. Postmenopausal women with early-stage ER-positive breast cancer were randomized to either exemestane 25 mg QD (n = 57), 25 mg TIW (n = 57), or 25 mg/week (QW, n = 62) for 4-6 weeks before breast surgery. The presented case study details a 17-year-old patient undergoing a dental procedure requiring nasotracheal intubation (NTI). A red rubber catheter was utilized to assist with endotracheal tube placement. The nasal cavity contains highly vascular turbinates that can be injured during NTI. Even minor trauma can cause epistaxis, making it the most prominent complication of NTI. Catheter-guided NTI is one technique suggested to reduce nasal trauma. Does catheter-guided versus non-catheter-guided NTI reduce the risk of nasal bleeding or swelling in adult patients without a known risk of a difficult airway within the perioperative timeframe?

BACKGROUND: The early management of polytrauma patients with traumatic spinal cord injury (tSCI) is a major challenge. Sparse data is available to provide optimal care in this scenario and worldwide variability in clinical practice has been documented in recent studies.
METHODS: A multidisciplinary consensus panel of physicians selected for their established clinical and scientific expertise in the acute management of tSCI polytrauma patients with different specializations was established. The World Society of Emergency Surgery (WSES) and the European Association of Neurosurgical Societies (EANS) endorsed the consensus, and a modified Delphi approach was adopted.

Mild traumatic brain injury (mTBI) is a public health concern. The present study aimed to develop an automatic classifier to distinguish between patients with chronic mTBI (n = 83) and healthy controls (HCs) (n = 40). Resting-state functional MRI (rs-fMRI) and positron emission tomography (PET) imaging were acquired from the subjects. We proposed a novel deep-learning-based framework, including an autoencoder (AE), to extract high-level latent and rectified linear unit (ReLU) and sigmoid activation functions. This study aims to develop an advanced mathematic model and investigate when and how will the COVID-19 in the US be evolved to endemic. We employed a nonlinear ordinary differential equations-based model to simulate COVID-19 transmission dynamics, factoring in vaccination efforts. Multi-stability analysis was performed on daily new infection data from January 12, 2021 to December 12, 2022 across 50 states in the US. Food insecurity is a known health equity threat for formerly chronically homeless populations even after they transition into permanent housing. This project utilized a human-centered design methodology to plan and implement a nutrition-focused community-health-worker (CHW) intervention in permanent supportive housing (PSH). The project aimed to increase access to healthy foods, improve nutritional literacy, healthy cooking/eating practices, and build community/social connectedness among 140 PSH residents. Validated food-security screening conducted by CHWs identified low or very low food security among 64% of 83 residents who completed the baseline survey, which is similar to rates found in a previous study among formerly homeless …

OBJECTIVE: The early stages of chronic disease typically progress slowly, so symptoms are usually only noticed until the disease is advanced. Slow progression and heterogeneous manifestations make it challenging to model the transition from normal to disease status. As patient conditions are only observed at discrete timestamps with varying intervals, an incomplete understanding of disease progression and heterogeneity affects clinical practice and drug development.

Objective: To construct an exhaustive Complementary and Integrative Health (CIH) Lexicon (CIHLex) to help better represent the often underrepresented physical and psychological CIH approaches in standard terminologies, and to also apply state-of-the-art natural language processing (NLP) techniques to help recognize them in the biomedical literature.
Materials and methods: We constructed the CIHLex by integrating various resources, compiling and integrating data from biomedical literature and relevant sources of knowledge. The Lexicon encompasses 724 unique concepts with 885 corresponding unique terms. OBJECTIVE: Given the importance AI in genomics and its potential impact on human health, the American Medical Informatics Association-Genomics and Translational Biomedical Informatics (GenTBI) Workgroup developed this assessment of factors that can further enable the clinical application of AI in this space.
PROCESS: A list of relevant factors was developed through GenTBI workgroup discussions in multiple in-person and online meetings, along with review of pertinent publications. This list was then summarized and reviewed to achieve consensus among the group members.
CONCLUSIONS: Substantial informatics research and development are needed to fully realize the clinical potential of such technologies. The development of …
