INTRODUCTION: Improvements in splenomegaly, anemia, and overall survival (OS) are key considerations in the management of patients with myelofibrosis. In the phase III SIMPLIFY-1 trial (NCT01969838), momelotinib was noninferior to ruxolitinib for spleen volume reduction ≥ 35% (SVR35) and nominally superior for transfusion independence (TI) at week 24 in Janus kinase (JAK) inhibitor-naive patients. However, the relative impact of these endpoints on OS in anemic patients has not been described.

MATERIALS AND METHODS: The present post hoc analysis evaluated week 24 SVR35, TI, and dual responses (SVR35 + TI) in patients with baseline hemoglobin < 10 g/dL.

IMPORTANCE: Female patients with heart failure (HF) are older and more often present with preserved left ventricular ejection fraction (LVEF), whereas male patients present with more ischemic disease. Despite these differences, an emergency department-based acute HF strategy may be equally applicable to both sexes.

OBJECTIVE: To determine whether the strategy for acute HF management in the Comparison of Outcomes and Access to Care for Heart Failure (COACH) trial differed by sex.

Modern administrative-law scholarship, theory, and doctrine generally conceptualize agencies as engaging in three primary functions: rulemaking, enforcement, and adjudication. This understanding of agencies has informed deep debates surrounding the power, independence, and constitutional legitimacy of the administrative state. But the prevailing account is incomplete. It overlooks a fourth core function of the administrative state: its fact-making, or epistemic, function. Across the administrative state, agencies create and disseminate information in ways that are uniquely comprehensive and uniquely powerful. Hematopoietic stem cell transplantation (HSCT) is one of the only curative therapy options for many hematologic malignancies. HSCT relies on effective conditioning regimens to deplete host hematopoietic cells and enable donor engraftment. The alkylating agent Busulfan is widely used in myeloablative conditioning strategies due to its potent cytotoxic effects on hematopoietic cells and ability to clear the bone marrow. However, optimization of conditioning strategies remains an important area of investigation to improve transplant outcomes and reduce toxicity. Preliminary studies have shown that the FLT3 inhibitor Quizartinib synergizes with some chemotherapeutic agents, suggesting that targeted inhibitors may enhance conventional conditioning regimens. …

Connecticut argued that the Bill of Rights contains penumbras—implied zones of privacy protections— that emanate from the First, Third, Fourth, and Fifth Amendments. His penumbral conception of privacy rights has sparked extensive debate among legal scholars, reflecting both its influence and its ongoing controversy. Constitutional law haunts our efforts to reduce the number of firearm deaths each year. But even with the evolving scope of individual rights, sensible regulation remains possible.

This Article proposes one such regulatory tool. State governments legally can and pragmatically should require firearm sellers to advise their customers of the association between improper gun storage and the risk of accidental death and suicide. Mandatory safe storage advisories implicate two strands of constitutional jurisprudence: the First Amendment’s protection against compelled speech and the Second Amendment’s right to bear arms. This Article is the first to examine whether trademark anti-tarnishment laws should be deemed constitutional after the Supreme Court’s 2024 decision in Vidal v. Elster. Anti-tarnishment laws protect famous brands from uses that could damage their reputation, even when consumers are not confused about who makes the tarnishing product.

The Supreme Court recently declared some trademark registration restrictions unconstitutional in Matal v. Tam and Iancu v. Brunetti. In these cases, the Court struck down federal laws that prevented registration of “disparaging” trademarks and “scandalous or immoral” trademarks because they categorized the registration bars as unconstitutional viewpoint-based restrictions. At the University of Alaska, Anchorage (UAA), these processes op- erate within a complex institutional environment shaped by Institutional Review Board (IRB) requirements, Internal Revenue Service (IRS) reporting rules, and administrative procedures that remain only partially formalized. FD lesions may affect a single bone or occur throughout the entire skeleton and can grow rapidly. Supramolecular hydrogels have emerged as a versatile class of biomaterials capable of mimicking the dynamic behavior of natural tissues due to their inherent hydrophilicity, stimuli-responsiveness, and viscoelastic behavior. However, achieving both biocompatibility and mechanical tunability remains a challenge. This work utilizes DNA intercalation to create a programmable, sustainable hydrogel platform. Phytoplankton are central to marine ecosystem functioning, driving primary production, nutrient cycling, and higher trophic level dynamics. In the spring and summer of 2023, the Gulf of Maine experienced an unprecedented bloom of the dinoflagellate Tripos muelleri, reaching densities exceeding 50,000 cells L-1, an order of magnitude higher than previously recorded, and disrupting the region’s typical spring diatom-dominated succession. This bloom provided a unique opportunity to examine the metabolic strategies, ecological interactions, and environmental drivers underlying the bloom persistence and decline. This dissertation is a narrative microhistory of American diplomacy during Costa Rica’s transformative 1948 civil war. Emerging out of a contested presidential election, the war pitted the Costa Rican government of Teodoro Picado and its communist allies against an anticommunist, nationalist rebel movement led by José Figueres in a monthlong fight for political power. While existing histories of American policymaking have concluded almost unanimously that the United States intervened to support the rebels as part of its Cold War strategy in Latin America—labeling the civil war a precursor to the more infamous coup in Guatemala in 1954—this dissertation seeks to …

PURPOSE: To evaluate the accuracy and prose responses of 2 large language models (LLMs) to ophthalmology continuing medical education questions.

DESIGN: Question prompts and multiple choice (MC) answer options were input into the 2 LLMs, and responses were analyzed for accuracy and assessed for evidence of correctness, completeness, bias, and potential harm using a previously reported standardized rubric.

SUBJECTS: Basic and Clinical Science Course questions and MC answer options from the American Academy of Ophthalmology question bank were used as inputs into the 2 LLMs (ChatGPT-4 and Google Vertex's Gemini Pro 1.5).

Disparities in patient portal use undermine equitable chronic disease management, particularly for adults with diabetes in rural and historically underserved communities. This explanatory sequential mixed-methods study examined the associations among trust in the healthcare system and patient portal use among U.S. adults, using nationally representative data from the Health Information National Trends Survey (HINTS) 2023 (N = 3,711), supplemented by semi-structured interviews with frontline healthcare providers. Bivariate analyses revealed that adults with diabetes reported lower trust (83% vs. 86%, p = 0.003) and lower portal use (62% vs. 66%, p = 0.014) compared to those without a diagnosis. Many college students experience financial stress, yet financial hardship is often explained as a simple lack of financial knowledge. Recent research suggests that how students are socialized around money may matter just as much as what they know. In this study, I examined whether parental education, specifically first-generation college status, changes the relationship between household income and financial health among undergraduate students at a regional public university in the southeastern United States. Catholic nuns in the United States have experienced a substantial decline in membership, with an 80% decrease in new members from 1965 to 2023 (CARA, 2023). Sisters impact their communities in a multitude of ways, often through education or volunteerism efforts (CCUSA, 2026; Levin, 2011; Sisters of Mercy, 2025). This research study examines the impact of mentorship within the South Chattanooga Community Center’s after-school program, with a focus on elementary-aged students. The purpose of this study is to evaluate how one-on-one mentorship affects students’ academic development, behavior, and social-emotional learning. The program operates as a cost-free, five-day-a-week initiative designed to provide a safe and supportive environment for children who may lack consistent guidance outside of school.
