Background: Tools for mortality prediction in patients with the severe hypercholesterolemia phenotype (low-density lipoprotein cholesterol ≥190 mg/dL) are limited and restricted to specific racial and ethnic cohorts. We sought to evaluate the predictors of long-term mortality in a large racially and ethnically diverse US patient cohort with low-density lipoprotein cholesterol ≥190 mg/dL.

Methods: We conducted a retrospective analysis of all patients with a low-density lipoprotein cholesterol ≥190 mg/dL seeking care at Montefiore from 2010 through 2020. Patients < 18 years of age or with previous malignancy were excluded. The primary end point was all-cause mortality. Analyses were stratified by age, sex, and race and ethnicity. Patients were stratified by primary and secondary prevention. Cox regression analyses were used to adjust for demographic, clinical, and treatment variables.

Results: A total of 18 740 patients were included (37% non-Hispanic Black, 30% Hispanic, 12% non-Hispanic White, and 2% non-Hispanic Asian patients). The …

Association Of Cardiovascular Risk Profile With Premature All-Cause And Cardiovascular Mortality In Us Adults: Findings From A National Study, Ryan T Nguyen, Vardhmaan Jain, Isaac Acquah, Safi U Khan, Tarang Parekh, Mohamad Taha, Salim S Virani, Michael J Blaha, Khurram Nasir, Zulqarnain Javed

Objective: To assess the association between cardiovascular risk factor (CRF) profile and premature all-cause and cardiovascular disease (CVD) mortality among US adults (age < 65).

Methods: This study used data from the National Health Interview Survey from 2006 to 2014, linked to the National Death Index for non-elderly adults aged < 65 years. A composite CRF score (range = 0-6) was calculated, based on the presence or absence of six established cardiovascular risk factors: hypertension, diabetes, hypercholesterolemia, smoking, obesity, and insufficient physical activity. CRF profile was defined as "Poor" (≥ 3 risk factors), "Average" (1-2), or "Optimal" (0 risk factors). Age-adjusted mortality rates (AAMR) were reported across CRF profile categories, separately for all-cause and CVD mortality. Cox proportional hazard models were used to evaluate the association between CRF profile and all-cause and CVD mortality.

Results: Among 195,901 non-elderly individuals (mean age: 40.4 ± 13.0, 50% females and 70% Non-Hispanic (NH) White adults), 24.8% had optimal, 58.9% average, and 16.2% poor CRF profiles, respectively. Background: American College of Cardiology/American Heart Association guidelines recommend distinct risk classification systems for primary and secondary cardiovascular disease prevention. However, both systems rely on similar predictors (eg, age and diabetes), indicating the possibility of a universal risk prediction approach for major adverse cardiovascular events (MACEs).

Objectives: The authors examined the performance of predictors in persons with and without atherosclerotic cardiovascular disease (ASCVD) and developed and validated a universal risk prediction model.

Congenital vertebral malformation, affecting 0.13-0.50 per 1000 live births, has an immense locus heterogeneity and complex genetic architecture. In this study, we analyze exome/genome sequencing data from 873 probands with congenital vertebral malformation and 3794 control individuals. Clinical interpretation identifies Mendelian etiologies in 12.0% of the probands and reveals a muscle-related disease mechanism. Gene-based burden test of ultra-rare variants identifies risk genes with large effect sizes (ITPR2, TBX6, TPO, H6PD, and SEC24B). To further investigate the biological relevance of the genetic association signals, we perform single-nucleus RNAseq on human embryonic spines. The objective of this study was to develop a caretaker-completed questionnaire entitled “Cats’ Behavioral Suitability for Animal-Assisted Interventions” (CatSAAI), covering two categories of items associated with seven feline personality factors. These feline personality factors were previously identified and validated by Mikkola et al. (2021) on a large sample of domestic cat guardians. The aim of the CatSAAI questionnaire is to facilitate the interpretation of the behavioral suitability or unsuitability of each individual cat for inclusion in feline-assisted interventions, while considering the prevention of stress and risks in AAI settings for both people and animals. Alzheimer’s Disease (AD) is a progressive neurodegenerative disease often characterized by memory loss, confusion, and overall cognitive decline. The aging global population has, in recent years, highlighted the fundamental lack of pharmacological treatments for individuals facing an AD diagnosis. In response, a growing body of research has shifted focus to non-pharmacological humanities-based interventions. One such intervention has been music therapy (MT). Embedded Support In The College Writing Classroom: A Teaching Reflection On Late Pandemic Pedagogy For Trio Students In An Intensive Transitional Summer Course, James P. Austin, John Gavin Iv

Pedagogy and the Human Sciences

In this teaching reflection, the authors discuss their experiences as professor and embedded support for an intensive summer college writing course for incoming undergraduates participating in a TRIO program. The reflection considers the contextual factors making this cohort of students vulnerable, including the relationship between family income level and pandemic-era learning loss. The authors devised a pedagogy to "flip" the classroom, allowing students to write deeply during long class sessions, and delivered intensive, layered support at the point of writing to accelerate progress through challenges in writing development.

Automation is in every aspect of our lives. Autonomous cars, or ‘self- driving cars,’ dominate today’s headlines. There is just one problem: the United States’ legislative infrastructure is not suited for these autonomous cars to become commonplace on the roads. The United States’ fractured system results in a maze of state laws that make it difficult for manufacturers to comply. Other countries, like Germany and Singapore, have enacted systems to make the testing and implementation of autonomous cars more efficient. The global shortage of viable organs for transplantation, exacerbated by the COVID-19 pandemic, highlights a stark contrast between organ donation systems, particularly evident in the United States and Iran. While the United States relies on an altruistic donation system, resulting in millions on its waiting list, Iran's compensation-based approach has nearly eradicated its waitlist. The legal framework of the United States, shaped by property law, historical views on the human body, and federal statutes, complicates its approach to organ donation. With human space exploration expanding to establish bases on the Moon, there are increased challenges involved to sustain astronauts. One major limitation is the food supply, which must be constantly replaced and increases mission costs. However, with long-duration missions to the Moon, the lunar environment can provide resources that can be accessed in-situ for plant growth. Plant production in space, however, poses challenges inherent to the biological stress response imposed by factors like microgravity and radiation, as shown by multiple experiments at the ISS or in simulated space environments. In this practice brief, we outline our research focused on grandparents raising grandchildren in Prince Edward Island (PEI), Canada. Specifically, we highlight a series of collaborative community workshops implemented as part of our research project. Using a participatory action research approach, we worked with grandparents raising grandchildren to co-design and implement the workshops, which we titled It Takes a Village to Raise a Grandchild. These workshops brought together grandparents raising grandchildren and key community members, including clergy, educators, healthcare providers, politicians, social services, child services, and researchers. Grand Connections: A Workshop Series To Support Grandparents Caring For Grandchildren Under 5, Kathy Kinsner

GrandFamilies: The Contemporary Journal of Research, Practice and Policy

A workshop series for grandparents caring for grandchildren under five was developed by ZERO TO THREE and piloted online in 2021 as part of a multi-year effort to create resources specifically for grandparents and kinship caregivers. Seven workshops, each 90 minutes long, offer a unique 3-generation approach: Grandparents receive the latest information on child-rearing, strategies for collaborating with their grandchild’s parents, and opportunities to connect with their grandchildren through read-alouds and art activities. Workshop materials (detailed lesson plans, slide decks, and grandparent handouts) are available online at no charge to organizations wishing to run a program of their own.

Globally, many grandparents are taking on the caregiving role for their grandchildren without public or government interjection of support, particularly financial assistance for basic needs such as housing, health care and living expenses. This paper aimed to broaden understanding of social and systemic barriers experienced by grandparent-caregivers across the globe. Of the 2,828 relevant grandparent caregiving studies identified in the literature, 34 representing eight countries met our inclusion criteria to answer the research question and the focus of this paper: What are the social and systemic barriers experienced by grandparent-caregivers across the globe? Air Force nurses must be prepared for deployment to support military missions on short notice. During deployments, they may be asked to practice in demanding and austere wartime environments. The required competencies for deployment include the care of multi-trauma patients, military vehicles and aircraft, mass casualty care, and military-specific skills. Many Air Force nurses work in low acuity roles and do not maintain the clinical competency and skills necessary if called upon to deploy. Purpose: The purpose of this phenomenological study was to identify and describe strategies used by superintendents of California public school districts in Los Angeles County to build an adaptive capacity based on the five key characteristics of adaptive leadership by Heifetz et al. (2009).

Methodology: This phenomenological study explored the lived experiences of superintendents serving in public school districts in Los Angeles County. Purposeful, convenience sampling was used to identify 10 participants who met the study’s criteria.
