Introduction: Buccal shelf area (BSA) is a recommended anatomic site for mini-screw insertion. Stability of miniscrew is determined by quantity of bone available in the region.

Objective: To determine bone thickness and height of BSA for optimum placement of miniscrew.

Materials and Methods: This cross-sectional observational study included 40 cone-beam computed tomography (CBCT) scans (23 males, 17 females) of patients aged 19-38 years. Advanced Issues in Criminal Law: Sex Offenses

This paper examines the long-term psychological, emotional, and financial harms suffered by child victims of sexual abuse and argues that existing restitution statutes in the United States fail to provide meaningful, sustained support for survivors. Drawing on developmental psychology research, criminal restitution frameworks, and comparative state statutes, the paper demonstrates how current laws often limit recovery to narrow categories of offenses or leave compensation largely discretionary. Through historical analysis and statutory comparison, the paper traces the origins of marital rape exemptions to common law doctrines rooted in implied consent, coverture, and the treatment of women as property. Leadership and workforce preparation are vital roles of the nation’s academic institutions to advance health care services for the members of our communities. Leadership Education in Neurodevelopmental and other Related Disabilities (LEND) programs aim to prepare leaders in health care services and advocacy to improve systems and the quality of care and supports for the disability community, specifically children with developmental disabilities. College students are sometimes reluctant to purchase textbooks for various reasons, but they still must study the necessary content in a course or academic program. This Scholarship of Teaching and Learning (SoTL) research project examined college students’ perceptions of reading middle-grade novels in place of a traditional, formal textbook within an Adolescent Development course. All participants were part of a teacher education program at a four-year, public institution in the Midwest United States. A waveform security model may use a holographic sharding process to transform a data object into a set of mathematically incomplete and individually unintelligible interference patterns. An orchestration engine can distribute these shards across multiple storage providers and/or jurisdictions, configured to prevent a single entity from possessing sufficient information for reconstruction. The complete data object can be reconstituted, for example, ephemerally within a secure processing environment on an authorized client device (e.g., a smartphone, a laptop computer) at a time of use. A system is described for displaying persistent, live-data overlays on a computing device, such as a smart television or media streaming device, which can reduce interruption to a primary, full-screen viewing experience. The system may use hardware-decoupled composition, where a method can include rendering a primary video stream onto a video plane and a data widget onto a separate graphics plane. These hardware planes can then be combined by a hardware composer. While Wernicke encephalopathy (WE) is well-recognized as a condition most frequently associated with chronic excessive alcohol consumption, non-alcoholic WE (NAWE) remain underrecognized despite their clinical importance.  An 85-year-old nursing home resident was brought to the Emergency Department (ED) with impaired consciousness. She had been taking furosemide 40 mg and spironolactone 25 mg daily for chronic heart failure for five months prior to presentation. One month before admission, she began receiving parenteral nutrition without thiamine due to poor oral intake accompanied by vomiting. Staphylococcus lugdunensis is a coagulase-negative staphylococcus (CoNS) with unique virulence traits that mimic Staphylococcus aureus, despite traditionally being grouped among less pathogenic skin flora. Unlike other CoNS, S. lugdunensis can infect native valves and cause aggressive infective endocarditis (IE). We present a rare and instructive case of S. lugdunensis native mitral valve endocarditis complicated by cerebral embolic infarctions and requiring surgical valve replacement in a patient on chronic peritoneal dialysis. Methanol poisoning remains an important cause of severe metabolic and neurologic toxicity, most commonly resulting from ingestion of illicit or improperly distilled alcoholic beverages such as Moonshine. We report a case of acute methanol toxicity following Moonshine ingestion, presenting with central nervous system manifestations and visual symptoms. The patient experienced rapid clinical and visual improvement after timely treatment with fomepizole and emergent hemodialysis. In addition to standard therapy, corticosteroids were administered as an adjunctive treatment for visual involvement. Mycobacterium chimaera is a slow-growing, non-tuberculous mycobacterium (NTM) that belongs to the Mycobacterium avium complex (MAC). It first emerged as a pathogen after being associated with infections in patients who underwent cardiothoracic surgery when patients were getting exposed to heater cooler units, especially those requiring extracorporeal membrane oxygenation (ECMO) or prosthetic valve placement. We report an atypical presentation of M. chimaera infection in a patient with COPD, but no prior history of cardiac surgery or immunosuppression.

Introduction: Vitamin B12 deficiency may rarely present with hemolysis and schistocytosis, mimicking thrombotic microangiopathy (TMA). This pseudo-TMA variant is clinically significant, as delayed recognition may lead to unnecessary treatments and adverse outcomes.

Case Presentation: A 44-year-old male with no significant medical history or dietary restrictions presented with progressive lightheadedness, epigastric discomfort, and unintentional weight loss. Laboratory evaluation revealed macrocytic anemia with evidence of hemolysis and reticulocytopenia. A negative Coombs test made autoimmune hemolytic anemia less likely. Further workup demonstrated low serum vitamin B12 and elevated methylmalonic acid, consistent with B12 deficiency. Upper endoscopy with gastric biopsy revealed atrophic mucosa lacking …

This case report underscores the critical interplay between adrenal crisis and seizure activity, emphasizing the importance of rapid recognition and management of this life-threatening condition. Adrenal crisis, characterized by severe hypotension and electrolyte disturbances, often manifests with nonspecific symptoms such as fatigue, nausea, and altered consciousness, which can obscure diagnosis, especially in pediatric and young adult populations. The report details a 21-year-old female with a complex endocrinologic background who developed seizures precipitated by an adrenal crisis triggered by gastroenteritis and compounded by medication noncompliance. Thyrotoxic periodic paralysis (TPP) is a rare but life-threatening cause of acute flaccid paralysis associated with hypokalemia. Although more common in males and individuals of Asian descent, it may present atypically in young females and be easily misdiagnosed. We report a case of a 30-year-old female who presented with acute quadriparesis. Extensive neuromuscular evaluation initially pointed toward Guillain-Barré syndrome (GBS), but subsequent investigations revealed profound hypokalemia. Further workup confirmed thyrotoxicosis as the underlying cause. Prompt potassium replacement and initiation of antithyroid therapy led to complete clinical recovery. Background

Effective healthcare delivery involves coordinated communication between healthcare team members.

Objective

This study compares challenges in communication between nurses and residents.

Methods

This is a single-centered survey study in which 58 Nurses and 54 Internal Medicine residents at Temple University Hospital anonymously completed a 14-item questionnaire between August 2023 and March, 2024.

Results

High turnover among millennial hospitalists, who constitute a large proportion of early-career physicians, remains a concern. Using purposeful sampling, we surveyed 136 millennial hospitalists to understand what factors influence their decisions to leave a hospital medicine group. Family reasons (81%) and geographical location (80%) were the factors most commonly identified as important influences on decisions to leave. While these factors may be non-modifiable to a large extent, these findings underscore the importance of identifying such needs at the time of recruitment. Pleural fibrosis involves excessive extracellular matrix (ECM) deposition driven by chronic injury and inflammation. Transforming growth factor-β (TGF-β) induces mesothelial–mesenchymal transition (MesoMT), promoting a myofibroblast phenotype. Our laboratory identified myocardin (Myocd), a transcriptional coactivator, as a key mediator of MesoMT that regulates ECM-remodeling proteases. RNA-seq and qPCR analyses showed that ADAM12 and ADAM19 are upregulated by TGF-β but suppressed by Myocd knockdown in Human pleural mesothelial and IPF-derived cells. Silencing ADAM12 reduced fibrotic markers, including fibronectin and smooth muscle actin (α-SMA), whereas ADAM19 showed relatively minor effects. Background: Traditional nursing education may limit active learning, clinical judgment development, and cognitive engagement among prelicensure nursing students. Evidence supporting measurable outcomes in traditional clinical education is limited (Leighton et al., 2021). Emerging technologies, including virtual reality (VR), augmented reality (AR), and artificial intelligence (AI)-enhanced learning, offer alternative approaches.

This qualitative research study examined the discourse of school personnel in the twice-exceptional (2E; Kaufman, 2018) student identification process (Gilman & Peters, 2018). It used disability critical race studies (DisCrit; Annamma et al., 2016) to investigate how educators’ discourse signals the inclusion or exclusion of 2E Black girls. Critical discourse analysis (Gee, 2014) was used to understand the ways their intersectional identities of race, ability, and gender impact their identification in schools. It highlighted the identification barriers for 2E Black girls and conceptualized a normative 2E construct (N.A.T.E.) aligned with the dominant institutional discourse of twice-exceptionality.
