Implementation Of Chlorhexidine Gluconate Dressings And The Effect On Bloodstream Infections In Extracorporeal Membrane Oxygenation Patients At A Pediatric Hospital, Kaitlin Vernier
Dissertations
Extracorporeal membrane oxygenation (ECMO) is an advanced lifesaving therapy used for patients with significant cardiorespiratory failure not responding to conventional therapies. The most common and life-threatening complications associated with ECMO are blood stream infections (BSIs). In addition to increased mortality, BSI's increase patient length of stay and cost. Chlorhexidine Gluconate (CHG) is known to be a significant factor in BSI reduction. The purpose of this quality improvement project is to assess the utilization of CHG-impregnated dressings on the incidence of BSI rates in pediatric ECMO patients. The setting was a 40-bed pediatric intensive care unit (PICU) and a 30-bed cardiac …

Inter-Kingdom Communication And The Sympoietic Way Of Life, Scott F. Gilbert
Inter-Kingdom Communication And The Sympoietic Way Of Life, Scott F. Gilbert
Biology Faculty Works
Organisms are now seen as holobionts, consortia of several species that interact metabolically such that they sustain and scaffold each other’s existence and propagation. Sympoiesis, the development of the symbiotic relationships that form holobionts, is critical for our understanding the origins and maintenance of biodiversity. Rather than being the read-out of a single genome, development has been found to be sympoietic, based on multigenomic interactions between zygote-derived cells and symbiotic microbes. These symbiotic and sympoietic interactions are predicated on the ability of cells from different kingdoms of life (e.g., bacteria and animals) to communicate with one another and to have …

Wiping Out Central Line Associated-Bloodstream Infections: Cleaning High Touch Surfaces In The Picu, Emily Heth
Dissertations
Problem: Central line-associated bloodstream infections (CLABSIs) are a significant, preventable healthcare-related complication of hospital admission and medical management of critically ill and medically complex patients. CLABSIs result in significant adverse outcomes such as increased morbidity and mortality, prolonged length of stay (LOS) in the intensive care unit (ICU) and in the hospital, increased cost of treatment, and increased rates of readmission. Direct and indirect contact within the patient’s hospital environment are potential means of transmission for microorganisms, increasing the inherent risk of acquiring a CLABSI.
Method: For this quality improvement (QI) project, a pre-post design was utilized to evaluate the …

The incidence of inflammatory bowel disease (IBD) is increasing annually. Children with IBD often suffer significant morbidity due to physical and emotional effects of the disease and treatment. Corticosteroids, often a component of therapy, carry undesirable side effects with long term use. Steroid-free remission has become a standard for care-quality improvement. Anticipating therapeutic outcomes is difficult, with treatments often leveraged in a trial-and-error fashion. Artificial intelligence (AI) has demonstrated success in medical imaging classification tasks. Predicting patients who will attain remission will help inform treatment decisions. The provided dataset comprises 951 tissue section scans (167 whole-slides) obtained from 18 pediatric …

Department of Psychology Faculty Scholarship and Creative Works
Objective: Children with attention-deficit/hyperactivity disorder (ADHD) exhibit difficulties with organizational skills such as task planning, managing materials, and organizing activities that have downstream consequences on academic functioning. At the same time, deficits in working memory have been linked with both the organizational skills difficulties and academic underachievement and underperformance observed in children with ADHD and have been hypothesized to account for the link between organizational and academic functioning. However, the extent to which working memory and organizational skills independently versus jointly contribute to ADHD-related academic difficulties remains unclear. Method: The present study is the first to examine the unique and …

Energy storage and endocrine functions of the Drosophila fat body make it an excellent model for elucidating mechanisms that underlie physiological and pathophysiological organismal metabolism. Combined with Drosophila's robust genetic and immunofluorescence microscopy toolkits, studies of Drosophila fat body function are ripe for cell biological analysis. Unlike the larval fat body, which is easily removed as a single, cohesive sheet of tissue, isolating intact adult fat body proves to be more challenging, thus hindering consistent immunofluorescence labeling even within a single piece of adipose tissue. Here, we describe an improved approach to handling Drosophila abdomens that ensures full access of …

An Interpretable Tree Interaction Model (Tim) For Eastern U.S. Forests: Machine Learning Insights Into Landscape Beta Diversity & Ecosystem Dynamics, George Michael Williford
Electronic Theses and Dissertations Archive
This thesis presents the development of a novel Tree Interaction Model (TIM), which investigates the interplay of biotic and abiotic factors influencing tree species growth and abundance in Eastern US forests. The TIM merges species importance values, growth metrics, beta diversity components, and environmental variables to address the limitations of previous species distribution models (SDMs), which have concentrated on abiotic drivers of niche. With tree data from the Forest Inventory and Analysis (FIA) program, the model applies a Random Forest machine-learning approach, augmented by SHAP (SHapley Additive exPlanations) analysis, to provide interpretable insights into the interactions of various variables and …

Hsv+ And Proud: Does Illness Identity Mediate The Relationship Between Genital Herpes Stigma And Well-Being?, Nikolas Forest Priest
Electronic Theses and Dissertations Archive
Genital herpes is a common, incurable, sexually transmitted infection that can be accompanied by significant social stigma that is associated with psychological distress, diminished quality of life, and diminished sexual well-being. How people incorporate chronic illnesses into their sense of self can also have significant effects on well-being. The illness identity model attempts to capture identity states that are more or less adaptive in adjusting to a chronic illness (Engulfment, Rejection, Acceptance, and Enrichment), but this model has not been studied in relation to stigmatized identities. The current study assessed if the different illness identity states mediated the relationship between …

In the current era of high-precision radiation therapy, real-time magnetic resonance (MR)-guided tracking of the tumor and organs at risk (OARs) is a novel approach that enables accurate and safe delivery of high-dose radiation. Organ tracking provides a general sense of the need for daily online adaptation but lacks precise information regarding exact dosimetry. To overcome this limitation, we developed the methodology for monitoring intrafraction motion with real-time MR-guided isodose line-based tracking of an OAR in combination with anatomic tumor-based tracking and reported the first case treated with this approach. An isolated para-aortic (PA) nodal recurrence from carcinosarcoma of the …

Centering The Leader: An Investigation Of Leader-Centric Variables In The Support Provision Process, Jordyn Jan Leslie
Dissertations and Theses
Few studies to date have investigated leader-centric variables associated with the support provision process. The following dissertation includes three studies that attempt to further understand such relationships. In Study 1, the interaction between leader sleep quantity and quality on various types of leader support (e.g., general supervisor support, family supportive supervisor behaviors, and sleep leadership) is studied using both employee and leader ratings. Study 2 integrates leader-centric research with work-life supportive leadership to propose a new theoretical model that delineates leader-centric variables (e.g., health & well-being, skills, role expectations, job demands) as precursors to the provision and perception of work-life …

Epigenetic dysregulation has emerged as an important etiological mechanism of neurodevelopmental disorders (NDDs). Pathogenic variation in epigenetic regulators can impair deposition of histone post-translational modifications leading to aberrant spatiotemporal gene expression during neurodevelopment. The male-specific lethal (MSL) complex is a prominent multi-subunit epigenetic regulator of gene expression and is responsible for histone 4 lysine 16 acetylation (H4K16ac). Using exome sequencing, here we identify a cohort of 25 individuals with heterozygous de novo variants in MSL complex member MSL2. MSL2 variants were associated with NDD phenotypes including global developmental delay, intellectual disability, hypotonia, and motor issues such as coordination problems, feeding …

Primary proteasomopathies have recently emerged as a new class of rare early-onset neurodevelopmental disorders (NDDs) caused by pathogenic variants in the PSMB1, PSMC1, PSMC3, or PSMD12 proteasome genes. Proteasomes are large multi-subunit protein complexes that maintain cellular protein homeostasis by clearing ubiquitin-tagged damaged, misfolded, or unnecessary proteins. In this study, we have identified PSMD11 as an additional proteasome gene in which pathogenic variation is associated with an NDD-causing proteasomopathy. PSMD11 loss-of-function variants caused early-onset syndromic intellectual disability and neurodevelopmental delay with recurrent obesity in 10 unrelated children. Our findings demonstrate that the cognitive impairment observed in these individuals could be …

Development Of A Simple And Accurate Polymerase Chain Reaction Screen For Human Papillomavirus In College Students, Emily Bishop, Owen Smith, Paul E. Richardson
Journal of the South Carolina Academy of Science
Human Papillomavirus (HPV) is, according to the CDC, the most common sexually transmitted infection (STI) with many infections developing in a person's late teens or early 20’s. HPV is a nonenveloped, relatively small, icosahedral DNA virus that infects squamous epithelial cells. This infection can happen in genital, anal, and oral cavities. 90% of these infections will self-clear in two years according to the CDC. Of the 10% of infections that do not clear in two years, they could develop into carcinomas later in life. Wound closure in surgeries is traditionally achieved using invasive methods such as sutures and staples. Adhesion-based wound closure methods such as tissue adhesives, sealants, and hemostats are slowly replacing these methods due to their ease of application. Although several chemistries have been developed and used commercially for wound closure, there is still a need for better tissue adhesives from the point of view of toxicity, wet-adhesion strength, and long-term bonding. Catechol chemistry has shown great promise in developing wet-set adhesives that meet these criteria. Herein, we have studied the biocompatibility of a catechol-based copolymer adhesive, poly([dopamine methacrylamide]-co-[methyl methacrylate]-co-[poly(ethylene glycol) methyl …

PURPOSE: To describe the experience of a single level 1 trauma center in the management of blunt splenic injuries (BSI). METHODS: This is a retrospective study with Institutional Review Board approval. The medical records of 450 patients with BSI treated between January 2016 and December 2022 were reviewed. Seventy-two patients were treated with splenic artery embolization (SAE), met the study criteria, and were eligible for data analysis. Spleen injuries were graded in accordance with the American Association for the Surgery of Trauma Organ Injury Scale. Univariate data analysis was performed, with P < 0.05 considered statistically significant. RESULTS: The splenic salvage rate was 90.3% (n = 65/72). Baseline demographics were similar between the groups (P > 0.05). Distal embolization with Gelfoam had similar rates …

Mystical Luminosity Experience: An Initiation Of A Construct Grounded In Phenomenology, Jonathan Dinsmore
Psychological Science Faculty Publications and Presentations
Light of a divine or transcendent nature is widely revered in various religious and mystical traditions around the world, and luminosity with mystical qualities such as love, bliss, peace, and noetic realization is also frequently reported by contemporary experiencers. Despite being described as a profoundly significant, sacred, and transformative experience, mystical luminosity has received relatively little attention in modern scholarship and scientific study, and has only been examined empirically within isolated contexts, such as NDEs or contemplative practices. This study examines the phenomenology which binds mystical luminosity across various experiential contexts to construct a phenomenologically grounded theoretical model. A three-part …

Frontotemporal dementia (FTD) is the second most common cause of early-onset dementia after Alzheimer disease (AD). Efforts in the field mainly focus on familial forms of disease (fFTDs), while studies of the genetic etiology of sporadic FTD (sFTD) have been less common. In the current work, we analyzed 4,685 sFTD cases and 15,308 controls looking for common genetic determinants for sFTD. We found a cluster of variants at the MAPT (rs199443; p = 2.5 × 10
