Chronic Guillain-Barré Syndrome From Misdiagnosed Lyme Disease, Carlos H. Cisneros Jr, Alberto E. Almeida
MEDI 9331 Scholarly Activities Clinical Years
Lyme disease (borreliosis) is caused by transmission of Borrelia burgdorferi from deer ticks in the United States. Neurological symptoms can manifest in the early disseminated stage of the disease and have the worst involvement in the late dissemination stage. Few, but important case reports exist of patients experiencing disseminated Lyme with associated Guillain-Barre Syndrome (GBS). GBS manifests as an ascending flaccid paralysis with additional sensory deficits and is characterized as an immune-mediated polyneuropathy attacking the peripheral nervous system. A college-aged, wheelchair bound, quadriplegic female presented to the clinic to establish care in October 2023. She reported being bitten by a …

Analyzing The Dosimetric Impact Of Ct Artifacts From High-Density Objects In Radiotherapy Planning, Stephen Reed
All ETDs from UAB
High-density objects such as prosthetics, cardiac implanted devices, or surgical clips can create artifacts in computed tomography (CT) images used for radiotherapy planning, leading to regions of inaccurate CT information. These high-density objects may cause artifacts that can impact the accuracy of dose calculations for radiotherapy treatment plans and may require large amounts of a clinician’s time to manually override. Previous research shows that larger ranges of CT numbers can impact the accuracy of dose calculations and that different dose calculations algorithms handle artifacts in CT images differently. Manual correction of voxels affected by high-density objects in CT scans is …
Examining Sense Of Belonging In Higher Education Leadership For Black Women With Intersecting Marginalized Identities, Dominique A. White
Examining Sense Of Belonging In Higher Education Leadership For Black Women With Intersecting Marginalized Identities, Dominique A. White
Walden Dissertations and Doctoral Studies
Black women juggling marginalized minority identities in executive leadership at public four-year universities contend with stigmas associated with race and gender, contributing to a decreased sense of belonging. Significant research has been done about the journeys of Black or African American women as they seek executive roles in higher education. However, minimal research explores Black women's sense of belonging once executive leadership status is reached. Understanding their sense of belonging could fill a gap in practice regarding Black women’s scarcity occupying executive university leadership. This qualitative study examined the perceptions of a sense of belonging for Black women juggling intersecting …

The joint analysis of imaging-genetics data facilitates the systematic investigation of genetic effects on brain structures and functions with spatial specificity. We focus on voxel-wise genome-wide association analysis, which may involve trillions of single nucleotide polymorphism (SNP)-voxel pairs. We attempt to identify underlying organized association patterns of SNP-voxel pairs and understand the polygenic and pleiotropic networks on brain imaging traits. We propose a bi-clique graph structure (ie, a set of SNPs highly correlated with a cluster of voxels) for the systematic association pattern. Next, we develop computational strategies to detect latent SNP-voxel bi-cliques and an inference model for statistical testing. …

Military burn pits, used for waste disposal in combat zones, involve the open-air burning of waste materials, including plastics, metals, chemicals, and medical waste. The pits release a complex mixture of occupational toxic substances, including particulate matter (PM), volatile organic compounds (VOCs), heavy metals, dioxins, and polycyclic aromatic hydrocarbons (PAHs). Air pollution significantly impacts brain health through mechanisms involving neuroinflammation. Pollutants penetrate the respiratory system, enter the bloodstream, and cross the blood-brain barrier (BBB), triggering inflammatory responses in the central nervous system (CNS). Chronic environmental exposures result in sustained inflammation, oxidative stress, and neuronal damage, contributing to neurodegenerative diseases and …

The safety and efficacy of chimeric antigen receptor T-cell therapy is not well described in older patients, a population that has higher frailty and comorbidities. In this multicenter retrospective study, we evaluated clinical outcomes along with frailty and geriatric characteristics such as comorbidities, polypharmacy, falls, neuropathy, organ dysfunction, and performance status in younger (aged < 65 years) vs older (aged ≥65 years) patients who received commercial idecabtagene vicleucel (ide-cel). A total of 156 patients (n = 75, aged ≥65 years) were infused with ide-cel by data cutoff. In older patients (median age: 69 years; range, 65-83; 66.7% frail; 77.3% did not meet KarMMa eligibility criteria), with a median follow-up duration of 14.2 months, best overall response rate (ORR) was 86.7%, which was comparable with pivotal KarMMa study results (ORR: 73%). Median progression-free survival and overall survival in older patients were 9.1 months and 26.5 months, respectively. Grade ≥3 cytokine-release syndrome and immune effector cell-associated neurotoxicity syndrome were observed in 1% and 4% of older patients, respectively. Compared with younger patients, the older patients had significantly higher prevalence of frailty, geriatric characteristics such as polypharmacy (≥5 drugs; 97%), ≥4 comorbidities (69%), and organ dysfunction (35%; P < .05). The safety and efficacy of ide-cel therapy were similar in younger and older patients. Frailty and geriatric characteristics such as polypharmacy, comorbidities, and organ dysfunction in older patients did not confer an inferior overall outcome.

Allogeneic hematopoietic cell transplantation (allo-HCT) recipients are susceptible to viral infections. We conducted a phase 2 trial evaluating the safety and rate of clinically significant infections (CSIs; viremia requiring treatment or end-organ disease) after infusion of posoleucel, a partially HLA-matched, allogeneic, off-the-shelf, multivirus-specific T-cell investigational product for preventing CSIs with adenovirus, BK virus, cytomegalovirus, Epstein-Barr virus, human herpesvirus-6, or JC virus. This open-label trial enrolled allo-HCT recipients at high risk based on receiving grafts from umbilical cord blood, haploidentical, mismatched, or matched unrelated donors; post-HCT lymphocytes of <180/mm3; or use of T-cell depletion. Posoleucel dosing was initiated within 15 to 49 days of allo-HCT and subsequently every 14 days for up to 7 doses. The primary end point was the number of CSIs due to the 6 target viruses by week 14. Of the 26 patients enrolled, only 3 (12%) had a CSI by week 14, each with a single target virus. In vivo expansion of functional virus-specific T cells detected via interferon-γ enzyme-linked immunosorbent spot assay was associated with viral control. Persistence of posoleucel-derived T-cell clones for up to 14 weeks after the last infusion was confirmed by T-cell-receptor deep sequencing. Five patients (19%) had acute graft-versus-host disease grade 2 to 4. No patient experienced cytokine release syndrome. All 6 deaths were due to relapse or disease progression. allo-HCT recipients at high risk who received posoleucel had low rates of CSIs from 6 targeted viruses. Repeat posoleucel dosing was generally safe and well tolerated and associated with functional immune reconstitution. This trial was registered at www.ClinicalTrials.gov as #NCT04693637.

Microfluidic systems combine multiple processing steps and components to perform complex assays in an autonomous fashion. To enable the integration of several bio-analytical processing steps into a single system, valving is used as a component that directs fluids and controls introduction of sample and reagents. While elastomer polydimethylsiloxane has been the material of choice for valving, it does not scale well to accommodate disposable integrated systems where inexpensive and fast production is needed. As an alternative to polydimethylsiloxane, we introduce a membrane made of thermoplastic elastomeric cyclic olefin copolymer (eCOC), that displays unique attributes for the fabrication of reliable valving. …

Recruitment of non-canonical BCOR-PRC1.1 to non-methylated CpG islands via KDM2B plays a fundamental role in transcription control during developmental processes and cancer progression. However, the mechanism is still largely unknown on how this recruitment is regulated. Here, we unveiled the importance of the Poly-D/E regions within the linker of BCOR for its binding to KDM2B. Interestingly, we also demonstrated that these negatively charged Poly-D/E regions on BCOR play autoinhibitory roles in liquid-liquid phase separation (LLPS) of BCORANK-linker-PUFD/PCGF1RAWUL. Through neutralizing negative charges of these Poly-D/E regions, Ca2+ not only weakens the interaction between BCOR/PCGF1 and KDM2B, but also promotes co-condensation of …

Covalent Bruton tyrosine kinase inhibitors (cBTKis), which bind to the BTK C481 residue, are now primary therapeutics for chronic lymphocytic leukemia (CLL). Alterations at C481, primarily C481S, prevent cBTKi binding and lead to the emergence of resistant clones. Pirtobrutinib is a noncovalent BTKi that binds to both wild-type (WT) and C481S-mutated BTK and has shown efficacy in BTK-WT and -mutated CLL patient groups. To compare baseline clinical, transcriptomic, and proteomic characteristics and their changes during treatment in these 2 groups, we used 67 longitudinal peripheral blood samples obtained during the first 3 cycles of treatment with pirtobrutinib from 18 patients …

Immune checkpoint blockade (ICB) has emerged as a promising therapeutic option for hepatocellular carcinoma (HCC), but resistance to ICB occurs and patient responses vary. Here, we uncover protein arginine methyltransferase 3 (PRMT3) as a driver for immunotherapy resistance in HCC. We show that PRMT3 expression is induced by ICB-activated T cells via an interferon-gamma (IFNγ)-STAT1 signaling pathway, and higher PRMT3 expression levels correlate with reduced numbers of tumor-infiltrating CD8+ T cells and poorer response to ICB. Genetic depletion or pharmacological inhibition of PRMT3 elicits an influx of T cells into tumors and reduces tumor size in HCC mouse models. Mechanistically, …

Neutrosophic Sets and Systems
In the context of aggregation operators and multiple criteria decision-making, this article presents the idea of cubic spherical neutrosophic sets. The notion of neutrosophic informations are transform into a geometric sphere by determining its center and radius. This advanced geometrical representation extends traditional neutrosophic informations. This study defines and discusses weighted additive and weighted geometric aggregation operators tailored to cubic spherical neutrosophic sets that are vital for handling complex and uncertain information. Practical example, such as evaluating fertilizer brands for coconut farming, illustrate their application in decision-making contexts. By integrating cubic spherical neutrosophic sets into multiple criteria decision-making frameworks, decision …

Neutrosophic Sets and Systems
Breast cancer is the most common type of cancer that affect women and yet there is no substantive cure. On early diagnosis of breast lesions with this size using mammogram would help the affected tumor patients have a good survival rate. Detecting cancer cells is a challenging task and hence we propose a single valued neutrosophic approach for segmenting breast tumor lesions in mammogram images. In this neutrosophic approach, the intensity values are represented as truth, indeterminacy and falsity membership in where the indeterminacy are noise, and the breast regions are the true and falsity values. The computations of these …

Neutrosophic Sets and Systems
Breast cancer is the most prevalent type of cancer that affects women worldwide and poses a serious risk to female mortality. In order to lower death rates and enhance treatment results, early detection is critical. Neutrosophic Set Theory (NST) and machine learning (ML) approaches are integrated in this study to provide a novel hybrid methodology (NS-ML) that improves breast cancer diagnosis. Using the Wisconsin Diagnostic Breast Cancer (WDBC) dataset, the research transforms these data into Neutrosophic (N) representations to effectively capture uncertainties. When trained on the N-dataset instead of traditional datasets, ML algorithms such as Decision Tree (DT), Random Forest …

Improved Method Of Interval Valued Neutrosophic Matrix Composition And Its Application In Medical Diagnosis, S. Sathiya, A. Puspalatha
Neutrosophic Sets and Systems
Neutrosophic sets are an important branch of topology that addresses issues with inconsistent, indeterminate, and uncertain situations in everyday life. Interval valued neutrosophic sets, known as subclasses of neutrosophic sets, have been specifically designed to address problems with a set of numbers in the real unit of interval rather than just a single number. Interval valued neutrosophic(IVN) matrix plays an influential role in decision making problems with indeterminant and inconsistent information. The objective of the paper is to discuss the concepts and operations of IVN matrix theory and propose a newly concept of an improved method of IVN matrix composition …

The developing nations of the Asian continent, especially India, are trying to elevate the educational standards by inaugurating the portals of international institutional linkages. Collaboration with foreign universities provides opportunities for the aspiring students to confer degrees from world premier institutions by stationing in their host nation. At the same time, the consequential impacts of the institutional linkages beyond the contours of countries need special attention to investigate on the dimensions of social, economic and culture. This chapter aims to make an intensive study on the challenges and impacts of institutional linkages between Institutions belonging to developing and developed nations. …
