Purpose: Caregivers play a critical role in providing support to stroke survivors during recovery, but they often face emotional, physical, and educational challenges that can lead to caregiver burden and burnout. Problem: Physiotherapy research increasingly requires methodological precision and clinical relevance. Among available designs, the randomized crossover model offers distinct advantages by allowing each participant to serve as their own control, thereby reducing inter-individual variability and improving statistical efficiency. Purpose: This perspective article discusses how appropriately designed crossover trials can enhance the quality and interpretability of physiotherapy research when applied under stable clinical conditions and with adequate washout periods. Methods: Three representative crossover studies illustrate these principles across physiotherapy specialties. In musculoskeletal practice, manual therapy for thoracic pain demonstrated measurable reductions in paraspinal electromyographic activity following counterstrain compared with spinal manipulation. …

The CMRS‑1 Sphere is a universal, multi‑domain physics engine integrating vortex induction, acoustic resonance, thermal equalization, structural load distribution, and energy harvesting into a single geometry. It consists of a spherical shell, a gyroid‑lattice interior, six helical intake channels, a central resonance cavity, three orthogonal acoustic chambers, and a thermal equalization layer. This disclosure provides complete geometry, build instructions, test procedures, failure‑mode analysis, and integration pathways. No biological, medical, or clinical instructions are included. Proposed herein is a system that automatically translates the scope of a professional services consulting engagement into a structured telemetry specification, referred to herein using a construct called a Telemetry Pack, that can be validated against a customer's actual product telemetry data. Instead of relying on manual status reports and consultant self-assessments, the system determines whether the engagement is achieving its intended product adoption outcomes by checking what the customer's product environment actually shows. No prior system connects what a consulting engagement is supposed to achieve with automated, telemetry-based verification of whether it has achieved the criteria of the engagement.

Firefighting is a complex and inherently dangerous profession. Firefighters today face a growing array of occupational hazards, not just from the effects of being exposed to fire and smoke, but also from the personal protective gear they wear, commonly referred to as “bunker” or “turnout” gear. While this gear is designed to protect firefighters in dangerous environments, prolonged use and repeated exposure to smoke, chemicals, and products of combustion can compromise the gear. This thesis investigates the computational challenges of constructing a database for affective incongruity, exploring the difficulties in automating the collection of contradictory affective states. Capturing these incongruities is essential for advancing Vision-Language Models (VLMs) and sentiment analysis, which struggle to interpret signals deviating from basic emotional archetypes. Curating non-congruent affect provides the data necessary for models to navigate complex social contexts, which is critical for applications like automated Audio Description (AD) for the visually impaired and nuanced Human-Computer Interaction (HCI). Miller-Dieker syndrome (MDS), also known as Miller-Dieker lissencephaly syndrome or chromosome 17p13.3 deletion syndrome, is a rare chromosomal disorder (microdeletion syndrome) characterized by lissencephaly, altered facial features, and neurologic disability. Classic phenotypic findings include a prominent forehead, micrognathia, and a short nose. Children with MDS are often born with associated end-organ involvement including congenital heart disease as well as motor and neurologic impairments including seizures. Progressive central nervous system involvement frequently leads to upper airway and swallowing dysfunction with recurrent bouts of aspiration, pneumonia, and respiratory failure. Adolescent sexual and reproductive health remains a critical public health priority in the United States, particularly given the persistent burden of HIV and sexually transmitted infections (STIs) among young people. Adolescents and young adults account for a disproportionate share of STI cases, reflecting ongoing gaps in prevention, early detection, treatment, and equitable access to sexual health care and services. Autoregulation Of Three Yeast Ribosomal Protein Genes By Splicing Inhibition, David Granas, Anirudh Kesanapally, Michael A White, Gary D Stormo

Yeast ribosomal protein genes RPL18B, RPL28, and RPS22B are autoregulated by inhibition of splicing. This is demonstrated by inserting their introns into a chromosomal copy of GFP and observing repression upon induction of the cognate protein. In RPL18B and RPS22B, a predicted conserved secondary structure within the intron is required for regulation, while in RPL28, it is not.

This project focuses on creating a locomotive device that is capable of movement without relying on active wheels. This kind of system is essential to exploration in rough terrains where wheel-based motion proves to be ineffective. Drawing inspiration from the natural movement of snakes- the animal most successful in traversing difficult and diverse environments- we will develop a modular snake-like robotic prototype. Our modular design will allow for simplified maintenance, easy component replacement, and opportunity for further development and research.

Traditional walkers provide essential physical support for people recovering from injury, surgery, or reduced mobility, but do not collect objective information about how a person walks. This thesis presents the design, implementation, and evaluation of a SmartWalker system that transforms a standard mobility walker into a portable gait data collection platform. The system integrates Intel RealSense cameras mounted on a walker, NVIDIA Jetson embedded computing units, local storage, and a graphical user interface to record lower-body movement, process video locally, and present clinician-reviewable gait metrics.

Department of Orthopaedic Surgery Faculty Papers

During skeletal growth, it is thought that the lactate secreted by the glycolytic nucleus pulposus (NP) cells exits the intervertebral disc into circulation via endplates. Our current studies challenge this long-held notion. Mice with early postnatal, endplate, and annulus fibrosus-specific deletion of lactate importer, MCT1, exhibited disc degeneration characterized by NP cell loss and pronounced endplate structural changes. Using metabolic and transcriptomic approaches, we demonstrate that MCT1 loss inhibits endplate chondrocyte differentiation and that lactate serves both as a crucial TCA metabolite and promotes protein and histone lactylation and gene expression. BACKGROUND: Simulation has played a vital role in training medical professionals. The COVID-19 Pandemic has highlighted the need for physically distant educational assessment methods. Screen-based simulation (SBS) demonstrates one alternative to traditional in-person simulation methods to assess learners clinical reasoning and communication skills. This study aims to compare in-person simulation to SBS as assessment methods in Emergency Medicine (EM).
