Background: Concurrent benzodiazepine (BZD) use among patients receiving buprenorphine for opioid use disorder (OUD) is common in routine care and presents a critical clinical dilemma. While buprenorphine improves survival and reduces overdose risk, co-exposure to central nervous system depressants may increase the risk of adverse events. However, medication-specific real-world evidence remains limited, and existing studies report conflicting findings. Objective: To synthesize real-world observational evidence on safety outcomes associated with concurrent BZD use during buprenorphine treatment and to critically appraise study quality using ROBINS-I V2. Background: Patients with opioid use disorder encounter challenges obtaining buprenorphine (BUP) at pharmacies, increasing their risk of treatment non-adherence and illicit drug use.   Objectives: The purpose of this quality improvement (QI) initiative was to implement and evaluate a multi-component intervention to ensure continuous BUP access.   Methods: Using a quasi-experimental study design, the initiative was developed and a convenience sample was recruited from an outpatient BUP treatment program and a pharmacy affiliated with the West Virginia University health system between November 20, 2023, to November 29, 2024. Background: Survey-based approaches for estimating pharmacy acquisition costs have become increasingly prevalent. While originally developed for Medicaid, these models have expanded and, in some states, have been adopted legislatively as a reimbursement price floor for all payers. The National Average Drug Acquisition Cost (NADAC) is the predominant national benchmark, but several states conduct independent surveys to establish their own Actual Acquisition Cost (AAC) benchmarks. Objective: To evaluate the comparative effectiveness of treatment augmentation, switching, or dose escalation for inadequate treatment response among patients living with depression. Methods: We conducted a retrospective cohort study using data from the Medicare Current Beneficiary Survey(MCBS; 2016-2022). We identified beneficiaries with depression who had both survey responses and had at least one antidepressant claim record 6 months before the outcome assessment. We excluded patients diagnosed with bipolar disorder, schizophrenia, and other psychotic disorders. Background: Schizophrenia is a chronic mental disorder that requires lifelong treatment. Medication nonadherence contributes to poor medical outcomes; the ability to predict a patient‚Äôs adherence trajectory cannot be understated in forestalling negative effects of nonadherence. This study aims to identify antipsychotic adherence latent classes among patients diagnosed with schizophrenia or schizoaffective disorder and evaluate predictors of adherence latent classes. Community pharmacists represent an accessible setting for care delivery, with nearly 90% of Americans living within five miles of a pharmacy; however, state laws governing pharmacist involvement in PrEP vary substantially. Drug overdose has been a public health crisis and north Mississippi remains a heavily impacted area. Adolescents and young adults, especially college and high school students, have increased exposure to opioids and other drugs, leading to an increased likelihood of fatal and non-fatal drug overdose events. Considering the burden of metabolic dysfunction-associated steatotic liver disease (MASLD) in individuals with type 2 diabetes (T2D), this study aimed to identify factors associated with MASLD among adults with T2D. Background: Diuretics are used to manage hypertension and heart failure; however, the data on their safety during pregnancy are limited. Objective: To examine contemporary trends in diuretic use among pregnant women with cardiovascular conditions in the United States from 2010 to 2023. Methods: We conducted a population-based study using two large U.S. administrative claims databases; Merative MarketScan (commercial) and Medicaid Analytic eXtract (public) from 2010 to 2023. Age-related decline in oocyte quality increases the risk of infertility, miscarriage, and birth defects. Mitochondrial dysfunction is a key contributor to this decline. Here, we report that oocyte-specific deletion of Uba3, which encodes the catalytic subunit of the E1 NEDDylation-activating complex, causes sterility in mice. Fully grown, germinal vesicle–stage Uba3 conditional knockout oocytes exhibit mitochondrial dysfunction, including elevated reactive oxygen species, impaired oxidative phosphorylation, and depletion of mitochondrially encoded RNA transcripts. Proteomic analysis identified alterations in mitochondrial-associated proteins, including enrichment of mitochondrial matrix and respiratory chain components and reduced abundance of electron transport chain complexes. Objectives: Oral cavity head and neck squamous cell carcinoma (HNSCC) represents a major global health burden, with over 40,000 new cases annually in the United States and >500,000 worldwide. Despite advances in surgery, radiation therapy, chemotherapy, and immunotherapy, overall survival rates have remained largely stagnant over the past three decades, and over half of patients ultimately die from the disease. Although immune checkpoint inhibitors (ICIs) have improved outcomes for a subset of patients with recurrent or metastatic HNSCC, the majority gain limited benefit. Wnt proteins are lipid-modified morphogens fundamental in development and disease. During Wnt biogenesis, the G-protein-coupled receptor (GPCR)-like transporter Wntless (WLS) escorts lipidated Wnts from the endoplasmic reticulum to the plasma membrane, then transfers them to extracellular carriers, forming active and soluble morphogen-carrier complexes. To dissect the mechanisms involved, we solve cryo-EM structures of Wnt-bound WLS and unliganded WLS, and perform structure-guided functional experiments. Wnts engage WLS via three conserved hairpins, which are all required for Wnt trafficking to the cell surface and carrier-mediated secretion. Mitochondria are hubs of metabolism and signaling. We previously demonstrated the importance of mitochondrial structure and function in chemotherapy-refractory triple-negative breast cancer (TNBC). Herein, we present the first 3D analysis of mitochondrial networks in human tumor tissues. Using serial block face scanning electron microscopy, we reconstructed 3,750 mitochondria and 800 lipid droplets (LDs) in naive and residual tumors persisting after conventional chemotherapies in two orthotopic patient-derived xenografts (PDX). Chemotherapies administered as monotherapy or in combination produced residual tumors that harbored mitochondria with significantly increased areas, volumes, and perimeters. Aggressive-variant prostate cancers (AVPCs) respond poorly to anti-androgen therapy but show sensitivity to taxane-platinum chemotherapy, though outcomes remain poor. We conducted a phase 2 trial testing induction cabazitaxel plus carboplatin (CabCarb) followed by olaparib maintenance versus observation in men with AVPC. The primary endpoint of improved progression-free survival (PFS) was not met, likely due to the study being underpowered after 38.5% of patients experienced early progression (ChemoPD) prior to randomization. No genomic alterations predicted ChemoPD; however, transcriptomic analysis revealed the enrichment of metabolic pathways, including arginine metabolism, in ChemoPD tumors. These findings were supported by metabolomics data from preclinical models. …

Background: Drug importation plays a critical role in Thailand’s healthcare system; however, barriers affecting its regulatory control and their public health implications remain inadequately examined. This study compared international drug importation procedures and evaluated Thailand’s control system with strategies for improvement.
Methods: Conducted in Bangkok between 2020 and 2023, this two-phase study employed a qualitative design. Phase 1 involved content analysis comparing drug importation procedures across Thailand, Singapore, Australia and the United States to identify best practices and potential solutions. Background: Ophthalmic diseases are a significant public health concern globally and in Uzbekistan, where approximately 1 million individuals are affected.
Objectives: The objectives are to conduct an assortment analysis of ophthalmic medicines registered in Uzbekistan (2010–2024), to identify trends in drug registration, composition, dosage forms, and manufacturing sources.
Materials and Methods: Based on State Register data, medicines classified under Anatomical Therapeutic Chemical S01 were analyzed using marketing and qualimetric methods, focusing on the International Nonproprietary Names, dosage forms, manufacturers, and countries of origin.

Background: Acute gout arthritis, which is caused by hyperuricemia, is a common inflammatory joint disease, leading to monosodium urate (MSU) crystal deposition. Wastewater treatment is necessary for environmental conservation and combating water pollution. This study demonstrates real-time dissolved oxygen (DO) monitoring in constructed wetland treatment. The system would ensure an adequate supply of oxygen through automated water circulation whenever the level is below 4 mg/L, and shut down when it reaches the normal level. From experiments conducted, it was observed that within three automated circulation processes, it is possible to increase DO concentrations from 0.5 mg/L to 5.0 mg/L.
