Background: Pharmacists practice within complex legal, regulatory, and ethical frameworks; however, PharmD curricula often offer limited direct exposure to regulatory decision-making in authentic practice settings. ackground: Motivational Interviewing (MI) involves interviewing a person in an autonomy-supporting, person-centered, encouraging way, eliciting the individuals internal change motivations. Thirteen modified Cochrane method (MCM) systematic reviews of MI impact on outcomes included varied populations, interventionist types, and target outcomes with a lens for whether MI training of interventionists was reported and/or met evidence-based MI training guidelines. Objectives: To assess the 1-year risk of all-cause hospitalization or mortality associated with initiation of sodium glucose cotransporter-2 inhibitors (SGLT2i) compared with renin angiotensin system inhibitors (RASi) among Medicare beneficiaries with heart failure (HF) and Alzheimers disease and related dementias (ADRD). Methods: This cohort study used 5% 2012-2020 Medicare fee-for-service claims data to identify beneficiaries with HF and ADRD who newly initiated SGLT2i or RASi between January 1, 2013, and December 31, 2019. Baseline demographics, comorbidities, and medication use were included as covariates. OBJECTIVES: To estimate healthcare expenditures associated with each CKM syndrome stage and to quantify incremental expenditures for each stage compared to stage 0. METHODS: A retrospective cross-sectional study was conducted using pooled data from adults (18 years) in the 2018, 2020 and 2022 Medical Expenditure Panel Survey (MEPS). CKM syndrome stages were defined based on the American Heart Association (AHA) staging construct using International Classification of Diseases, Tenth Revision (ICD-10). Background: Initial opioid prescribing for acute pain contributes substantially to opioid exposure and can drive persistent use, increasing the risk of overdose and opioid use disorder. Most U.S. states have implemented opioid prescribing limits (OPLs) to restrict initial opioid prescription duration, quantity, or dosage. However their intended and unintended impacts remain poorly understood. Background: Women veterans with HIV represent a growing, diverse population within the Veterans Health Administration (VHA). This study examined HIV care continuum outcomes among women veterans using a multilevel intersectional approach.
Methods: We conducted a retrospective analysis of 1154 women veterans with HIV in VHA care in fiscal year 2022, with outcomes assessed in fiscal year 2023. We applied Multilevel Analysis of Individual Heterogeneity and Discriminatory Accuracy (MAIHDA) to evaluate how combinations of social positions (age, race/ethnicity, housing status) were associated with HIV outcomes.
Results: MAIHDA models showed that younger age (< 45 years) and being unhoused were consistently associated with lower odds of care engagement and viral suppression than midlife (45-64) and older (65+) housed women veterans. Additionally, predicted probability analyses revealed distinct clustering patterns. Younger non-Hispanic White women consistently ranked among the lowest performing strata across all outcomes, while midlife and older Hispanic and non-Hispanic Black women veterans clustered among the highest. Variance Partition Coefficients from null models were modest (1.8-3.0%). Fully adjusted models showed no remaining between-stratum variance, suggesting that the included social positions explained the observed differences in our dataset.

Musculoskeletal injury risk in midlife women has traditionally been attributed to estrogen decline, but this narrow focus overlooks the broader biomechanical, neuromuscular, and structural changes associated with menopause. Estrogen plays a multisystem role in maintaining trabecular bone integrity, collagen turnover, and neuromuscular coordination. Its abrupt loss causes bone demineralization (2%-5% annually in early menopause), impairs tendon repair, and contributes to sarcopenia. Fluctuating estradiol levels during perimenopause may transiently increase ligament laxity, further destabilizing joints. Common injuries such as Achilles and rotator cuff tendinopathies, plantar fasciitis, and stress fractures are amplified by low bone mineral density, prior fracture history, aromatase inhibitor …

Objectives: Opioid tapering has increased in recent years with the implementation of national prescribing guidelines. This study aimed to examine the association of opioid tapering with takotsubo cardiomyopathy (TCM) and major adverse cardiovascular events (MACE) in older Medicare beneficiaries. Methods: A case-crossover design was employed using 5% Medicare administrative claims data (2012-2020). Older adults (65 years) with TCM or MACE (composite of myocardial infarction, stroke, or cardiovascular death) were identified, and the date of the first occurrence was designated as the index date. Background: Type 2 diabetes is a common chronic condition in the United States and requires ongoing follow-up to monitor glycemic control, review laboratory results, adjust medications, and address self-management needs. Telehealth has become an increasingly common component of diabetes care, yet its role in routine follow-up remains underexamined. In Indonesia, lung cancer is the leading cause of death related to cancer, according to Global Cancer Observatory. Chemotherapy is the most common treatment since it works systemically and can prevent cancer cells from spreading to other organs. Cytotoxicity of chemotherapy can also put patients at high risk of experiencing adverse effects. This study aimed to evaluate the congruity of chemotherapy use with treatment recommendations and the profile of adverse effects, not only acute but also delayed adverse drug reactions. State public pension systems manage over $6 trillion in assets while facing approximately $1.3 trillion in unfunded liabilities, raising fundamental questions about governance effectiveness and fiduciary accountability. This Article examines these challenges through the analytical framework of governance, applying insights from public choice, principal-agent models, and corporate governance to argue that details in implementation significantly impact outcomes. We offer Ohio’s State Teachers Retirement System (STRS) as a case study. Recent governance reforms in Ohio—including legislative restructuring of the STRS board and enactment of fiduciary standards legislation modeled on the American Legislative Exchange Council’s framework—suggest the potential for earlier governance problems. …

Analyzing Retirement Systems Through A Triad Framework: Lessons For U.S. Reform, Romina Boccia, Ivane Nachkebia

As U.S. Social Security approaches insolvency, this paper analyzes the program’s challenges through a political economy lens and makes two new contributions. First, it introduces a normative triad framework for evaluating retirement systems based on individual autonomy, fiscal sustainability, and poverty alleviation. Second, it applies this framework to five advanced economies—Canada, Germany, New Zealand, Sweden, and the U.S.—to identify institutional features associated with differences in outcomes. Deriving implications for U.S. reform, we propose four recommendations: transition Social Security to a flat-benefit system, slow benefit growth, adopt automatic stabilizers, and expand flexible savings options.

Teacher retention has reached crisis levels nationwide, with nearly 50% of beginning teachers leaving within five years. Teacher attrition increased to 27.3% in 2022–2023 in a large suburban school district in North Texas. This translated to over $1.1 million dollars in teacher salary expenditures alone. This Dissertation in Practice utilized mixed-methods research grounded in Improvement Science and Plan-Do-Study-Act cycles to explore five campus-level contributors to teacher retention that fell under the control of building-level administrators: job satisfaction, administrative support, professional development, campus culture, and student behavior supports. Background
Hepatitis B virus (HBV) establishes persistent infection through the formation of a covalently closed circular DNA (cccDNA) minichromosome, which enables immune evasion and sustained viral replication. While SIRT1, IFI16, and STING are known to modulate antiviral pathways, their cooperative role in regulating HBV transcription and host immunity remains poorly defined.
Methods
We investigated the functional interplay between SIRT1, IFI16, and STING using confocal microscopy, chromatin immunoprecipitation (ChIP), gene silencing, and quantitative PCR in HBV-replicating cell models. Teachers across the United States are experiencing declining psychological well-being due to increasing job demands and insufficient workplace resources. In Arkansas, this imbalance contributes to burnout, attrition, and persistent teacher shortages, affecting not only educators but also students, schools, and communities. Job resources are positively associated with well-being, whereas job demands are negatively associated with well-being. The problem addressed in this study is that the effects of Social Emotional Learning (SEL) competencies on academic achievement for virtual learners in a Grades 5-8 classroom are not well known.
