Problem: Workplace incivility (WI), lateral violence (LV), and bullying in healthcare settings are pervasive issues negatively impacting staff morale, turnover rates, and patient care outcomes. High turnover and low employee satisfaction scores from a September 2023 culture of safety survey signaled a toxic work environment characterized by ineffective communication and unprofessional behavior. Context: A 24-bed telemetry-stroke unit at a 172-bed acute care facility in the North Bay area of San Francisco.

Interventions: The Transtheoretical Model (TTM) of behavior change and the Crucial Conversation frameworks were used to improve staff communication and reduce incivility.

Problem: Hospital-associated disability (HAD) affects one-third of hospitalized adults, diminishing post-discharge independence and contributing to high mortality rates. Identifying at-risk patients through early assessment of nutritional and functional status upon admission is critical to personalizing care and improving discharge outcomes.

Context: A 24-bed adult medical-surgical unit in a 173-bed suburban hospital in Northern California is part of a large not-for-profit healthcare organization. Interventions: Nursing staff received education on the Enhanced Recovery Medicine (ERM) pathway designed to prevent HAD. Background: Cigarette smoking is the leading cause of preventable death in the United States. Although smoking rates have been declining, it is unclear how the COVID-19 pandemic impacted smoking behaviors. Methods: With population-based data from the Behavioral Risk Factor Surveillance System (2010–2023), we will analyze four outcomes: (1) currently smoking, (2) started smoking in the past year, (3) number of cigarettes per day, and (4) quit smoking in the past year. Although fasting is increasingly applied for disease prevention and treatment, consensus on terminology is lacking. Using Delphi methodology, an international, multidisciplinary panel of researchers and clinicians standardized definitions of various fasting approaches in humans. Five online surveys and a live online conference were conducted with 38 experts, 25 of whom completed all 5 surveys. A key challenge in aging research is to extend lifespan in tandem with slowing down functional decline so that the life with good health (healthspan) can be extended. Here, we show that monthly clearance of a small number of cells, which highly express p21Cip1 (p21high), starting from 20 months improves cardiac and metabolic function, and extends both median and maximum lifespan in mice. Importantly, by assessing health and physical function of these mice monthly until death, we show that clearance of p21high cells improves physical function at all remaining stages of life, suggesting healthspan extension. Inflammation biomarkers can provide valuable insight into the role of inflammatory processes in many diseases and conditions. Sequencing based analyses of such biomarkers can also serve as an exemplar of the genetic architecture of quantitative traits. To evaluate the biological insight, which can be provided by a multi-ancestry, whole-genome based association study, we performed a comprehensive analysis of 21 inflammation biomarkers from up to 38 465 individuals with whole-genome sequencing from the Trans-Omics for Precision Medicine (TOPMed) program (with varying sample size by trait, where the minimum sample size was n = 737 for MMP-1). INTRODUCTION: People with HIV (PWH) have an increased risk of atherosclerotic cardiovascular disease (ASCVD) compared to non-PWH, but the reasons for this increased risk remain elusive. We investigated the change in ASCVD risk scores over 4 years to identify clinical factors associated with change in risk scores or high-risk scores.

METHODS: We conducted a preliminary study using retrospective analysis of PWH, between 40 and 75 years old, seen at the Evelyn Jordan Center with at least two routine HIV visits. International students are an important component of the host country and universities bringing economic, social, and community contributions. International students are the most vulnerable students with challenges related to academic expectations, language proficiency, and socio-cultural integration. This study explores the challenges of international students in a Western Australian School of Nursing and Midwifery. An exploratory questionnaire (n=10), three focus groups (n=15) and a quality improvement survey (n=80) informed this study. Four themes emerged: stress in the first weeks, incorrect and/or late enrolments, lack of guidance and support, and system navigation nightmares. Essays On Aging, Health, And Labor Market, Lanjie Wang

This dissertation consists of four chapters on aging, health, and labor economics.

In the first chapter, we investigate the short-term impacts of unemployment on older workers in Singapore, a setting without public unemployment insurance. Using monthly panel data, we analyze dynamic impacts on income, spending, health, and subjective well-being over two years post-unemployment. We find substantial initial earnings losses with incomplete recovery, with income remaining 46.5% below pre-unemployment levels after 24 months. Despite this persistent income gap, consumption responses are modest and comparable to countries with strong social safety nets, suggesting robust self-insurance mechanisms. We observe increased retirement among married …

Type 2 diabetes mellitus (T2DM) is common and increasing in prevalence worldwide, with devastating public health consequences. While peripheral insulin resistance is a key feature of most forms of T2DM and has been investigated for over a century, research on brain insulin resistance (BIR) has more recently been developed, including in the context of T2DM and non-diabetes states. Recent data support the presence of BIR in the aging brain, even in non-diabetes states, and found that BIR may be a feature in Alzheimer's disease (AD) and contributes to cognitive impairment. This report presents the results of a comprehensive examination of issues relating to aging in Newton. A needs assessment was undertaken to support planning on the part of the City of Newton and the Department of Older Adult Services. Results presented here focus on the characteristics and needs of Newton residents who are aged 60 and older. Data was collected via key informant interviews, focus groups with residents and stakeholders and a mailed survey of residents aged 60 and older. This research focuses on thwarted belonging (TB), its associations with the neighborhood contextual factors of social cohesion and physical disorder in older adults, and the moderating role of neuroticism. TB is a key component of the Interpersonal Psychological Theory of Suicide (IPTS) that has serious implications for health and well-being (Van Orden et al., 2012). Retirement represents a significant life transition typically occurring in later adulthood, often accompanied by substantial lifestyle changes. Several theoretical frameworks suggest that these changes present both opportunities and challenges for well-being, and the extent to which individuals experience positive versus negative well-being outcomes may be influenced by various factors. To study such heterogeneity in retirement experiences, researchers have embraced person-centered methodologies. Yet, some previous studies have not robustly delineated retirement- from age-related changes in well-being, accounted for statistical uncertainties, or examined these diverse experiences outside of a Western context. Depression is a highly prevalent mental disorder in older adults, and among its highest risk factors is loneliness. Although this relationship is commonly evident, the mechanisms underlying it, such as sense of mattering and psychological adjustment, lack exploration. This paper examines the political constructions of people who use drugs in the Philippines throughout the presidency of Rodrigo Duterte (2016–2022), during which the government engaged in a ‘war on drugs’ and promoted a punitive drug regime.

Methods

Building on and drawing inspiration from the global drug policy scholarship that has looked at the ways in which drugs are framed and problematised in various domains, this study used qualitative content analysis to review 96 documents from national government agencies - including strategic action plans, directives, memorandums, guidelines, annual reports, and legislative measures.

Depression is one of the most common psychiatric concerns in older adults. Because of the intersection of biological, emotional, social, and cognitive factors that are at play in depression, symptoms are often heterogeneous in presence and severity. In combination with the expected changes in cognitive abilities as one ages, late-life depression can be additive to these changes. Adults with intellectual and developmental disabilities (IDD) are often dependent on direct care staff to engage in daily activities, such as personal care, eating, and engaging in leisure activities. One way direct care staff can support adults with IDD to become more independent is by using prompting strategies. Often times, however, direct care staff have not been trained on how to use prompts to encourage independence. This data brief presents an analysis of available claims data from New Hampshire’s commercial, Medicaid, and Medicare insured populations. Specifically, the study identifies the top ten most prevalent health conditions, the cost of treatment for those conditions, and the overall medical cost for those with one of the identified conditions. The analysis is based on the most recent calendar year of available claims data (January – December 2022).

Although prior literature has established the preliminary clinical benefits of aquatic exercise across various clinical populations, there is a need to explore the potential for including aquatic-based movement as a key component of a multi-modal approach toward improving mobility and reducing fall risk. Eccentric exercise may complement aquatic exercise given that movements performed in water immersion tend to be low impact. Eccentric exercise may also improve the passive stiffness of lower extremity extensors, which gives an additional rationale for the potential of eccentric exercise to reduce fall risk when combined with aquatic exercise. This dissertation investigates the utilization of telehealth services, initially focusing on the Arkansas healthcare system and then extending the analysis nationwide. It aims to understand the factors influencing telehealth adoption and its impact on healthcare delivery. After examining telehealth utilization in Arkansas from 2018 to 2022, the research utilizes a comprehensive dataset from Epic Cosmos, which includes a wide range of patient and visit data from multiple healthcare facilities across the United States from 2018 to 2023. Objectives: In ovarian and other cancers, low muscle mass and density are associated with poorer clinical outcomes. However, screening for cancer-related sarcopenia (typically defined as low muscle mass) is not routinely conducted. The European Working Group on Sarcopenia in Older People (EWGSOP) recommends an algorithm for sarcopenia screening and diagnosis in clinical settings, with sarcopenia based on muscle strength and mass, and severity on physical performance. We explored the application of the EWGSOP2 algorithm to assess sarcopenia in six ovarian cancer patients receiving neoadjuvant chemotherapy. Apolipoprotein ε4 (APOE ε4) may be a genetic risk factor for reduced bone mineral density (BMD) and muscle function, which could have implications for fall and fracture risk. We examined the association between APOE ε4 status and long-term fall- and fracture-related hospitalization risk in older women. A total of 1 276 community-dwelling women from the Perth Longitudinal Study of Aging Women (mean age ± SD = 75.2 ± 2.7 years) were included. At baseline, women underwent APOE genotyping and detailed phenotyping for covariates including prevalent falls and fractures, as well as health and lifestyle factors. Trichotillomania (TTM) is associated with impairments in response inhibition and cognitive flexibility, but it is unclear how such impairments relate to treatment outcome. The present study examined pre-treatment response inhibition and cognitive flexibility as predictors of treatment outcome, change in these domains from pre-to post-treatment, and associations with TTM severity. Participants were drawn from a randomized controlled trial comparing acceptance-enhanced behavior therapy (AEBT) to psychoeducation and supportive therapy (PST) for TTM.
