Preoperative Hypoalbuminemia Predicts 30-Day Complications In Head And Neck Microvascular Surgery, James R. Xu, Anish Kosanam, Khashayar Arianpour, Eric D. Lamarre, Christopher G. Hyland, Peter J. Ciolek

Student Scholarship

Introduction: Hypoalbuminemia, a marker for poor nutritional status, has been associated with postoperative complications, including head and neck cancer surgery. This study investigates the impact of hypoalbuminemia on head and neck microvascular free tissue transfer reconstruction. Methods: This retrospective cohort study queried the 2005–2021 American College of Surgeons National Surgical Quality Improvement Program databases. Reconstructive cases performed by otolaryngologists (CPT: 15756, 15757, 15758, 15842, 20955, 20956, 20957, 20962, 20969, 20970, 20972, 20973, 43116, 43496, 49006, and 49906) with available preoperative albumin, BMI, and age were included. Hypoalbuminemia was defined as a preoperative albumin < 3.5 g/dL. Univariate and multivariable logistic regression were performed. Results: A total of 3,886 cases met the inclusion criteria, of which 835 (21.5%) had hypoalbuminemia. The hypoalbuminemia cohort was older, had lower BMI, had higher ASA classification, and had worse functional health status. Adjusted multivariable logistic regression showed that hypoalbuminemia was associated with unplanned return to the operating room within 30 days (OR: 1.36, p < 0.01), unplanned reoperation (OR: 1.36, p < 0.01), any complication (OR: 1.77, p < 0.01), surgical complications (OR: 1.94, p < 0.01), and medical complications (OR: 1.34, p = 0.01). Hypoalbuminemia was correlated with a longer hospital stay, superficial surgical site infection, wound dehiscence, transfusion, deep vein thrombosis, and acute renal failure. Conclusion: Hypoalbuminemia is a risk factor for postoperative complications after microvascular free tissue transfer for head and neck reconstruction. This study suggests that preoperative optimization of hypoalbuminemia may be beneficial for these patients. Level of Evidence: 3 Laryngoscope, 135:648–656, 2025.

Making Waves: Comfort Women Justice Movements Across Generations, Julia A. Jacobellis

Dissertations, Theses, and Capstone Projects

Between 1932 and 1945 an estimated 200,000 women across the globe were coerced into becoming comfort women for the Japanese military. As comfort women they were forced to prostitute themselves to soldiers, and endured physical, psychological and emotional torture. While comfort stations were liberated in 1945 with the end of World War II, the Japanese government has yet to formally apologize for their treatment of comfort women, and has failed to meet the demands of survivors and their families who seek reconciliation and justice. The global leaders of these movements for reconciliation are institutions and organizations in South Korea, China, …

Relationship Between Non-Affirmation Of Gender Identity, Healthcare Stereotype Threat, And Perceived Employment Inequities, Len M. Meyer

Walden Dissertations and Doctoral Studies

The transgender and gender-diverse (TGD) population faces barriers when seeking healthcare and perceived employment inequities for simply identifying as their gender identity. This study examined the relationships between non-affirmation of gender identity, healthcare stereotype threat, and perceived employment inequities for TGD people across the United States. The purpose of this quantitative, retrospective, quasi-experimental study was to determine if there was a relationship between non-affirmation of gender identity, healthcare stereotype threat, and perceived employment inequities. This study was guided by the sexual citizenship theory devised by T.H. Marshall, which examines three elements of the model: (a) civil society, (b) political society, …

Since ChatGPT was introduced in November 2022, embedding (nearly) unnoticeable statistical signals into text generated by large language models (LLMs), also known as watermarking, has been used as a principled approach to provable detection of LLM-generated text from its human-written counterpart. In this paper, we introduce a general and flexible framework for reasoning about the statistical efficiency of watermarks and designing powerful detection rules. Inspired by the hypothesis testing formulation of watermark detection, our framework starts by selecting a pivotal statistic of the text and a secret key-provided by the LLM to the verifier-to control the false positive rate (the …

Transcriptome Size Matters For Single-Cell Rna-Seq Normalization And Bulk Deconvolution, Songjian Lu, Jiyuan Yang, Lei Yan, Jingjing Liu, Judy Jiaru Wang, Rhea Jain, Jiyang Yu

Faculty, Staff and Student Publications

The variation of transcriptome size across cell types significantly impacts single-cell RNA sequencing (scRNA-seq) data normalization and bulk RNA-seq cellular deconvolution, yet this intrinsic feature is often overlooked. Here we introduce ReDeconv, a computational algorithm that incorporates transcriptome size into scRNA-seq normalization and bulk deconvolution. ReDeconv introduces a scRNA-seq normalization approach, Count based on Linearized Transcriptome Size (CLTS), which corrects differential expressed genes typically misidentified by standard count per 10 K normalization, as confirmed by orthogonal validations. By maintaining transcriptome size variation, CLTS-normalized scRNA-seq enhances the accuracy of bulk deconvolution. Additionally, ReDeconv mitigates gene length effects and models expression variances, …

Immune checkpoint inhibitors can lead to 'exceptional', durable responses in a subset of persons. However, the molecular basis of exceptional response (ER) to immunotherapy in metastatic clear cell renal cell carcinoma (mccRCC) has not been well characterized. Here we analyzed pretherapy genomic and transcriptomic data in treatment-naive persons with mccRCC treated with standard-of-care immunotherapies: (1) combination of programmed cell death protein and ligand 1 (PD1/PDL1) and cytotoxic T lymphocyte-associated protein 4 inhibitors (IO/IO) or (2) combination of PD1/PDL1 and vascular endothelial growth factor (VEGF) receptor inhibitors (IO/VEGF). In the IO/IO cohort, clonal neoantigen load was significantly higher in persons with …

Although we are all familiar with the sensation of fatigue, there are still profound divergences on what it represents and its mechanisms. Fatigue can take various forms depending on the condition in which it develops. Cancer-related fatigue is considered a symptom of exhaustion that is often present at the time of diagnosis, increases in intensity during cancer therapy, and does not always recede after completion of treatment. It is usually attributed to the inflammation induced by damage-associated molecular patterns released by tumor cells during cancer progression and in response to its treatment. In this review, we argue that it is …

Heat shock protein 90 (HSP90), a vital chaperone involved in the folding and stabilization of various cellular proteins, regulates key functions in many tumor cells. In the context of gastric adenocarcinoma (GAC), where HSP90's role remains largely unexplored, we aimed to investigate the significance of HSP90 inhibitor, AUY922, in regulating the YAP1/TEAD pathway and its association with the tumor immune microenvironment (TME). Our results showed that AUY922 effectively inhibited GAC aggressiveness in both the invitro and invivo models, induced apoptosis, and cell-cycle arrest. Various functional assays elucidated that AUY922 potently inhibited the expression and interaction among YAP1/TEAD and HSP90, resulting …

The Role Of Double-Zero-Event Studies In Evidence Synthesis: Evaluating Robustness Using The Fragility Index, Zelin Wang, Xing Xing, Eun-Young Mun, Chong Wu, Lifeng Lin Feb 2025

The Role Of Double-Zero-Event Studies In Evidence Synthesis: Evaluating Robustness Using The Fragility Index, Zelin Wang, Xing Xing, Eun-Young Mun, Chong Wu, Lifeng Lin

Faculty, Staff and Student Publications

Rationale: Zero-event counts are common in clinical studies, particularly when assessing rare adverse events. These occurrences can result from low event rates, short follow-up periods, and small sample sizes. When both intervention and control groups report zero events in a clinical trial, the study is referred to as a double-zero-event study, which presents methodological challenges for evidence synthesis. There has been ongoing debate about whether these studies should be excluded from evidence synthesis, as traditional two-stage meta-analysis methods may not estimate an effect size for them. Recent research suggests that these studies may still contain valuable clinical and statistical information. …
