This study examines factors shaping digital transformation in local government and the mechanisms through which these factors influence implementation outcomes. Using a scoping review design, the study maps recent empirical and conceptual literature to capture the breadth and diversity of evidence. A systematic search was conducted in Scopus, Springer, and EBSCOhost using the keywords “digital transformation” AND “local government”, limited to English-language publications published between 2021 and 2025. The search identified 246 records, of which 22 studies met the inclusion criteria and were included in the final analysis. Background: Bladder cancer (BC) symptoms often overlap with benign conditions, while no routine screening exists for general population. We aim to develop a machine learning (ML)-based screening pipeline for early BC detection using electronic-health-records (EHRs) in primary care. Methods: A multi-centred case-control cohort (1995–2018; n = 64,884) was created for model training and testing. We further validated the model prospectively on an independent cohort (2019–2020; n = 4,569). In Fiji, physicality and hegemonic masculinity are largely defined by rugby and the military, institutions dominated by Indigenous Fijian men, who are predominantly Christian, often marginalising other races, genders, religions, and sports. While research in the Global South has examined intersections of race and gender, little is known about how masculinity is understood across different sports, sporting icons, and contexts—particularly from the perspective of South Asian migrant women in Oceania with hybrid identities. Mexican American caregivers of individuals with Alzheimer’s Disease face unique stressors and challenges, which may be influenced by cultural, socioeconomic, and systemic factors. This population often assumes caregiving responsibilities based on strong cultural values of familismo which emphasizes family duty and collective care. However, this commitment can lead to increased emotional strain, mental health concerns, financial and physical strain, particularly when caregivers have limited access to healthcare resources, financial support, or respite. Just over half of New Hampshire residents support extending the recently expired Obamacare subsidies and nearly half of residents would support instituting a national "single-payer" health care system. Granite Staters are divided along partisan lines on whether to expand or repeal Obamacare. Most Granite Staters say that medical costs have increased in the past year and nearly one-third say someone in their household has postponed getting health care due to cost. A majority disapprove of President Trump's handling of healthcare and Granite Staters are even more negative about the performance of Congress on the issue.

Over the past half-century, rapid technological change and globalization have reshaped innovation and intellectual property, yet the foundational international agreements anchoring the global patent system have remained essentially unchanged. The drafters of the Paris Convention for the Protection of Industrial Property and the Patent Cooperation Treaty (PCT) viewed these agreements as dynamic instruments that, through procedural coordination, would rationalize international patent administration. The Paris Convention has resisted reform for decades, however, while the PCT has failed to achieve its goals of reducing examination workloads and fostering worksharing among national patent offices. BACKGROUND AND OBJECTIVES: The onset of hearing loss due to vestibular schwannomas (VS) is inevitable but does not correlate with the size of the tumor. In patients with Neurofibromatosis Type 2 (NF2) and VS, we previously found an association between pre-contrast fluid-attenuated inversion recovery magnetic resonance imaging (FLAIR MRI) signal in the labyrinth and hearing loss. Here, we asked whether FLAIR hyper-intensity could serve as a predictive biomarker for hearing loss in NF2 patients with VS.

Introduction Solid pseudopapillary epithelial neoplasm (SPEN) is a rare pancreatic tumor that typically presents in adult women and is usually asymptomatic. Case presentation A 12-year-old male presented to the emergency department with worsening abdominal pain one day after suffering mild abdominal trauma during a soccer match. Physical exam showed diffuse abdominal tenderness and guarding. CT abdomen with IV contrast showed a heterogeneous, round structure adjacent to the pancreatic tail with mass effect upon the spleen. Background: Timely reperfusion is critical in reducing mortality in ST-segment elevation myocardial infarction (STEMI). Although electrocardiography-guided cardiac catheterization laboratory (CCL) activation on the basis of first medical contact recognition improves system-level response, diagnostic uncertainty, particularly in atypical presentations, contributes to false positive activations (FPAs) and reperfusion delays.

Objectives: The aim of this study was to evaluate the diagnostic performance and operational impact of artificial intelligence (AI)-based electrocardiographic (ECG) analysis in real-world STEMI triage across a multicenter U.S. registry.

Vermonters strongly disapprove of President Trump and Congress's handling of health care. Nearly half of state residents say that paying for overall medical expenses is difficult and two-thirds say that their costs have increased in the past twelve months.

BACKGROUND: Due to widespread shortages of pediatric intensive care unit (PICU) beds during the 2022–2023 respiratory virus surge, a protocol was implemented to identify and manage patients on high-flow nasal cannula (HFNC) in a pediatric observation unit (POU). This study aims to describe the characteristics and outcomes of patients who received HFNC in the observation unit.

Biallelic deficiencies of CD27 and its ligand CD70 underlie selective susceptibility to Epstein-Barr virus (EBV) infection and its acute and chronic complications, underscoring their non-redundant roles in anti-EBV immunity. To date, 16 pathogenic CD27 variants have been reported. Here, we describe three patients from two unrelated families, homozygous for a novel loss-of-function (LOF) CD27 variant, resulting in substitution of serine 70 with proline (S70P). All three patients presented with EBV viremia and lymphoproliferative disease, with variable immune dysregulation or recurrent otosinopulmonary infections. One patient developed EBV-associated Hodgkin lymphoma. Functional studies demonstrated that the S70P variant impaired surface expression of CD27 …

Oil Palm fronds (OPF) as the main waste of oil palm plantations have considerable promise as a small ruminant feed. Unfortunately, due to the diversity of previous findings, a clear response pattern has still to be determined. For this reason, in the present study, meta-analysis was employed to synthesize existing findings regarding the effect of OPF as a small ruminant feed. The database was constructed using data from ten published articles that fit the inclusion criteria. The applied statistical model was a mixed model. Tujuan penelitian untuk mengetahui spesifikasi ukuran morfometrik bagian kepala dan telinga antar bangsa kelinci New Zealand, Rex dan Flemish Giant dan menduga jarak genetik antar bangsa kelinci. Penelitian dilaksanakan tanggal 1 Agustus – 28 Oktober 2021 di daerah Jawa Tengah (Kabupaten Semarang, Kendal, Temanggung, Magelang) dan Yogyakarta. Penelitian ini bertujuan untuk mengetahui kecernaan bahan kering, bahan organik, dan kandungan VFA, serta mengetahui persentase penggunaan limbah kubis sebagai sumber protein terbaik pada silase ransum komplit berbasis limbah tebu secara in vitro. Metode penelitian yang digunakan yaitu Rancangan Acak Kelompok yang terdiri 4 perlakuan dan 4 kelompok pengambilan cairan rumen sebagai ulangan. Recent advances in agentic artificial intelligence have been driven primarily by scale: larger models, increased data, and expanding computational resources. However, rising energy costs, inference latency, and hardware constraints increasingly challenge this trajectory. This paper argues that intelligence—biological or artificial—does not primarily scale through raw computational expansion, but through the management of exchange under constraint. Drawing on cognitive science, systems theory, and prior work on exchange-based models of intelligence, the paper proposes a theoretical framework in which agentic intelligence scales through context management, proceduralization, and the assembly of reusable units of exchange. When undergraduate EFL students engage in authentic research, they begin to see themselves not merely as language learners but as legitimate researchers, a transformation that reshapes their academic belonging. This study investigates how authentic undergraduate research experiences influence English as a Foreign Language (EFL) students’ sense of belonging in a Saudi higher-education context. Using a mixed-methods design, data were collected from 122 female undergraduate EFL students enrolled in a Research Methods course. Objective and Methods Driven by the goal of carbon neutrality, geologic CO2 storage has become a critical national priority in China. Compared to conventional reservoirs in sedimentary basins, such as saline aquifers, depleted hydrocarbon reservoirs, and unmineable coal seams, basaltic formations offer more significant advantages in geologic CO2 storage. Specifically, basaltic formations can form stable carbonates through reactions between CO2 and rock minerals while also exhibiting extensive distribution, considerable potential for CO2 storage, and low risk of CO2 leakage. Insomnia, The Witch, And The Dark Passenger As Shadow Projections In Joharah Al-Remal's Al-Awsaj, Dr. Afaf Al-Enezi

This study investigates how psychic repression and identity fragmentation are represented in Joharah Alremal’s Al-Awsaj (2023), asking how inherited trauma and the symbolic Dark Passenger shape the protagonist’s fractured selfhood. It aims to clarify the mechanisms through which insomnia, shadow projection, and the witch’s prophecies externalize unresolved familial wounds. Methodologically, the paper employs a psychoanalytic close reading informed by Jungian theory, situating the novel in comparative dialogue with Jeff Lindsay’s Darkly Dreaming Dexter (2004).
