Pertussis In Infancy: A Case Of Delayed Diagnosis, Ana Hernandez-Puga

Bordetella pertussis is a gram-negative bacterium that primarily infects the respiratory tract. We report the case of a 13-month-old male patient who developed pertussis. At the initial clinic presentation, the diagnosis was not immediately considered, as the early manifestations closely mimicked those of a viral upper respiratory infection. As the illness progressed, the classic paroxysmal cough with inspiratory "whoop" became apparent, prompting the correct diagnosis. At that time, a nasopharyngeal swab was obtained, which tested positive for Bordetella pertussis by polymerase chain reaction (PCR). The patient was prescribed a five-day course of azithromycin, and close contacts were given prophylactic treatment. …

Background
Languages vary in their syntactic (e.g. word order in sentences), lexical (e.g. presence of specific word classes), and morphological (e.g. inflectional or derivational forms of words) properties. This cross-linguistic variation in language typology influences the manifestation of agrammatic symptoms. However, most theoretical models of agrammatism are based on English and a few European languages, limiting their generalizability. This narrow focus neglects the rich syntactic, lexical and morphological variations available in languages globally. Clinically, the lack of language-specific characteristics of agrammatic impairments limits translational potential for precise and improved diagnosis. INTRODUCTION: Millions of patients receive general anaesthesia every year with either propofol total intravenous anaesthesia (TIVA) or inhaled volatile anaesthesia (INVA). It is currently unknown which of these techniques is superior in relation to patient experience, safety and clinical outcomes. The primary aims of this trial are to determine (1) whether patients undergoing (a) major inpatient surgery, (b) minor inpatient surgery or (c) outpatient surgery have a superior quality of recovery after INVA or TIVA and (2) whether TIVA confers no more than a small (0.2%) increased risk of definite intraoperative awareness than INVA.

The U.S. Department of the Interior, Bureau of Reclamation (Reclamation) supports water temperature management for fishery species protection in downstream river reaches below Central Valley Project (CVP) reservoirs in the Sacramento, American, and Stanislaus River systems. The Water Temperature Modeling Platform (WTMP) Project was initiated to modernize and enhance modeling capabilities to predict summer–fall water temperature through reservoir cold water pool management using temperature control facilities designed for temperature management. The WTMP supports forecasts, historical analyses, and long-term planning efforts and advances previous modeling approaches by using an integrated modeling platform. Background and aims
Intra-individual variability in language performance has been proposed as a factor associated with treatment outcomes in chronic aphasia. However, the nature of linguistic variability and the degree to which it informs therapeutic success remains poorly understood. In this study, we sought to (1) assess person- and item-level factors associated with naming variability, practice effects, and item-based inconsistencies, (2) determine the relation between intra-individual naming variability, practice effects, and item-based inconsistencies at baseline and treatment outcome in aphasia, and (3) determine the treatment impact on the change in variability and consistency of naming responses.Method

Background: Somatostatin receptor 2 (SSTR2), a G protein-coupled receptor, is overexpressed in multiple malignancies, including hepatocellular carcinoma (HCC). While SSTR2 has traditionally been viewed as an inhibitory receptor involved in suppressing hormone secretion and cell proliferation, emerging evidence suggests a more complex role in cancer biology. However, the functional implications of SSTR2 expression in HCC remain poorly understood. This study aimed to systematically investigate the molecular landscape associated with SSTR2 expression in HCC and evaluate its potential as a therapeutic target.

Register Blocking (RB), also known as ‘Register-level Tiling’ or ‘unroll-and-jam,’ is a key compiler optimizationfor developing efficient micro-kernels. However, applying RB effectively is a complex task due to severalchallenges. First, the exploration space of possible RB configurations is vast. Second, RB and loop permutationare interdependent; therefore, addressing both optimizations simultaneously further inflates the explorationspace. Third, the effectiveness of RB is highly dependent on the target hardware platform and the specific loopkernel being optimized. The purpose of this study was to evaluate the entry-to-practice (ETP) licensure requirements for RNs in The Kingdom of Eswatini. Eswatini implemented RN licensing requirements in 2020. Previously, RNs were licensed by graduation from a nursing training institution. Now, nurses must also pass a written general nursing exam and submit a signed record book of clinical practice skills (SCCRB) (1). RNs who want to practice midwifery or mental health have additional requirements. Background: Barcode technology in healthcare has advanced significantly and has become essential for patient safety. Multifunctional devices, such as personal digital assistants (PDAs), are integrated with electronic health records, enabling real-time documentation through barcode scanning at the patient’s bedside.
Aim: This study aimed to explore nurses’ perspectives regarding the barcode point-of-care (POC) system using mobile PDAs, focusing on medication administration, blood transfusions, and specimen collection.
Methods: This qualitative descriptive study collected data through focus group interviews with 24 nurses between September and November 2023. The data were analyzed using content analysis.

Breast cancer typically metastasizes to the lymph nodes, lungs, bones, and liver. However, gastrointestinal (GI) involvement is exceedingly rare, particularly presenting as duodenal ulcers. Such metastases often mimic primary GI malignancies, leading to nonspecific symptoms like abdominal pain, anorexia, bleeding, and vomiting, which can delay accurate diagnosis. Early recognition through appropriate diagnostic evaluations is crucial to prevent disease progression. We report a unique case of an 86-year-old woman with multiple comorbidities who presented with multiple gastrointestinal ulcers. Hyponatremia is a common electrolyte disturbance; however, serum sodium level of 99 mEq/L without severe symptoms or known duration is rare and noteworthy. We report a case of an 87-year-old female who presented with fatigue, decreased appetite, and mild emesis for the past 2 days in which blood work revealed a serum sodium level 99 mEq/L. The etiology of hyponatremia in an elderly patient may be complex due to higher likelihood of co-morbid medical conditions. Methotrexate (MTX) is an anti-folate drug with anti-inflammatory and anti-proliferative properties. Low-dose methotrexate (10-25mg/week) is widely recognized as a safe and effective treatment for various autoimmune disorders. While toxicity from low dose methotrexate is uncommon it can lead to severe complications like pancytopenia. We present a case of a 68-year-old male with a history of steroid-resistant Polymyalgia Rheumatica on methotrexate (20 mg weekly) and prednisone. He presented to the emergency room with oral mucosal lesions and reduced oral intake. Upon evaluation, he was found to have pancytopenia and transaminitis, raising concern for methotrexate toxicity. Background:
Histoplasmosis capsulatum and Blastomyces dermatitidis are fungi that are uniquely distributed across the United States. Co-infection with both fungi remains uncommon. The literature review of coinfection was limited to three cases. Only two cases were recorded in humans. Infection can yield symptoms ranging from asymptomatic to life-threatening. On radiographic imaging, diffuse reticulonodular infiltrates with hilar or mediastinal lymphadenopathy can be seen. Management depends on the severity of the disease and the patient’s immune state.

The present disclosure relates to a system and method for Hamiltonian-Based Quantum Generative Autoencoder. The autoencoder leverages a novel Hamiltonian based quantum Al generative model to resolve anomaly detection in financial transaction data. An hybrid architecture, i.e., potential of quantum computing and generative AI based approach is leveraged in this disclosure to capture complex patterns and correlations in high-dimensional financial data. "Elements Don’T Mix!": Unearthing Racial Scripts And A Rhetorical Economy Of Whiteness In Disney And Pixar's Elemental, Linsay M. Cramer, Gabriel A. Cruz

Using a narrative rhetorical approach, we map how Disney and Pixar’s 2023 film, Elemental, sustains a rhetorical economy of whiteness, as theorized by Asen and Kelly, which emphasizes the degree to which rhetorical constructions of racial difference are foundational to whiteness and economic systems. We identify and critique how Elemental discursively contributes to a system of exclusivity for those within the bounds of whiteness concerning economic mobility via racial scripts. Influenza A viruses remain a global health threat, yet no universal antibody therapy exists. Clinical programs have centered on neutralizing mAbs, only to be thwarted by strain specificity and rapid viral escape. We instead engineered three non-neutralizing IgG2a mAbs that target distinct, overlapping epitopes within the conserved N terminus of the M2 ectodomain (M2e). Combined at low dose, this "triple M2e-mAb" confers robust prophylactic and therapeutic protection in mice challenged with diverse human and zoonotic IAV strains, including highly pathogenic variants. Therapeutic efficacy depends on Fc-mediated effector activity via FcγRI, FcγRIII, and FcγRIV, rather than in vitro neutralization. Serial passaging …
