Background: Respiratory viral testing in pediatric emergency departments (EDs) is frequently performed despite limited impact on clinical management for many low-risk presentations. Unnecessary testing increases cost and operational burden may delay patient throughput and can provide false reassurance to caregivers. Additionally, pediatric emergency department nurses agreed with overburden and impact to throughput based on overuse of viral testing (qualitative nursing survey conducted on September 4, 2024 showed >98% felt testing was unnecessary). We hypothesized that reducing testing through standardized team education and discharge processes would not negatively affect patient experience. Background Diabetes impairs tendon healing after rotator cuff repair (RCR), with early postoperative poor glycemic control linked to a higher failure risk in studies. GLP-1 receptor agonists (GLP-1RAs) are increasingly used for glycemic control. Preclinical studies indicate that GLP-1RA may benefit rotator cuff healing through anti-inflammatory mechanisms as well. However, real-world data on the effects of GLP-1RAs on RCR outcomes remain incompletely understood.
Methods We conducted a retrospective cohort study using an institutional dataset (01/01/2016 to 09/30/2024) of adults aged 18-74 years with type 2 diabetes who underwent RCR. GLP-1RA exposure was established by home medication lists. Patients with autoimmune …

BACKGROUND: Primary Aortoenteric Fistulas (PAEFs) are a rare condition that occurs when the native aorta develops an abnormal connection with the gastrointestinal lumen. PAEFs tend to develop within the duodenum, specifically the distal region of the third part of the duodenum, which is retroperitoneal and is anterior to the aorta. Duodenal PAEFs can be difficult to diagnose and are often lethal, given their non-specific presenting symptoms and rapid progression to hemorrhagic shock.

The Staggers Rail Act of 1980 requires the Surface Transportation Board ("STB ") to be proactive in making sure freight railroads ("carriers") earn adequate revenues. Forty-five years later there is still debate on whether carriers are earning adequate revenues, how adequate revenues are to be defined and calculated, whether the Act allows the STB to put an upper bound on a carrier's revenues when they are determined to be earning adequate revenues, and how revenue adequacy impacts rate setting in market dominant situations.

Whether active fund managers can generate consistent outperformance is one of the central questions in financial economics. While the evidence from US equity markets broadly favors passive strategies, the emerging markets are a different environment, with weaker disclosure standards and more opportunity for mispricing to persist. This paper examines whether the information environment of actively managed emerging market equity funds explains cross-sectional variations in alpha. using a sample of 474 funds drawn from Bloomberg's fund database and evaluated over a three-year alpha measurement period. This thesis investigates the relationship between architecture, human perception, and well-being through the lens of human-centered and biophilic design. Drawing from philosophical traditions, phenomenology, and contemporary neuroscience, this thesis argues that the built environment is an active agent in shaping cognition, emotion, and lived experience. Architecture is understood as multisensory, embodied, and temporally experienced, influencing individuals in both conscious and nonconscious ways. Through a comparative analysis of three case studies, Paimio Sanatorium, AEA House, and the Amazon Spheres, this research examines how biophilic design principles are incorporated into different contexts such as healthcare, residential, and workplace. Oxidopyrylium-based (5 + 2) cycloadditions are crucial reactions to construct seven-membered carbocycles containing an ether bridge (i.e. oxabicyclo[3.2.1]octanes). Intramolecular silyloxypyrone-based (5 + 2) cycloadditions were investigated and revealed several features: (1) the TBDPS thermal process proceeds via a zwitterionic oxidopyrylium intermediate similar to previously reported TBS variants; (2) the TBSOTf-mediated reaction proceeds through a cationic oxidopyrylium intermediate; (3) quantum chemical calculations predict a stepwise process for an electron-rich dipolarophile for each set of conditions. The thermal silyloxypyrone-based (5 + 2) cycloadditions were extremely dependent on the nature of the dipolarophile and the silyl transfer group. The TBDPS enhances the …

Scientific simulations and instruments now produce data at rates that overwhelm the storage, memory, and network subsystems of modern high-performance computing (HPC) facilities. Error-bounded lossy compression reduces data movement costs while bounding reconstruction error, yet three barriers limit its adoption in mission-critical workflows: existing compressors cannot guarantee the accuracy of domain-specific quantities of interest (QoIs) derived from compressed data; compression-induced artifacts such as posterization, blocking, and interpolation banding erode user confidence in decompressed fields; and significant compressibility in the quantization index arrays of interpolation-based pipelines remains unexploited. Task-oriented dialog (TOD) systems enable conversational interfaces for complex tasks like flight booking and restaurant reservations. However, deploying TOD systems at scale faces three critical barriers: scalability, generalization, and evaluation. Scalability is primarily restricted by the human-annotation bottleneck, as current systems depend on vast quantities of manually labeled data for every new domain, making deployment prohibitively expensive. Generalization remains a persistent challenge, as systems optimized for known domains often suffer significant performance degradation when encountering new, unseen ones. Due to rise of machine learning workloads, high performance computing platforms have increased GPU resources. Consequently, use of surrogate models, to utilize GPU compute and speedup scientific applications, is on the rise. Most of the time, the whole program is replaced with a surrogate model. In this work a shock simulation program (LULESH) was taken and three functions of varying complexity were replaced with small sized neural network as surrogate models. Title VI of the U.S. Civil Rights Act prohibits institutions receiving federal financial assistance from discriminating based on race, color, or national origin. The term “national origin” includes people with limited English proficiency (“LEP”). The number of LEP residents has increased in the United States to over 10% of the residents in the most populous states (Texas, California, New York, Florida, and Nevada). Executive Order 13166, issued in 2000, requires all federal agencies to comply with Title VI by establishing guidelines that remove language barriers to federal programs, services, benefits, and activities. As of 2021, the United States imprisoned one out of every five prisoners in the world. Additionally, of its own population, the United States imprisoned one out of every one hundred people.

International Cocoa Trading Arrangements Between Industrialised And Developing Countries 1951–1999: A Structural Gravity Analysis, John Brown, Xenia Matschke, Juan René Rojas Rodríguez

Using a state-of-the-art structural gravity model, we investigate the impact of various (post-)colonial cocoa trading arrangements on the cocoa trade between developing cocoa producer countries and rich industrial cocoa consumer nations in the time period 1951–1999. We find that in particular the trading arrangements of the European (Economic) Community (EEC/EC), such as the Association, Yaoundé and Lomé Agreements, as well as the UNCTAD International Cocoa Agreement, had a measurable positive impact on cocoa trade flows, in contrast to the British Commonwealth partnership or the GATT Generalised System of Preferences. Generative artificial intelligence (GenAI) is reshaping consumer shopping by introducing new forms of decision support while also creating new tensions. Using the technology paradox perspective as a sensitizing theoretical lens alongside delegation theory, this study examines how perceived GenAI benefits (process efficiency, personalization, creative discovery, and immersive experience) and constraints (hallucination potential, privacy intrusiveness, algorithmic opacity, and information biases) are associated with performance ambiguity, emotional ambivalence, and autonomy threat through substitutive and augmentative delegation-related use patterns. Purpose: Previous research shows that the capability approach is association with lower poverty rates and higher well-being. Nonetheless, little research applied this approach to understanding economic hardship. To fill this gap, this study applies this approach to explore whether financial capability mitigates racial/ethnic disparities in economic hardship in the U.S.
Design/methodology/approach: Data were from the U.S. 2022 Survey of Consumer Finances. Organizational support theory (OST) explains how employees develop and respond to beliefs about their organization, with perceived organizational support (POS) representing a central construct within this framework. Integrating psychological empowerment theory, we examine an intrinsic motivational pathway through which POS relates to employee in-role performance and identify transformational leadership as a contextual catalyst that may strengthen this process. We test a multilevel moderated mediation model using multisource and multi-time data from 217 employees and 41 supervisors. Ammonia’s (NH3) properties as a carbon-free hydrogen carrier, its phase characteristics, and its wide global infrastructure present a solid case as a potential surrogate for carbon-based aviation fuels. Delivering a zero-emission NH3-based engine requires careful integration and optimization of the combustor and thermal management systems, a process that requires high-fidelity chemical reacting models experimentally validated through the use of simplified combustors.
