Background: Sepsis is a leading cause of mortality in U.S. hospitals, with delays in recognition and treatment significantly contributing to poor patient outcomes. Nurses are essential to early identification and timely intervention; however, gaps in confidence and preparedness persist, particularly among newly hired staff. Traditional educational approaches may be insufficient to support rapid clinical recognition and evidence-based management. Lung cancer is the leading cause of cancer-related mortality in the United States, with underserved populations experiencing higher rates of late-stage diagnosis and preventable death. Despite established screening guidelines, adherence within primary care remains inconsistent, particularly in safety-net settings, reflecting a healthcare administration problem driven by organizational structures and care processes. While drug overdose deaths have generally decreased in the past year, close to 60,000 overdoses involving opioids occurred in 2024. The opioid crisis has lasted over two decades and is now in the fourth wave. The fourth wave has been characterized by overdoses resulting from fentanyl adulteration in the drug supply and polysubstance overdoses involving fentanyl. The purpose of the current study is to compare the perceived harm of fentanyl to other drugs of abuse and identify differences in the perceived harm of fentanyl between groups of people who use substances. Goal Statement

The goal of this Social Change Portfolio is to reduce relapse risk among individuals with opioid use disorder who are reentering society after their release from a Delaware correctional facility, by enhancing clinical continuity of care, including counseling, relapse-prevention planning, and linkage to community-based treatment programs.

Significant Findings

This portfolio aims to raise the awareness that justice-involved individuals with opioid use disorder in Delaware face extremely high rates of relapse, overdose, and death during the post-release period, particularly within the first few weeks and months after reentering society. Objectives: This study evaluated the efficacy and safety of misoprostol, letrozole and their combination in first trimester missed miscarriage. Methods: This single-blind, randomised controlled trial was conducted between March 2023 and August 2024 among women diagnosed with first trimester missed miscarriage at Kasr Al-Aini Hospital, Cairo, Egypt. Participants were randomly assigned to three groups: misoprostol-only (Group A), letrozole plus misoprostol (Group B) and letrozole-only (Group C). The primary outcome was the rate of complete miscarriage. Secondary outcomes included abortion timing, adverse effects, need for surgical evacuation and the effect of age, body mass index (BMI) and gestational age on success. …

The origin of cancer is poorly understood because premalignant cells are rarely followed in their native environments. Although the spatial compartmentalization of metabolic functions is critical for proper liver function, it is unknown whether cancers arise from some zones but not others and whether there are metabolic determinants of cancer risk. Zone-specific, mosaic introduction of Ctnnb1 (catenin beta 1) and Arid2 (AT-rich interaction domain 2) mutations, commonly co-mutated genes in hepatocellular carcinoma (HCC), in mouse models showed that position and metabolic context determine clone fates. Ctnnb1/Arid2-driven cancers were much more likely to arise in zone 3. Accurate precipitation mapping is essential for effective disaster management; however, individual radar, satellite, and numerical weather prediction products often struggle in the topographically complex terrain of South Korea. This study proposes a high-resolution (~500 m) daily precipitation fusion framework that integrates Korea Meteorological Administration (KMA) radar, Global Precipitation Measurement (GPM) Integrated Multi-Satellite Retrievals for GPM (IMERG), and Local Data Assimilation and Prediction System (LDAPS) data. The framework employs a Random Forest model augmented with a monthly Empirical Cumulative Distribution Function (ECDF) correction. Objective: Sacral colpopexy is a well-established procedure for apical suspension, but the relative outcomes of abdominal, laparoscopic, and vaginal approaches remain debated. This study compared perioperative outcomes across these surgical routes using a national database and evaluated trends in robotic assistance.

Materials and methods: We analyzed sacral colpopexy cases from the American College of Surgeons National Surgical Quality Improvement Program between 2014 and 2022. Patient demographics, complication rates, and surgical outcomes were compared among abdominal, laparoscopic, and vaginal procedures. Relative risks (RR) were adjusted for confounders. The utilization of robotic systems in laparoscopic procedures was examined.

Galway: Making A Capital Of Culture (Patrick Collins, Orpen Press, 2023), Connell Vaughan

Patrick Collins’ recent book details the development of the cultural industries and placemaking in the Galway region over the past century or so. Focusing on the “peripheral” position of the region, its long relationship to film making, and informed by Galway’s successful application for the European Capital of Culture in 2020, Collins argues for the unique nature of the region as a site of cultural production and consumption. While the book offers moments of insight, particularly in its discussion of ‘festivalisation’, it ultimately lacks the analytical depth and coherence needed to fully capture Galway’s complex cultural identity. The binary black hole signal GW250114, the loudest gravitational wave detected to date, offers a unique opportunity to test Einstein’s general relativity (GR) in the high-velocity, strong-gravity regime and probe whether the remnant conforms to the Kerr metric. Upon perturbation, black holes emit a spectrum of damped sinusoids with specific, complex frequencies. Our analysis of the postmerger signal shows that at least two quasinormal modes are required to explain the data, with the most damped remaining statistically significant for about one cycle. ABSTRACTBackground: Psychotropics (sometimes off-label), mental health diagnoses and behaviours of concern are common in olderadults with intellectual disability. Guidelines recommend non-pharmacological interventions and regular medication review.This study examined changes in psychotropics among older adults (≥40) with intellectual disability.Methods: Longitudinal data were obtained from the Intellectual Disability Supplement to the Irish Longitudinal Study onAgeing (IDS-TILDA) at two timepoints [Wave 1(2009/10); Wave 4(2019/20)]. Post hoc analysis, Chi-squared tests and univariatebinary logistic regression were conducted.Results: Overall, psychotropics decreased (59.2% to 56.5%). Significant decreases in antipsychotics (43.1%–40.1%) and sedatives/hypnotics (13.6%–8.1%) and significant increase in antidepressants (26.2%–31.8%) were found. Criminal law purports to care about the individual culpability of those committing crimes. Individual mental states or defenses based on one’s beliefs are considered foundational to moral responsibility and criminal conviction. The expansion of algorithmic risk assessment tools in the criminal legal system, however, threatens these principles, changing the criminal law subject from an autonomous individual to merely a predicted object. This Article argues that risk assessment tools—now functioning as gatekeepers at both the entry and exit stages of the criminal process, from predictive policing to compassionate release eligibility—have shifted the criminal system away from its foundation in individual culpability. …

Cuba has been referred to as "the world's first national experiment in sustainable agriculture." Given the critical shortages in chemical inputs to Cuba's agricultural sector following the loss of Soviet economic support and subsidization, this may well be a fitting description. Cuba's use of the term "organic" to describe some of its agricultural production can be misleading. Not all of Cuba's "organic" products would satisfy the organic certification requirements for most developed countries.
