Divergent Outcomes In Two Pediatric Cases Of Epstein-Barr Virus-Associated Hemophagocytic Lymphohistiocytosis, Zhang Kai-Jing, Li Ran, You Deng-Xia, Li Yinjie, Qin Tian-Tian, Liang Xiao-Jing, Huang Jing
Journal of Pediatric Infectious Diseases
This study retrospectively analyzed the clinical data of two 19-month-old children diagnosed with Epstein-Barr virus-associated hemophagocytic lymphohistiocytosis (EBV-HLH) who received treatment with a dexamethasone combined with etoposide (VP-16) regimen. Both children met the early remission criteria for body temperature, IL-10, and IFN-γ within 48-72 hours after treatment initiation, but their clinical outcomes were markedly different. In Case 1, the EBV-DNA load decreased significantly during the initial treatment phase, and platelet counts steadily increased after treatment; the patient had a favorable prognosis at the 2-year follow-up. In Case 2, the EBV-DNA load did not decrease but instead increased during the initial …

Impact Of Erythropoietin Therapy On Inflammatory Biomarkers In Chronic Kidney Disease: A Comparative Study, Riyam Naji Al_Alwani, Jahanbakhsh Asadi, Zainab Mohammadi
Al-Mustaqbal Journal of Pharmaceutical and Medical Sciences
Background: Chronic kidney disease (CKD) is a progressive disorder frequently complicated by anemia and a persistent inflammatory state, both of which significantly contribute to morbidity and mortality. Reduced renal production of erythropoietin (EPO) is the primary cause of anemia in CKD. Emerging evidence suggests that EPO may exert anti-inflammatory and metabolic effects beyond its hematopoietic role. This study aimed to compare serum EPO levels and inflammatory biomarkers in CKD patients treated with EPO versus untreated CKD controls.
Material and Methods: This cross-sectional analytical study was conducted on 60 adult patients with CKD undergoing regular hemodialysis. Participants were divided into two …

Computational Design, Molecular Docking, Sar, And Admet Analysis Of Novel Jak1 Inhibitors As A Possible Treatment For Rheumatoid Arthritis, Sarah Ahmed Waheed
Al-Mustaqbal Journal of Pharmaceutical and Medical Sciences
Rheumatoid arthritis is an inflammatory autoimmune disease affecting the joints which eventually leads to systemic comorbidities. Its pathogenesis was attributed to disturbances in the Janus Kinase/Signal Transducer and Activator of Transcription signaling pathway. In this research, an innovative lead compound C6 was created and docked toward Janus Kinase1 (JAK1). The docking process was confirmed by re-docking of the co-crystallized ligand MI1 (Tofacitinib). C6 showed a predicted docking score of -9.17 kcal/mol, with enhancement of 1.26 kcal/mol over Tofacitinib. Evaluation of the interactions showed a framework of five hydrogen bonds within the binding site involving an important contact with Glu966 as …

Diagnostic Accuracy Of Quantitative Coronary Analysis As Compared With Instantaneous Wave-Free Ratio In Intermediate Coronary Lesions: A Prospective Vessel-Level Study, Tamer Mosaad Ragab, Khaled Ahmed Alkhashab, Mohamed Zaki Hussain, Ibrahim Mohammed Nosseir, Marwa Salah Sayed, Waleed Khaled Ali Salem
Journal of the Saudi Heart Association
Background: Instantaneous wave-free ratio (IFR) represents a new vasodilator-free innovative approach which is considered as a gold standard for functional assessment of the hemodynamic significance of intermediate coronary stenoses. Although relatively safe, iFR is still an invasive procedure that has its potential hazards, complications and cost. Two-dimensional quantitative coronary analysis (2D-QCA) is a non-invasive technique that has emerged robustly as an excellent and attractive tool for delineating coronary anatomy.
Aims: To assess the ability of two-dimensional quantitative coronary analysis (2D-QCA) to guide decision-making for quantification of severity of intermediate coronary stenoses (40–80% diameter stenosis) in each of the three main …
Theta-Xd: Integer-Only Direction Change And Slow-Drift Detection In Multivariate Telemetry Streams, Nenad Micic
Theta-Xd: Integer-Only Direction Change And Slow-Drift Detection In Multivariate Telemetry Streams, Nenad Micic
Defensive Publications Series
A system is proposed herein for detecting sudden turns, changes in turning behavior, and slow drift in the joint movement of multivariate telemetry counters. The system uses an overflow-bounded integer direction metric and an exact division-free threshold gate, requires work linear in the number of counters and fixed-size per-stream state, and produces exactly reproducible results without floating-point arithmetic or model training. Optional discrete direction keys support compact grouping, and peer-relative integer classification identifies outliers among comparable elements. The resulting signals can be generated close to the telemetry source, where independent per-counter thresholds may miss coordinated changes.
Single-Queue, Multi-Kind Escalation: One Human-Confirmation Gate For Three Structurally Different Automated Judgments In An Autonomous Compliance Pipeline, Vipul Panday
Defensive Publications Series
An autonomous pipeline that both detects issues and acts on them (filing tickets, refreshing a reference corpus, adopting a new rule) will, almost as soon as it does more than one kind of consequential thing, produce more than one kind of moment where its own judgment is not trustworthy enough to act on unsupervised. The conventional response is a bespoke review mechanism per kind of uncertainty, added piecemeal as each is discovered during development: a findings-review screen here, an admin flag there, a Slack thread for whatever comes up next. This disclosure documents a mechanism from a production website-accessibility scanning …

Corrosion Inhibition By Carboxymethyl Cellulose Doped With Graphite Nanosheet And Silica Nanoparticles In Seawater Environment, Mohamed R. Mohssen, Sinan S. Hamdi, Rana A. Anaee, Mathew T. Mathew
Engineering and Technology Journal
This work aims to inhibit carbon steel surface as pipelines by adding carboxymethyl cellulose (CMC) with three concentrations (1, 3 and 5 g/L) in seawater environment at four temperatures in the range of 298 – 328 K. The investigation was done by recording Tafel plots and EIS curves to show that the lowest concentration is the best to protect the steel surface with efficiencies ranged between 95.68 – 89.36 % that decrease with increasing temperature due to the decomposition of CMC that begin after ≈ 308 K. An attempt to enhance the protection by CMC at higher temperature has been …

Reconfiguring Distribution Networks Using The Aco+Tnc Algorithm, Specifying The Location, Size, And Number Of Added Renewable Energy Sources, Mohammed Falah Hasan, Rana Ali Abttan
Engineering and Technology Journal
Distribution networks face increasing pressure from rising loads and renewable energy integration, making the simultaneous optimization of reconfiguration and distributed generation placement a critical engineering challenge, and conventional single-objective methods are insufficient for simultaneously addressing power losses, voltage profile degradation, and the structural constraints of radial networks. This paper proposes a hybrid framework that combines Ant Colony Optimization with the Truncated Newton Constrained method to jointly solve network reconfiguration alongside the optimal placement, sizing, and type selection of renewable distributed generation—both solar PV and wind—within a single unified model. The approach relies on a four-part weighted objective function covering active …

Young Adults’ Perceptions Of Their Life Course Development After Their Parents Were Imprisoned For Violent Crimes, Selena Robinson
Young Adults’ Perceptions Of Their Life Course Development After Their Parents Were Imprisoned For Violent Crimes, Selena Robinson
Walden Dissertations and Doctoral Studies
Parental incarceration is an adverse childhood experience that can influence children’s emotional, educational, relational, and developmental outcomes into adulthood. Although previous research had examined the effects of parental incarceration on children, limited research had explored how young adults perceive their life course development when a parent was incarcerated for a violent crime. The purpose of this generic qualitative study was to explore the perceptions of young adults, age 18–25, regarding their life course development following parental incarceration for a violent crime. Elder’s life course development theory was the framework for this study. Data were collected through semistructured interviews with three …

Colour Doppler-Guided Superthin Inferior Gluteal Artery Perforator Flap Elevation For Ischial Pressure Ulcer Reconstruction, Muhammed Said Aydin, Gökay Özler, Şükrü Kasap, Mustafa Nişanci
Sultan Qaboos University Medical Journal
Ischial pressure ulcer reconstruction requires durable coverage while preserving local donor options for possible recurrence. The inferior gluteal artery perforator (IGAP) flap is a muscle-sparing option, but superthin elevation requires precise knowledge of the superficial fascial plane and the subcutaneous course of the selected perforator. This technical note describes colour Doppler ultrasonography-guided superficial fascial plane mapping before superthin IGAP flap elevation in a 27-year-old paraplegic patient with a chronic left ischial pressure ulcer who presented in 2025 to a tertiary care university hospital in Muğla, Turkey. Preoperative high-frequency colour Doppler ultrasonography identified the dominant perforator, measured the relevant fascial intervals …

Measurements of surface-atmosphere carbon dioxide (CO2) and methane (CH4) fluxes have been relatively sparse across the Arctic tundra and boreal biomes, causing significant uncertainties in carbon budget estimates from the region. While the availability of Arctic-boreal carbon flux data has increased substantially over the past decade, the data have remained spread across different repositories, scientific articles, and unpublished sources, making it difficult to leverage. Here we present a new dataset of monthly Arctic-boreal carbon fluxes (ABCFlux v2) across terrestrial (wetlands and uplands) and freshwater (lakes and rivers) ecosystems compiled from previous syntheses including the Arctic-boreal CO2 flux database (ABCFlux v1), …

Electroencephalography is the most commonly used diagnostic tool for epilepsy. However, interpreting electroencephalograms (EEGs) requires expertise that is not widely available. Advances in digital technology and wearables have enabled large-scale EEG recording, generating vast amounts of data that cannot be managed through traditional visual interpretation by experts. Artificial intelligence (AI) has the potential to augment human expertise and reduce workloads. The application of artificial neural networks in analysing clinical EEG recordings has led to major breakthroughs, bringing AI-based EEG interpretation closer to clinical implementation. In this Review, we summarise the most important research and development results in this field from …
