Joubert syndrome (JS) is a rare genetic disorder characterized by developmental abnormalities, particularly in the brainstem and cerebellar vermis, alongside multisystem manifestations such as kidney and liver anomalies, polydactyly, cleft lip or palate, and tongue defects. The underlying ciliopathy causing JS may also contribute to gastrointestinal symptoms and immune dysregulation via Wnt signaling and impaired epithelial maintenance. Dysmotility including Hirschsprung disease has been documented at increased rates in JS and in other ciliopathies. Our case series highlights how JS patients frequently exhibit feeding intolerance, vomiting, and poor growth, which may raise suspicion for an underlying gastrointestinal condition, such as eosinophilic …

Background: Knowledge of proteomic mechanisms explaining the link between psychosocial stress and cardiovascular disease is limited. This study aimed to (1) identify plasma proteins associated with psychosocial factors and (2) assess associational pathways between psychosocial factors, identified proteins, and incident cardiovascular disease events in a discovery cohort, JHS (Jackson Heart Study), and 2 replication cohorts, the CHS (Cardiovascular Health Study), and the MESA (Multi-Ethnic Study of Atherosclerosis).

Internalizing disorders disproportionately affect autistic adolescents, though limited research has focused on tailoring therapy for this population. The purposes of this paper were (1) to describe the development of a transdiagnostic exposure therapy/behavioral activation intervention for autistic adolescents with internalizing disorders, and (2) to present a pilot test of feasibility, acceptability, and efficacy of the intervention. Introduction/aim: As humans return to the Moon, safe efficient ambulation and fall prevention on the lunar surface are key concerns. The analysis of gait from archived Apollo mission video footage is now possible with pose estimation video analysis software. Thus, this study aims to retrospectively quantify parameters of Apollo astronauts' stance and gait on the Moon.

Methods: Publicly available National Aeronautics and Space Administration (NASA) footage of astronauts ambulating on the Lunar surface was examined using body pose analysis software. Background: Relapsed medulloblastoma remains a significant therapeutic challenge as it is near universally fatal. The tumor microenvironment of medulloblastoma plays a critical role in tumor progression, influencing tumor growth, immune evasion, and therapeutic resistance. We hypothesized that defining tumor-immune interactions in diagnostic and relapsed medulloblastoma may uncover mechanisms of immune evasion and identify novel therapeutic targets.

Methods: We analyzed paired primary and recurrent RNA-sequencing data from 140 medulloblastoma patients to profile immune cell composition and validate spatial relationships within the TME. Background & aims: Helicobacter pylori screening and eradication is a cost-effective strategy for primary gastric cancer prevention in high-risk populations outside the United States. There is a paucity of population-based H pylori seroprevalence data available in high-risk U.S. groups, particularly data for low-income, uninsured patient populations.

Methods: We used data from a cross-sectional study of a largely underserved, immigrant, and minority patient population. Inclusion criteria included asymptomatic individuals 30-65 years of age without prior H pylori testing or treatment. Background: Self-reported smoking may not fully capture individualized risk of smoking-related cancer, but circulating proteins may reflect biological consequences of smoking. Thus, we developed a score based on smoking-related proteins and evaluated its association with smoking-related cancer.

Methods: This prospective cohort study included 10 563 participants aged 47-70 years in the Atherosclerosis Risk in Communities study. Plasma proteins were measured using the SomaScan Platform (SomaLogic Operating Co, Inc). The score was constructed from proteins associated with current smoking, pack-years, or recent quitting identified by linear regression and elastic net regression. Integrase strand transfer inhibitors have transformed human immunodeficiency virus (HIV) therapy, yet the widely prescribed drug dolutegravir (DTG) has been linked to developmental toxicity, and its teratogenic mechanism remains unclear. Here, we used zebrafish to dissect DTG toxicity during early vertebrate development. DTG exposure from 2-4 h post-fertilization (hpf) to 24 hpf produced high mortality and abnormal morphology. Co-treatment with folates partially restored normal morphology, whereas calcium had no effect. Strikingly, supplementation with magnesium (Mg) partially rescued DTG-exposed embryos, implicating Mg availability in protection. Purpose: To evaluate the accuracy and prose responses of 2 large language models (LLMs) to ophthalmology continuing medical education questions.

Design: Question prompts and multiple choice (MC) answer options were input into the 2 LLMs, and responses were analyzed for accuracy and assessed for evidence of correctness, completeness, bias, and potential harm using a previously reported standardized rubric.

Subjects: Basic and Clinical Science Course questions and MC answer options from the American Academy of Ophthalmology question bank were used as inputs into the 2 LLMs (ChatGPT-4 and Google Vertex's Gemini Pro 1.5).

Background: Pediatric spinal fusion can result in substantial blood loss, particularly in patients with scoliosis related to underlying neuromuscular (NM) disease. Transfusion is frequently required but carries risks including transfusion reactions, alloimmunization, volume overload, and infection.

Local problem: The American College of Surgeons National Surgical Quality Improvement Program-Pediatrics (ACS NSQIP-P) is a nationally validated, risk-adjusted national outcomes registry. At our institution, ACS NSQIP-P data identified our institution as a high outlier in transfusion for neuromuscular scoliosis patients.

The digestive system is essential for nutrient absorption, processing, and waste elimination. The expression of many genes and physiological processes within this system exhibits a 24-h rhythmicity. However, modern lifestyle factors, such as jet lag, shift work, and irregular eating patterns, significantly disrupt these rhythms, triggering various stress responses and contributing to numerous digestive disorders. Post-Exercise Vitals Recovery Response In Barrel Horses: A Field Study On Cooling Methods Used By Competitors, Lauran E. Hetrick

Barrel racing demands peak physical performance from equine athletes, yet limited research evaluates the field-based strategies used to optimize equine recovery at competition. This study assessed recovery management approaches used by competitors to elucidate their influence of physiological recovery by measuring heart rate, respiratory rate, and body temperature during a high-profile barrel race competition. Physiological data were collected at baseline, immediately post-exercise, and during a 15–30-minute cooldown period to characterize recovery under real-world conditions. Significant increases in HR, RR, and BT occurred from baseline to post-run (p < 0.001), followed by reductions during cooldown (p < 0.01), though values remained elevated (p < 0.05). Rinsing reduced HR and RR (p < 0.01), while shade and fans decreased RR and variably influenced HR. Noise, as a confounding variable, decreased HR but increased BT (p < 0.001). Ultimately, these results emphasize multimodal recovery approaches in competitive equine management in the field.

The term “R&B” has existed within the musical lexicon for nearly eighty years. It was first introduced in 1949 to replace the label “race music,” and was a combination of blues, jazz, gospel, and rock. Since then, the genre has transitioned from swing-based, live instrumentation toward straight-eighth backbeats and electronically produced sounds. Despite these shifts, contemporary R&B continues to borrow from its ancestral roots while maintaining a modern aesthetic. Confirmation bias — the tendency to favor belief-consistent information over contradictory evidence — represents a central challenge in contemporary misinformation research, yet its behavioral measurement has remained inconsistent across studies. This study introduced the Confirmation Bias Index (CBI), a preregistered behavioral measure defined as the difference in accuracy between ideologically congruent and incongruent headline evaluations. Using a 3 (headline slant: Conservative, Neutral, Liberal) × 2 (veracity: True, False) within-participants design, 198 participants evaluated 36 validated news headlines for accuracy. This study investigates the geotechnical, geophysical, and mineralogical characteristics of soils derived from the Weches Formation in Nacogdoches County, East Texas, with emphasis on the relationships among electrical resistivity, moisture condition, compaction state, and mineralogical composition. The Weches Formation is well known for producing clay-rich, moisture-sensitive soils that pose challenges to subgrade performance because of their plasticity and swelling behavior. Representative soil samples were collected and subjected to laboratory testing, including particle size analysis, Atterberg limits, Standard Proctor compaction, free swell tests, electrical resistivity measurements, and X-ray diffraction (XRD) mineralogical analysis. ABSTRACT

This study examined the relationship between Disciplinary Alternative Education Program (DAEP) placement and graduation outcomes among students in special populations within a large, urban public school district in Texas. Grounded in Social Cognitive Theory, this study analyzed a four-year longitudinal cohort to determine whether DAEP placement was associated with differences in graduation outcomes, including on-time graduation, late graduation, and dropout status. The study examined differences between students with single and multiple DAEP placements. The sample included over 4,000 students including students who experienced DAEP placement. Surgical complications remain a major source of preventable morbidity, mortality, and health care expenditure, but existing frameworks such as the Clavien-Dindo classification and Comprehensive Complication Index are clinician-centred and intervention-focused and fail to capture cumulative patient-centred outcomes. This protocol outlines the Complications After Major and Minor Urological Surgery (CAMUS) initiative, a global, multiphase effort to redefine complication reporting, risk stratification, and outcome measurement in urological surgery. BACKGROUND: The modified Blalock-Taussig-Thomas shunt (mBTTS) is a critical palliative procedure for infants with single-ventricle physiology, but thrombosis-related occlusion affects 8% to 12% of infants and carries nearly 50% mortality. Meanwhile, existing antithrombotic strategies fail to address the hemodynamic factors driving thrombosis, highlighting the need for a deeper understanding of flow dynamics in shunt failure.

OBJECTIVES: This study aims to identify how mBTTS geometry influences hemodynamics and thrombosis risk, providing quantitative guidance for surgical planning and shunt design optimization.

IMPORTANCE: Perinatal depression is common in sub-Saharan Africa, yet treatment gaps are substantial. Cognitive behavioral therapy (CBT) is effective but relies on written materials.

OBJECTIVE: To evaluate the efficacy of a culturally adapted CBT intervention delivered by lay counselors to women with perinatal depression in rural Sierra Leone.

DESIGN, SETTING, AND PARTICIPANTS: This study was an individually randomized, outcomes assessor- and investigator-blinded, clinical trial of an adapted CBT intervention vs enhanced usual care. Three-dimensional reconstruction of cortical surfaces from MRI for subsequent morphometric analysis is fundamental for understanding brain structure. While high-field Magnetic Resonance Imaging (HF-MRI) is the standard in research and clinical settings, its relatively limited availability hinders widespread use. Low-field MRI (LF-MRI), particularly portable systems, offers a cost-effective and accessible alternative. This study examines the impact of sharia compliance on stock price crash risk, controlling for company fundamental characteristics, among firms listed on the Indonesia Stock Exchange from 2021 to 2024. Panel data was analyzed using a dynamic panel model (System GMM) and crash risk is measured using NCSKEW and DUVOL. The results show that Sharia compliance significantly reduces stock price crash risk when measured using NCSKEW, while its effect on DUVOL is insignificant. This finding is consistent with the robustness check using the two-step System GMM, suggesting that Sharia principles can be a mitigating mechanism for extreme crash risk. These …

This reflective article examines the limitations of control-based approaches in traditional vocal training and proposes a shift toward understanding the voice as an integrated, coordinated bodily system. This field article introduces The Secret Science of Songs, a research-informed, practice-based framework that reconceptualizes songs as powerful, transdisciplinary tools in early childhood education. Drawing on over 40 years of classroom-based action research alongside established findings in cognitive science and music perception, the framework argues that young children’s engagement with songs is rooted not primarily in performance skill, but in their innate capacity to perceive and process auditory patterns in music and language. These pattern-based experiences constitute a rich aesthetic and cognitive domain through which children construct meaning, long before mastery of melody or verbal precision emerges.
