Homebound adults represent a medically vulnerable and rapidly growing population at heightened risk for unmet healthcare needs. Although quantitative studies document disparities in utilization and outcomes, less is known about how homebound individuals experience healthcare access. Grounded in Husserlian transcendental phenomenology, this qualitative study explored the lived experiences of homebound adults facing healthcare access barriers and examined how those experiences align with Levesque et al.’s patient-centered conceptual framework of access. Twenty homebound adults were recruited from a single residential care setting in the southeastern United States. Oxytocin And Vasopressin Receptors Mediate Dopamine Release In The Nucleus Accumbens, Rebecca Chari Crenshaw

Oxytocin (OXT) is a potential therapeutic target for substance use disorders due to its modulatory effects on mesolimbic dopamine function. While OXT activity in the nucleus accumbens (NAc) influences dopamine release, the receptor mechanisms underlying these effects remain unclear. Oxytocin receptors (OXTRs) and vasopressin 1A receptors (V1aRs) are both expressed in the NAc and may differentially regulate dopamine function. This study examined how selective activation and blockade of OXTRs and V1aRs within the NAc modulate phasic and tonic dopamine release using in vivo amperometry in male and female mice. Introduction: Clozapine is the most effective antipsychotic for treatment-resistant schizophrenia but remains underutilized due to safety concerns and monitoring complexity. Recent discontinuation of the Clozapine Risk Evaluation and Mitigation Strategy (REMS) program reduced regulatory barriers but shifted responsibility for monitoring to clinicians. Psychiatry residents may lack adequate training and confidence to prescribe clozapine safely in this new context. The goal of this study is to evaluate the impact of a structured educational intervention on psychiatry residents’ knowledge of clozapine prescribing and management.

Residential flexible loads, such as heat pump water heaters, have great potential for grid reliability services; however, their effectiveness depends on reliable performance in response to coordinated grid service requests. While the ANSI/CTA-2045-B protocol standardizes communication, it does not mandate specific functional behaviors, creating a mismatch in which the device may comply with the communication protocol but fail to meet performance expectations. This manuscript demonstrates the need for a ANSI/CTA-2045-B implementation guide by quantifying these discrepancies through gray-box conformance testing of market-available units followed by large-scale simulations.

On July 14, 1864, the Battle of Tupelo, or the Battle of Harrisburg as it is also known, occurred within fifteen miles of the community of Palmetto in northeast Mississippi. As the battle ensued, wounded and dying soldiers were transported to different sites in and around Tupelo, including the home of Dr. William Henry and Jane Calhoun in Palmetto that served as the main hospital. The bodies of some of those soldiers still remain on the former Calhoun Plantation, including the grave of D. D. Drake. We report on the randomized portion of the phase 2, open-label CheckMate 650 trial (NCT02985957), in which docetaxel-experienced, biologically male patients with chemotherapy-refractory metastatic castration-resistant prostate cancer were randomized 2:2:1:2 to nivolumab 3 mg /kg plus ipilimumab 1 mg /kg (n = 73), nivolumab 1 mg /kg plus ipilimumab 3 mg /kg (n = 74), ipilimumab 3 mg /kg (n = 38), or cabazitaxel (n = 74). Primary endpoints were objective response rate and radiographic progression-free survival; key secondary endpoints included overall survival, prostate-specific antigen response rate, and safety. BACKGROUND: Systemic inflammation is hypothesized to contribute to post-traumatic stress disorder (PTSD) vulnerability. Few studies have examined inflammation shortly after trauma as a predictor of later PTSD symptoms. We examined whether inflammation from the emergency department (ED) post-trauma is associated with PTSD symptom severity over the following 6 months.

METHODS: Our sample included 742 AURORA participants, a longitudinal cohort of patients in 29 EDs across the United States after a traumatic stressor, followed up to 6 months. This study explores the use of Duolingo as a web-based language learning tool and its influence on students’ speaking anxiety at MTs Nurul Falah Kota Jambi. Speaking anxiety remains a significant affective barrier in English as a Foreign Language (EFL) learning, particularly in traditional classroom settings where students often experience communication apprehension, fear of negative evaluation, and test anxiety. Employing a qualitative case study design, this research involved approximately 15 students who had used Duolingo for at least six weeks, along with one English teacher for triangulation. Nature As The Foundation: A Shift In Pedagogy, Mary Moroney-Fernandez

Somewhere in the United States right now, a child is sitting beneath fluorescent lights while the world outside continues without her. This paper argues that her confinement is not accidental, but rather theoretical. Examining the divergent frameworks of Friedrich Fröbel and Jean Piaget, it traces how American public schooling absorbed a vision of learning organized around cognitive individualism while displacing Fröbel's relational vision of child and nature. Drawing on reflection, historical analysis, and research, this paper contends that lasting reform requires confronting the foundational assumptions that built the classroom in the first place.

This piece offers an introspective reflection by an educator at the University of North Dakota, blending phenomenological perspectives on the nature of education with the embodied experience of participating in a sweat lodge. Framed by a meandering walk across campus, the narrative interweaves several thematic threads: the tension between clarification and explanation, the values that should guide technology education, and the role of nature with its inherent, chaotic vitality.
