Clinician Education Intervention: Adolescent Depression Management, Gina Botello Oct 2024

Clinician Education Intervention: Adolescent Depression Management, Gina Botello

Group: Northern Arizona University School of Nursing, DNP Doctoral Papers

Major depressive disorder is a mental illness that impacts adolescents' mental health. Effective treatments for major depressive disorder are available. Barriers such as clinicians' perceptions that adolescents should be referred to a child and adolescent psychiatrist when seeking mental health services marginalize the value of screening. This evidence-based quality improvement project aimed to implement a clinician-education intervention in an outpatient behavioral health clinic in a border town in Southwest Arizona using established guidelines from the American Academy of Pediatrics and the Canadian Network for Mood and Anxiety Treatments. This quality improvement project aimed to strengthen clinician use of the patient …

Adapting An Evidence-Based Insomnia Practice Guideline For Use In A University Healthcare Setting, Maria M. Denny

Group: Northern Arizona University School of Nursing, DNP Doctoral Papers

Recent evidence-based practice guidelines (EBPGs) recommend cognitive-behavioral therapy for insomnia (CBT-I) as first-line treatment for adults with chronic insomnia, with pharmacotherapy as a secondary option. Pharmacotherapy is often used in primary care settings, which have limited access to specialty CBT-I services. Proven lower-intensity CBT-I components and delivery modes can be customized for primary care. The purpose of this DNP project was to adapt an insomnia EBPG for use at a university health service with integrated primary care and mental health resources. The integrated Promoting Action on Research Implementation in Health Services (i-PARIHS) framework guided this project, involving primary care and mental …

Mentorship Impact On Novice Advanced Practice Provider Satisfaction And Turnover, Michelle Gillard Oct 2024

Mentorship Impact On Novice Advanced Practice Provider Satisfaction And Turnover, Michelle Gillard

Group: Northern Arizona University School of Nursing, DNP Doctoral Papers

In response to a healthcare provider shortage, the Advanced Practice Registered Nurse (APRN) workforce is rapidly expanding with more practitioners entering clinical practice than ever before. Unfortunately, once APRNs enter the workforce, decreased job satisfaction, role strain, and increased job demands may contribute to high turnover rates. To increase institutional support for novice Advanced Practice Providers (APPs) during the transition period, a large academic quaternary pediatric hospital in the southwestern United States developed a mentorship subcommittee for APPs with fewer than two years of advanced practice experience. This quality improvement project evaluated the impact of formal mentorship on job satisfaction …

Transparency And Advanced Practice Nurse Value On An Interprofessional Team By Time-Driven Activity-Based Costing, Melissa M. Davis Oct 2024

Transparency And Advanced Practice Nurse Value On An Interprofessional Team By Time-Driven Activity-Based Costing, Melissa M. Davis

Group: Northern Arizona University School of Nursing, DNP Doctoral Papers

To address the current challenge in the United States of increasing access to high quality healthcare, healthcare systems must analyze and understand the value of their resources in terms of patient outcomes achieved per dollar expended (Kaplan & Porter, 2011). A review of the literature supports that advanced practice nurses (APN) are not only in a position to increase access to healthcare, but capable of improving patient care outcomes. There is little available evidence, however, that the APN decreases costs, or impacts value, in healthcare. For this retrospective, longitudinal study, the transparent methodology of Time-Driven Activity-Based Costing (TDABC), guided by …

Promoting Safe Infant Sleep: The Creation And Appraisal Of A Practice Guideline For Nurses, Sharon C. Hitchcock Oct 2024

Promoting Safe Infant Sleep: The Creation And Appraisal Of A Practice Guideline For Nurses, Sharon C. Hitchcock

Group: Northern Arizona University School of Nursing, DNP Doctoral Papers

Background: Every day in America, healthy infants are dying from sudden infant death syndrome (SIDS) and other sleep-related deaths. Once a terrifying mystery, these deaths, also called sudden unexpected infant death (SUID), are now considered preventable (American Academy of Pediatrics [AAP], 2016). SUID rates in the United States have remained stagnant and unacceptably high since the late 1990’s despite known modifiable risk factors (Centers for Disease Control and Prevention [CDC], 2017). These rates are higher than most other developed nations (MacDorman, Matthews, Mohangoo, & Zeitlin, 2014). Although prevention strategies are evidence-based and generally known and understood by nurses, studies have …

Depression Screening To Improve Early Detection, Referral, And Treatment Of Depression In Patients With Diabetes, Karina Avechuco Oct 2024

Depression Screening To Improve Early Detection, Referral, And Treatment Of Depression In Patients With Diabetes, Karina Avechuco

Group: Northern Arizona University School of Nursing, DNP Doctoral Papers

Comorbid depression is common among adults with diabetes and is associated with negative impacts on quality of life, health outcomes, and mortality rates. As of 2015, it was estimated that 415 million people were living with diabetes in the United States, and of those affected, 20 to 25 percent have some degree of depression. Evidence-based guidelines recommend routine depression screening. This appears to decrease negative health outcomes and may be a cost-effective way to reduce the risk of adverse outcomes. Despite recommendations, depression screening in patients with diabetes is not consistent. This is true for patients at the study clinic …
