Leadership Connection
Session presented on Thursday, September 25, 2014:
Exclusive breastfeeding for the first six months of life is strongly associated with optimal health outcomes for mother and child, in the short and long term. Yet exclusive breastfeeding is practiced by a minority of families, the duration rates of any and exclusive breastfeeding at six months are well below the United States Department of Health and Human Services Healthy People 2020 objectives. According to the 2008 report by the United States Breastfeeding Committee barriers that effect why moms do not breastfeed include: 1) limited provider awareness, knowledge, skills, and practices and limited …
Grams Vs Miligrams: A Fatal Mistake, Brandon De Horta
Grams Vs Miligrams: A Fatal Mistake, Brandon De Horta
Leadership Connection
Medication errors are preventable adverse effects in patient care. With medical errors being one of the leading causes of inpatient & outpatient deaths, the nursing community has implemented several safety nets for improving medication practices. They happen for many reasons such as poor communication, improper documentation, illegible handwriting, similar medications, and high patient-to-nurse ratio. Unfortunately with all the guidelines for reducing medical errors, the nursing community still uses the improper abbreviation of gm for grams than the International Standard of Unit (ISU) approved abbreviation g. Nursing has become a very diverse practice that has been utilized by corporations, military branches, …

Leadership Connection
Session presented on Thursday, September 25, 2014:
Numerous barriers exist for patient adherence to post operative care following total laryngectomy. Without complete voice restoration, the single most challenging postoperative consequence is the inability for patients to effectively communicate with their care team. This protocol explores the use of a smartphone application to improve post-operative outcomes following major Head and Neck Cancer and Reconstructive Surgery by providing timely resources and support to the patient and their caregivers. The health care field will be enriched with new ways to interact, support, and communicate with patients. The innovative model of care strives to …

Session presented on Friday, September 26, 2014: Effective nursing leaders at all levels of health care organizations and academic institutions worldwide require skills in evaluating and using sound evidence in practice. The seven evidence-based practice competencies for leaders include: (a) generating relevant inquiries that research can answer; (b) engaging resources to address priority inquiries, including shared governance systems; (c) searching for relevant literature in effective, efficient ways; (d) identifying the best levels of evidence; (e) critiquing research studies relevant for leaders; (f) appraising clusters of studies for readiness-to-use in practice; and (g) translating evidence into practice. Nurse leaders often express a …
Factors Influencing The Quality Of Emergency Department Nurse Shift Handover, Heather K. Thomson
Factors Influencing The Quality Of Emergency Department Nurse Shift Handover, Heather K. Thomson
Leadership Connection
Session presented on Friday, September 26, 2014:
Background: Nurse-to-nurse shift handover is defined as defined as bidirectional communication that results in the transfer of information and responsibility for a patient (Friesen et al., 2008; McFetridge et al., 2007; Tregunno, 2009). Nurse-to-nurse shift handover is an integral part of safe nursing care as incoming nurses depend on handover information to inform decision making, establish priorities, and to plan and provide patient care. The Emergency Department (ED) is a clinical area where nurse-to-nurse handover communication is particularly vulnerable as a result of the unpredictable, often chaotic nature of the environment. Although a …

End Of Life In African American Older Adults: An Integrative Literature Review, Karen O. Moss, Karen M. Rose
Leadership Connection
Session presented on Friday, September 26, 2014:
End-of-life care in the United States is described as deficient and fragmented. Minority populations engage in end-of-life planning less frequently than Caucasians. African Americans represent the second largest minority group in the United States with its older adult population projected to increase by 114% by 2030. The Patient-Self Determination Act of 1990 requires Medicare and Medicaid funded healthcare organizations to empower patients to refuse or accept medical care and execute advance directives. The Study to Understand Prognoses and Preferences for Outcomes and Risks of Treatments (SUPPORT) confirmed substantial shortcomings of healthcare for seriously …

Session presented on Friday, September 26, 2014:
The Institute of Medicine 1999 report, To Err is Human, shocked the world (Kohn, Corrigan, Donaldson, 2000). The realization that our healthcare system caused the death of patients at alarming rates was a call to action for many. The quest to find ways to mitigate risk began 15 years ago. Leaders in the patient safety field spoke before the United States Senate Subcommittee on Primary Health and Aging in July, 2014 and noted that the patient safety situation has not improved since 1999. Subcommittee Chair Sen. Bernie Sanders is working to bring the …

ACS also estimated that 1,700,000 TBIs occur annually, 250,000 patients are hospitalized, and 52,000 patients die. US data estimates 80,000 - 90,000 patients each year suffer a TBI with long-term disability. The primary goal in …

It is estimated that 1,960 pediatric patients will die of cancer in the United States in 2014 and approximately 15,780 new diagnoses will be made (Ward, DeSantis, Robbins, Kohler, & Jamal, 2014, p. 75). Research is being done regarding: the end-of-life decision-making process; the corresponding satisfaction outcomes for the family and clinicians; and the cognitive level of …

Optimal care of the community client, including those considered senior or elderly has become a critical consideration of the Canadian health care system. As such, meeting the health needs of the community client, a fundamental role of home care, has been theoretically improved by home cares adoption of an electronic client assessment and information system known as Resident Assessment Instrument Home Care (RAI-HC), one of the many assessment systems within the inter-RAI suite of assessments. Valid and reliable client outcome data and algorithms are generated from a completed RAI-HC, and available to home care and its assessor coordinators to support …
Amish Communities, Meghan J. Cook
Amish Communities, Meghan J. Cook
Leadership Connection
Session presented on Thursday, September 25, 2014:
The use of herbal medications or alternative therapies is a practice widely used in many Amish cultures and rural communities. Often times, Amish patients do not view these therapies as medications and do not communicate their use with healthcare providers when seeking traditional healthcare. Other reasons for not disclosing the use of alternative therapies include mistrust among English healthcare providers or fear of being discriminated against. Many of these alternative and complementary therapies and medicines have potential adverse effects and many not be ideal co-therapies with traditional western medicine. Healthcare providers must create …
