We conducted a global characterization of the microbial communities of shipping ports to serve as a novel system to investigate microbial biogeography. The community structures of port microbes from marine and freshwater habitats house relatively similar phyla, despite spanning large spatial scales. As part of this project, we collected 1,218 surface water samples from 604 locations across eight countries and three continents to catalogue a total of 20 shipping ports distributed across the East and West Coast of the United States, Europe, and Asia to represent the largest study of port-associated microbial communities to date. Clostridioides difficile is the leading cause of diarrhea in hospitalized U.S. patients and results in over 400,000 cases of C. difficile infection per year. C. difficile infections have mortality rates of 6 to 30% and significantly increase health care costs, because of increased length of stay and increased frequency of readmissions due to recurrences. Efforts to reduce the spread of C. difficile in hospitals have led to the development of rapid sensitive diagnostic methods. The United States has spent—and continues to spend—billions of dollars building Iraq’s military capabilities. Despite that fact, Iraq’s military performance, even after wresting control of its territory from the Islamic State, remains inconsistent at best. A survey of Iraqi military history suggests a pattern of strengths, weaknesses, and performance that includes courageous soldiers, cohesive units, incompetent leaders, divided loyalties, poor combat support, and weak institutions that have, on occasion, risen to the defense challenge. INTRODUCTION: Academic Emergency Medicine (EM) departments are not immune to natural disasters, economic or political forces that disrupt a training program's operations and educational mission. Due process concerns are closely intertwined with the challenges that program disruption brings. Due process is a protection whereby an individual will not lose rights without access to a fair procedural process. Effects of natural disasters similarly create disruptions in the physical structure of training programs that at times have led to the displacement of faculty and trainees. Currently, the United States has no standardized requirement for sex education. This has precipitated a large gap in knowledge about safe sex and a lack of consensus in current social and educational policy. Debates about abstinence-only and comprehensive sex education have reached a standstill. In an effort to advance the discussion, this paper reveals that the neuroscience behind adolescent sexual risk taking provides underutilized evidence for comprehensive sex education programs. This article explores the political consequences of four decades of consistent humiliation of the poor by the most authoritative voices in the land, and offers insights into ways that new movements are creating spaces for poor people’s political voices to surface and become relevant again. This monograph analyzes the current political tensions between the United States and Turkey and suggests ways to manage them. The two countries have been strategic allies since at least the end of World War II—Turkey became a North Atlantic Treaty Organization member and participated with its military forces in the Korea War, and during the Cold War protected NATO’s southern flank against Soviet communism, and Turkey’s military and intelligence services maintained close relationships with their Western and Israeli counterparts. BACKGROUND: The Affordable Care Act (ACA) waived deductibles and eliminated coinsurance for colonoscopies for Medicare beneficiaries beginning in January 1, 2011. This study investigated the effect of the ACA's directive to remove the financial barriers on the receipt of colonoscopies among the elderly insured, who are predominantly covered by Medicare. METHODS: Data from the 2008-2016 Behavioral Risk Factor Surveillance System (BRFSS) were used to examine the receipt of colonoscopies in two years prior to the implementation of the ACA (2008 and 2010) and three years after the change (2012, 2014, and 2016). OBJECTIVES: The National Cancer Institute (NCI) National Clinical Trials Network performs phase II and III clinical trials, which increasingly rely on the submission of diagnostic formalin-fixed, paraffin-embedded tissue blocks for biomarker assessment. Simultaneously, advances in precision oncology require that clinical centers maintain diagnostic specimens for ancillary, standard-of-care diagnostics. This has caused tissue blocks to become a limited resource for advancing the NCI clinical trial enterprise and the practice of modern molecular pathology.

The Cold War in the 1950s and 1960s placed newly-independent India in a liminal ideological position: as a committed representative democracy, India seemed a natural ally for the United States, but its socialist, planned economy was patterned on that of the Soviet Union. The United States (US) has a complex healthcare system with a mix of public, private, nonprofit, and for-profit insurers, healthcare institutions and organizations, and providers. Unlike other developed countries, there is not a single payer healthcare system or a national pharmaceutical benefits scheme/plan. Despite spending over USD 10,000 per capita in healthcare, the US is among the worst performers compared to other developed countries in outcomes including life expectancy at birth, infant mortality, safety during childbirth, and unmanaged chronic conditions (e.g., asthma, diabetes). This chapter uses the dynamic federalism model of constitutional dual sovereignty as an analytic window into the larger legal pluralism discourse that has emerged in recent decades. Legal pluralism explores the significance of the multiple sources of legal authority and identity with which individuals simultaneously engage. These overlapping sources of normative authority range from local, national, and international institutions of government to private sources of “quasi-legal” norms generated by tribal, religious, commercial, professional, or other associations. Scholarly advocates of legal pluralism challenge the tradition of legal monism—so entrenched that its presumptions often go unnoticed—which views legitimate legal authority as deriving …

Immigration is at the forefront of national, state, and local policy struggles in the United States, and Latinx/@ immigrants have experienced increased deportations, detention, and individual threats. A mobilities perspective allows analysis to extend our view of migration beyond frameworks confined to pre- and postmigration, examining trajectories of social inclusion and exclusion that are influenced by multiple factors in the receiving country. We review methodological opportunities and lessons learned in conducting a longitudinal, prospective study of same-sex couples with civil unions, recruited from a population-based sample, who were compared with same-sex couples in their friendship circle who did not have civil unions, and heterosexual married siblings and their spouse. At Time 1 (2002), Vermont was the only US state to provide legal recognition similar to marriage to same-sex couples; couples came from other US states and other countries to obtain a civil union. Ecological studies indicate that impervious cover (IC) greater than approximately 5%–20% may have adverse effects on receiving-stream ecology. It is difficult to separate the effects of runoff quality from other effects of urbanization on receiving streams. This study presents the results of a numerical experiment to assess the effects of increasing IC on water quality using the Stochastic Empirical Loading and Dilution Model (SELDM). Hydrologic and physiographic variables representative of southern New England were used to simulate receiving water quality in a basin with IC ranging from 0.1% to 30%. Understanding what motivates people to work is a critical concern for management. In preparing students for a global economy, awareness of cultural differences is motivation is important. Researchers have studied the effect of cultural values and most recently generational cohorts in identifying motivational values. This study examines motivational work values of Russian and US. millennials/Generation Z. While the findings show that US. and Russians have become more similar in motivation than in earlier studies supporting research on generational cohorts, there are still important cultural differences separating the cohorts. Implications for business and suggestions for classroom use are given.
