"The United States health care system is broken." This common refrain has been echoed by policymakers and politicians, Democrats and Republicans, and liberals and conservatives alike. They point to the unacceptably high number of uninsured Americans, out of control costs, and questionable quality as just some of the problems that afflict American health care. Before the passage of the Patient Protection and Affordable Care Act in March, 2010, over 45 million Americans were without health insurance coverage.1 Health care spending currently accounts for 17% of this country‘s gross domestic product. Socioeconomic Status, Self-Rated Health, And Mortality In A Multiethnic Sample Of Us Adults, Sivaranjani Suuresh, Charumathi Sabanayagam, Anoop Shankar

Objective: To examine the association between socioeconomic status (SES), self-rated health (SRH), and mortality separately by race-ethnicity in a nationally representative sample of US adults. Methods: We analyzed data from 16 716 adult women and men who were followed up for mortality for up to 12 years as part of the third National Health and Nutrition Examination survey (NHANES III). Poverty-income ratio (PIR) and education were assessed as measures of SES. All-cause mortality (n = 2850) was recorded from the NHANES III linked mortality file.

BACKGROUND AND METHODS: Active duty US Army and Air Force military personnel undergo mandatory biennial HIV antibody screening. We compared pre- and post-HIV seroconversion health status by conducting a case-control study, which evaluated all medical encounters and sociodemographic factors among incident HIV seroconverters and HIV-negative controls from June 2000 through February 2004.

RESULTS: A total of 274 HIV seroconverters and 6205 HIV-negative personnel were included. In multivariate analysis restricted to male personnel (cases = 261, controls = 5801), single marital status (adjusted odds ratio [AOR] = 14.37), clinical indicators or symptoms within four years of HIV diagnosis (AOR = 6.22), …

In this short symposium contribution, I attempt first to add some further evidence on the interpretive question. That evidence weighs strongly, in my view, in favor of Kagan's conclusion that the terminology does not communicate any particular congressional intent regarding presidential directive authority. Assessed in context, the "whole code" textual analysis presented by Stack does not justify the conclusion that Congress, by delegating to an executive branch official, meant to limit presidential control. Independent agencies excluded, interpreting the terms of simple and presidential delegations to speak to directive authority fails, in general, to make sense of the various statutes. Absent …

Settlement and growth of quagga mussels Dreissena rostriformis bugensis were monitored in Lake Mead, NV, USA, where the first dreissenid occurrence was confirmed in the western United States. To measure the settlement rate of these invasive mussels, seven acrylonitrile butadiene styrene (ABS) pipes were attached to a line in shallow water (7.7 m below the surface) since November 23, 2007; eight ABS pipes were placed on another line in deep water (13.4 m below the surface) since January 3, 2008. Quagga mussels were sampled from these pipes on March 19, May 21, July 9, October 20, and December 19, 2008. …

Recent Trends In Exposure To Secondhand Smoke In The United States Population, Chieh-I Chen, Tanya Burton, Christine L. Baker, Vera Mastey, David Mannino

BACKGROUND: Previous research using the National Health and Nutrition Examination Surveys (NHANES) data documented a significant downward trend in secondhand smoke (SHS) exposure between 1988 and 2002. The objective of this study was to assess whether the downward trend in exposure continued from 2001 through 2006.

METHODS: We analyzed data from the 2001-2006 NHANES to estimate exposure of nonsmokers to SHS. Geometric means of serum cotinine levels for all nonsmokers were computed.

RESULTS: Overall serum cotinine levels (95% Confidence Intervals) in 2001-2002, 2003-2004, and 2005-2006 were 0.06 ng/mL (0.05-0.07), 0.07 ng/mL (0.06-0.09), and 0.05 ng/mL (0.05-0.06), respectively. Subgroup analysis by …

A Cross Country Study Of The Role Of The Three Sectors On The Acceptance And Integration Of People With Intellectual And Physical Disabilities In Kenya And The United States Of America, Whitney Nash

This study examines acceptance and integration of people with intellectual and physical disabilities in the United States and Kenya and the involvement of the three sectors. The literature review revealed that significant progress was made over the past century in the United States, and the past decade in Kenya in treatment of people with disabilities, but there are still inefficiencies. Respondents to the survey reported that their clients struggle with acceptance and integration in their local community, and that services provided by the public and private sectors are inadequate. This study analyzed GE Centricity Electronic Medical Record (EMR) data to examine the effects of body mass index (BMI) and obesity, key risk factor components of metabolic syndrome, on the prevalence of 3 chronic diseases: type II diabetes mellitus, hyperlipidemia, and hypertension. These chronic diseases occur with high prevalence and impose high disease burdens. The rationale for using Centricity EMR data is 2-fold. First, EMRs may be a good source of BMI/obesity data, which are often underreported in surveys and administrative databases. The Journal of Sociology & Social Welfare

According to skew selection theory, working citizens who build wealth and, at the same time, share portions of their wealth with those in need are more likely to survive economic downturns than citizens who hoard wealth. In this article, skew selection is employed as a theoreticalframework to support governmental efforts to develop social policies that protect the income of working citizens and, at the same time, provide for vulnerable, non-working children and elders. Background: Vitamin D deficiency in the US is more prevalent than before estimated. Given the numerous roles vitamin D plays in the body, it is becoming apparent that the need for adequate intake is vital. It is generally accepted that adults require 1,000 IU per day.

Methods: Electronic searches were conducted using Google Scholar and Elsevier databases. Search terms included “vitamin D deficiency”, “vitamin D deficiency US”, and “vitamin D screening”. Databases were reviewed for prevalence of vitamin D deficiency in the US.

Objective: To assess the impact of a national immunization training certificate program on the perceived knowledge, skills and attitudes of pharmacy students toward pharmacy-based immunizations.

Methods: The study design utilized a pre- and post- survey administered to pharmacy students before and after the American Pharmacists Association’s (APhA) Pharmacy-Based Immunization Delivery program. The primary outcome explored was a change in the perceived knowledge, skills, and attitudes of the pharmacy students. A five-point Likert scale (i.e. strongly agree = 5, strongly disagree = 1) was used for measuring the main outcomes, which was summated by adding the individual item scores in each …
