STUDY OBJECTIVE: To investigate whether the phase of menstrual cycle influences the incidence of postoperative nausea and vomiting (PONV) in women undergoing general anesthesia for elective laparoscopic gynecological surgery.
DESIGN: Prospective, observational, blinded study.
SETTING: General hospital, Postanesthesia Care Unit, and gynecologic floor room.
PATIENTS: 111 ASA physical status 1 and 2 women, aged 18 to 53 years.
INTERVENTIONS: Patients were classified into three groups according to the phase of menstrual cycle at the time of anesthesia: Group F1: follicular phase (menstrual days 1-8; n = 34); Group O2: ovulatory phase (days 9-15; n = 40); and Group L3: luteal …

Some first ladies are often condemned for being too involved with the presidents' power in politics while other first ladies find themselves condemned for the lack of involvement. First ladies, it seems, are damned if they do and damned if they don't. Consequently, Michelle Obama faces rhetorical problems that in some respects are similar to those of previous first ladies and in other respects are quite different. China’s engagement with Africa has drawn the attention of policy makers worldwide. While Chinese interests in the region appear relatively passive at present, there is the potential for either a detrimental or largely positive impact on US interests. The U.S. Department of State estimates that 600,000 to 800,000 victims are trafficked across international borders each year, approximately 14,500 to 17,500 of which are trafficked into the United States. The Victims of Trafficking and Violence Protection Act of 2000 (VTVPA) was created to combat human trafficking in the country, and as a result created the T-visa to provide immigration relief to non-citizen victims of trafficking. In this work I analyze U.S. government efforts to combat trafficking in the twelve years following implementation of the VTVPA. In June 1954, President Jacobo Arbenz Guzman of Guatemala resigned in the face of a coup led by Colonel Carlos Castillo Armas. While the United States publicly denied involvement, the coup was in fact the culmination of a plan called PBSUCCESS (CIA codeword), led by the Central Intelligence Agency (CIA). Although PBSUCCESS lived up to its namesake, it was aided (both intentionally and unintentionally) by various U.S. media outlets. For the duration of Arbenz Guzman's regime, he and his country had been the subject of U.S. suspicions of undue Communist and Soviet influence. Factors Affecting Social Attainment Among Individuals: Evidence From The National Educational Longitudinal Study 1988-2000, Michael Foleng Ndofor

This study draws from extant literature on social attainment to examine what factors affect the attaining of higher incomes, education, and occupational ranks. Research on five distinct factors is examined and analyzed using a sample of about 6,000 students from a national longitudinal study across the United States between 1988-2000 as they transitioned from eight grade through high school and into the labor force: (1) background characteristics: household type, race, and gender, (2) social capital, (3) cultural capital, (4) academic ability, and (5) parental social class. With full-time jobs, hourly wages are appropriate primary indicators of job quality. However, in sectors where full-time schedules do not dominate, total hours matter for job quality and worker outcomes. We explored hour levels and trends in retail trade and its largest subsector, grocery stores. Retail is known for part-time and short shifts. With a comparison of retail hours in three countries—the United States, Canada, and Mexico—we contribute insights into aspects of the U.S. policy and regulatory systems that could be altered in order to improve retail jobs.

When America’s founding fathers sought to create their new nation, they turned to the reliability and practicality of the Roman Republic. While careful to not create a carbon copy of the Republic, the founders drew inspiration from Rome’s symbolism and everyday life, government, philosophy, military strategies, and religious tolerance. This paper will highlight the similarities that exist between the Roman Republic and the United States in the above mentioned five areas. Much of Rome’s example has outlived both Rome and the founding fathers in its effectiveness and viability. The Roman Republic may have fallen, but her influence lives on through …

A project began in 2002 to develop a database tracking all the admissions to the Sisters of Charity of Saint Joseph’s (1809–1849) and the Daughters of Charity (1850–1909) in the United States. This article explains how the project was conducted, what sources were used, and what the study’s major findings were. The database encompassed many different demographics: family backgrounds, birthplaces, and age of entrants, their marital and/or convert status, their years of entrance, and whether they had relatives in the community. Military service, withdrawals, transfers, separations, and burial places were also tracked.

Recently, a Surveillance Epidemiology and End Results (SEER) survey of melanoma patterns of care by the Mayo Clinic, Scottsdale showed remarkable deviations from best practice patterns throughout the country. The study, which analyzed the SEER records of 35,126 stage I to III cutaneous malignant melanoma patients treated from 2004 to 2006, showed that adherence to National Comprehensive Cancer Network (NCCN) therapeutic resection margins occurred in less than 36% of patients. Similarly, considerable variation in the quality of melanoma care in the United States when assessed using 26 quality indicators drawn by a panel of melanoma experts was independently reported. This study presents a semiotic analysis of several magazine advertisements in an attempt to explore body image and sexuality as it is illustrated in marketing campaigns in both the United States and Japan. Each of the magazines from which the artifacts were drawn was published in 2011, and each presents a similar focus on fitness, fashion, and television in each country. A comparison of the sexuality portrayed in advertisements was conducted to explore similarities and differences. It was found that there is a difference in how sexual imagery was used between the United States and Japan, with the United States …

It mentions that the separation of powers has always worked in favour of development of constitutional structure despite of the political incentives and ambitions of elected officials. The article presents information on the adoption of law "Uniform Determination of Death Act (UDDA)" by the U.S. government that defines the criteria for determining death. It discusses the neurological standard for determining death that is the cessation of integrative functioning of the brain, the cardiopulmonary system, and the respiratory system. It further discusses the use of legal fictions to retain both the neurological criterion and the dead donor rule.

More than two years after the official end of the Great Recession, state governments still face significant budget deficits that cannot be addressed without further drastic spending cuts or substantial revenue increases. The structural origins of the ongoing state fiscal crisis are well known. Excessively procyclical revenue structures, combined with spending obligations that increase with economic downturns, have resulted in a budget dynamic for the states that is not sustainable over the long term. The consensus solution to this problem is for states to save money during boom times (via budget stabilization or “rainy day” funds) and to draw on …

The home remains a very common location for deadly injuries among children younger than 5 years. The aim of this study is to describe the demographic and injury characteristics of domestic injuries in children younger than 5 years. The National Trauma Data Bank's National Sample Program data set was queried for children younger than 5 years with the injury site classified as home. Bivariate analysis was performed to determine unadjusted differences by ethnicity. Appropriate weight was applied to the sample to determine accurate national estimates. President Obama has proposed that the United States increase the number of college graduates by 8 million by 2020 in order to raise national educational standards and to engage in global competition. Obama’s policies and rhetoric frequently describe “education as a pathway to success” and “education as a pathway to jobs,” but fail to emphasize its comprehensive, intrinsic values. America’s national discussion on education needs to shift away from framing higher education merely as pathway to the workforce, and should focus instead on education as a vital part of a well-lived life. In settings where most workers have full-time schedules, hourly wages are appropriate primary indicators of job quality and worker outcomes. However, in sectors where full-time schedules do not dominate—primarily service-producing activities—total hours matter, in addition to hourly wages, for job quality and worker outcomes. In this paper we employ a sector-focused, comparative framework to further examine hours levels—measured as average weekly hours—and trends in Canada, the United States, and Mexico. AIM: Financial barriers are a major factor of slow electronic health record (EHR) adoption among US hospitals. All existing literature focuses on relationships between current or short-term financial position and EHR adoption. This study examines relationship between financial position in previous years and the current level of EHR adoption.
METHODS: Retrospective longitudinal data were extracted from (1) the 2009 American Hospital Association (AHA) EHR implementation survey; (2) the 2002 and 2006 Centers for Medicare & Medicaid Cost Reports; and (3) the 2002 and 2006 AHA Annual Survey containing organizational and operational data. BACKGROUND: Fibrinogen is a marker of systemic inflammation and may be important in the pathogenesis and progression of chronic obstructive pulmonary disease (COPD).
METHODS: We used baseline data from Atherosclerosis Risk in Communities and Cardiovascular Health Studies to determine the relation between fibrinogen levels and COPD and to examine how fibrinogen levels at baseline affected outcomes of death, development of COPD, lung function decline, and COPD-hospitalizations.
RESULTS: Our study sample included 20,192 subjects, of whom 2995 died during the follow-up period. The mean fibrinogen level was 307.6 mg/dL and 10% of the sample had levels >393.0 mg/dL. Subjects with Stage …
