Myocardial Dysfunction After Out-Of-Hospital Cardiac Arrest: Predictors And Prognostic Implications., Yuan Yao, Nicholas James Johnson, Sarah Muirhead Perman, Vimal Ramjee, Anne Victoria Grossestreuer, David Foster Gaieski

Department of Emergency Medicine Faculty Papers

We aim to determine the incidence of early myocardial dysfunction after out-of-hospital cardiac arrest, risk factors associated with its development, and association with outcome. A retrospective chart review was performed among consecutive out-of-hospital cardiac arrest (OHCA) patients who underwent echocardiography within 24 h of return of spontaneous circulation at three urban teaching hospitals. Our primary outcome is early myocardial dysfunction, defined as a left ventricular ejection fraction < 40% on initial echocardiogram. We also determine risk factors associated with myocardial dysfunction using multivariate analysis, and examine its association with survival and neurologic outcome. A total of 190 patients achieved ROSC and underwent echocardiography within 24 h. Of these, 83 (44%) patients had myocardial dysfunction. A total of 37 (45%) patients with myocardial dysfunction survived to discharge, 39% with intact neurologic status. History of congestive heart failure (OR 6.21; 95% CI 2.54-15.19), male gender (OR 2.27; 95% CI 1.08-4.78), witnessed arrest (OR 4.20; 95% CI 1.78-9.93), more than three doses of epinephrine (OR 6.10; 95% CI 1.12-33.14), more than four defibrillations (OR 4.7; 95% CI 1.35-16.43), longer duration of resuscitation (OR 1.06; 95% CI 1.01-1.10), and therapeutic hypothermia (OR 3.93; 95% CI 1.32-11.75) were associated with myocardial dysfunction. Cardiopulmonary resuscitation immediately initiated by healthcare personnel was associated with lower odds of myocardial dysfunction (OR 0.40; 95% CI 0.17-0.97). There was no association between early myocardial dysfunction and mortality or neurological outcome. Nearly half of OHCA patients have myocardial dysfunction. A number of clinical factors are associated with myocardial dysfunction, and may aid providers in anticipating which patients need early diagnostic evaluation and specific treatments. Early myocardial dysfunction is not associated with neurologically intact survival.

State Court Litigation: The New Front In The War Against Partisan Gerrymandering, Charlie Stewart Jun 2018

State Court Litigation: The New Front In The War Against Partisan Gerrymandering, Charlie Stewart

Michigan Law Review Online

Partisan gerrymandering is the process of drafting state and congressional districts in a manner that gives one political party an advantage over another. The end goal is simple: help your party win more seats or protect existing ones. The tactic is as old as the United States. In 1788, Patrick Henry convinced the Virginia state legislature to draw the 5th Congressional District to pit his rival James Madison against James Monroe. The term “gerrymander” itself is a hybrid: in 1810, democratic Governor Gerry signed a partisan redistricting plan into law—one that contained a district that infamously looked like a salamander. …

Modeling The Health Effects And Economic Costs Of Particulate Matter Emissions From High Volume Hydraulic Fracturing, Marissa Steinheimer May 2018

Modeling The Health Effects And Economic Costs Of Particulate Matter Emissions From High Volume Hydraulic Fracturing, Marissa Steinheimer

Theses

With natural gas drilling on the rise (Penn State, 2012), there is a general lack of data on the emissions from the entire lifecycle of hydraulic fracturing. This research project is designed to study the health impacts of emissions from high volume hydraulic fracturing (HVHF) production and at HVHF well sites. Using data from previous research (Korfmacher et al., 2015 and Korfmacher et al., 2016) and the Environmental Protections Agency's Environmental Benefits Mapping and Analysis Program - Community Edition (BenMAP-CE), estimated health impacts and economic costs of emissions are analyzed. This study models the health impacts and economic costs of …

Harrisburg University Research Symposium (2018 & 2019)

The social issue this project deals with is the budget disparity in America's public schools. This problem is important to investigate because today's children are tomorrow's leaders and if they are not being educated properly, then how do we as a society expect them to lead us in the future.

Assisting Mid-Atlantic Wine Industry Stakeholders In Developing Consumer-Centric Marketing Strategies: Internet Survey Results, Abigail Miller, Kathleen M. Kelley, Denise M. Gardner, Ramu Govindasamy, Jeffrey Hyde, Bradley Rickard, Karl Storchmann

Journal of Extension

Two Internet surveys were administered to wine consumers in New Jersey, New York, and Pennsylvania. Purchasing frequencies, consumption occasions, and sources used to learn about wine were documented. National data are readily available; however, the study discussed in this article focused on Mid-Atlantic consumers, whose behaviors and attitudes were not well understood. These data provide Extension personnel in the region with the ability to better inform industry members about their clientele and help them develop marketing strategies that appeal to local wine drinkers.

Migrant Farmworkers' Perceptions Of Pesticide Risk Exposure In Adams County, Pennsylvania: A Cultural Risk Assessment, Micaela S. G. Edelson, Salma Monani, Rutherford V. Platt

Environmental Studies Faculty Publications

Agricultural exceptionalism, a system in which regular labor laws and standards do not apply to farm labor, makes migrant farmworkers particularly vulnerable populations—economically, socially, and in terms of environmental health. To address inequities inherent in migrant farmworker marginalization, studies advocate for actively engaging the migrant farmworker population in the conversation surrounding these issues. We conducted 40 semistructured interviews with migrant farmworkers in Adams County, Pennsylvania, to understand pesticide risk exposure perceptions and practices. We employed the Health Belief Model as our cultural risk assessment frame, using it in combination with technical risk assessment, which uses government calculations (from the Environmental …

An Evaluation Of Juvenile Lifers In Pennsylvania Pre And Post Act 33 Of 1995, Kieshia Martin

Walden Dissertations and Doctoral Studies

Act 33 of 1995 is an amendment to Pennsylvania's Juvenile Act (2008). This amendment changed jurisdictional boundaries for juvenile offenders who committed violent crimes with weapons. As a result, youth who committed violent crimes with weapons were automatically transferred to the adult criminal justice system. Using punctuated equilibrium as the theoretical foundation, the purpose of this study was to determine if Act 33, as a punctuating event, resulted in an increase in the number of youth transferred to the adult criminal justice system and sentenced to life without the possibility of parole in an urban county in Pennsylvania. Secondary data …
