The first two years after a child’s death, defined by parents as early bereavement, are a particularly vulnerable period for parents. More than 40% of bereaved parents report severe, prolonged grief symptoms during early bereavement. High-quality parent-clinician communication, which is delivered honestly, clearly, and with sensitivity, helps parents make informed decisions about medical care, address their complex needs during their child’s end-of-life (EOL), and is related to lower severity of prolonged grief symptoms many years after a child’s death. Non-alcoholic fatty liver disease (NAFLD), now known as metabolic dysfunction-associated steatotic liver disease (MASLD), is the leading cause of chronic liver disease in the world. The mechanisms involved in its development and progression continue to be a mystery that scientists are trying to unravel. However, many research studies found a correlation between uncontrolled obstructive sleep apnea (OSA) and worsening of NAFLD. Chronic intermittent hypoxia and sleep fragmentation in OSA increase sympathetic activity and promote oxidative stress, pro-inflammatory cytokine production, and metabolic dysregulation, which increases the risk of various diseases, including NAFLD/MASLD progression. Currently, there is an epidemic of elders in nursing homes with swallowing difficulties, Alzheimer's, and other maladies, which cause diminished decision-making capacity and initiate the discussion of whether to insert a gastrostomy tube. Furthermore, there is an overriding belief among health care practitioners that once inserted the tube will remain until the death of the resident.

Purpose: Discharge processes for patients with heart failure on home intravenous inotrope support is fraught with barriers and complexities, and causes patients, caregivers, and bedside nurses’ high levels of stress leading to potentially unsafe, complicated discharges. This project aimed to decrease discharge times and improve nurses’ ability to safely and effectively discharge patients.

Background: The project was implemented in a 42-bed advanced heart failure IMU in the Texas Medical Center, where discharge times were longer than hospital standards at over 6 hours.

The Centers for Disease Control and Prevention (2020) estimated about 24% of adults in the United States live with a disability. However, the literature suggested only 4.4% of healthcare workers reported having a disability in the United States. Stigmatization and ableism have likely led to nurses hiding their disabilities while practicing, creating less diversity and representation in practice. When healthcare workers have little resemblance to the population they serve, it can lead to distrust, uncaring behaviors, and feelings of exclusion. This leads to limited availability of home health services to patients in the community and can also contribute to increased length of stay for patients in acute care hospitals. However, there is lack of evidence and research relating to the many students of other healthcare professions who may encounter sharps instruments. The Comprehensive Mobility Evaluation Tool (CMET) was developed with skill-based testing procedures to assess patient mobility status, predict level of assistance (LOA), and promote mobility. The Advocate Health (AH) Midwest (MW) Region Internal Agency was developed in the peak of the pandemic in response to the growing volumes of nurses leaving their positions for travel. Teammates leaving (interceptions) or who had left (boomerangs) their positions were offered the opportunity for a travel assignment within the MW Region. Internal agency rates were set at 60% of the current average bill rate for external agency, and in the first year, over $12 million were saved by utilizing internal agency nurses in lieu of external agency. Recent stressors, such as COVID-19, have provided new challenges for healthcare workers, especially nurses, and have contributed to the increase in burnout rates in the hospital setting. Therefore, relief in this space is critical. Ample research demonstrates that mindfulness-based interventions (MBIs) may help to improve well-being among healthcare providers more broadly. Literature supports that the practice of mindful communication, in particular, is helpful, not only in improving patient outcomes, but also in decreasing interpersonal stress among healthcare workers.
