Evaluation Of Testing And Implementation Of Evidence-Based Rn Bedside Swallow Screen For Dysphagia: A Clinical Practice Change, Edith Matesic

Stroke patients initially experience dysphagia approximately 42 - 67% of the time with noted improvement reducing rates to approximately 43% at seven days post-acute event (Perry & Love, 2001). The dysphagia experienced by these patients makes them at high risk to develop aspiration pneumonia. Overall, the financial impact of reducing aspiration pneumonia reduces the national cost of morbidity, mortality, and disability burden in this population. There is no generally recognized swallow screen. Background. Despite the consequences of physical inactivity, it is under recognized and not addressed consistently in primary care. The Healthy People 2010 recommendations are that adults should engage in at least 30 minutes of moderate physical activity on most, preferably all, days of the week. Clinical practice guidelines advise that primary care practitioners take the opportunity whenever possible to advise, discuss, negotiate, or encourage physical activity in inactive adults. Objective. The primary objective was to identify beliefs about physical activity that act as barriers among providers and to educate them about the current recommendations for physical activity. The purposes of this poster presentation are to 1) describe the implementation of a doctor of nursing practice (DNP) program by providing access to rigorous distance education to students living in rural Pennsylvania; 2) discuss building a critical mass of doctorally prepared advanced practice nurse experts in both urban and rural communities; and 3) share formative and summative evaluation information.

Through support from the Department of Health and Human Services, Health Resources and Services Administration, Bureau of Health Professions, Division of Nursing, the Jefferson School of Nursing (JSN) has expanded its Adult, Family and Neonatal Nurse Practitioner programs, currently offered at the urban Center City Campus, to the rural Danville Campus. In addition, the Doctor of Nursing Practice (DNP) program is offered at both sites. Nursing job satisfaction is related to nursing retention. During this current nursing shortage, nursing leaders need to investigate ways to improve work environments to increase nurse satisfaction. Research has shown nurse job satisfaction greatly impacts the quality of patient care and nurse retention. This research project implemented a nursing model of practice that included organizational values that could be linked to improved job satisfaction. Responding to changes in technology, economics, and the demographics of society, nurse leaders incorporate new ideas and systems into clinical practice. Nurse educators are challenged to adopt these ideas and systems and make them part of the nursing curriculum. Educators develop tools and strategies to help students gain knowledge and skills to work with patients in the clinical setting. Lack of staff nurse participation in hospital decision-making has been cited as a major reason for the dissatisfaction in nursing. Shared governance has been proposed as an organizational model that provides staff nurses with both the structure and the mechanism for having increased decision-making authority. The purpose of this study was to investigate the relationship of organizational culture, perceived importance of involvement and actual involvement in decision-making, the discrepancy between importance and involvement, staff nurse years of involvement in shared governance, control over nursing practice, and work satisfaction among staff nurses working in hospitals with shared governance. Recent innovations in patient care delivery and nursing governance require an empowered workforce. However, little is known about what combination of organizational, leadership, and personal characteristics maximize the prediction of registered nurse empowerment. The purpose of this study was to develop and test an explanatory model of registered nurse empowerment. The predictor variables included personal characteristics of registered nurses (age, experience, organizational tenure, education, and position level of clinical and/or management responsibility), perceptions of organizational culture, and connective leadership.
