Diabetes is increasing globally and nationally. Diabetes complications and costs can be reduced through modification of lifestyle risks and diabetes self-management education (DSME). The Cooperative Extension System (CES) is uniquely positioned to implement DSME. This study assessed the role and impact of the Cooperative Extension System (CES) in DSME. A survey was sent to CES professionals throughout the U.S. a total of 43 participants provided information on 73 DSME programs. Most participants were from the South (n=22, 51.16%) and Midwest (n=12, 27.91%) and most programs targeted adults with and at risk for type 2 diabetes. Most programs were developed and …

Image-based dietary records have limited evidence evaluating their performance and use among adults with a chronic disease. This study evaluated the performance of a 3-day mobile phone image-based dietary record, the Nutricam Dietary Assessment Method (NuDAM), in adults with type 2 diabetes mellitus (T2DM). Criterion validity was determined by comparing energy intake (EI) with total energy expenditure (TEE) measured by the doubly-labelled water technique. Relative validity was established by comparison to a weighed food record (WFR). Inter-rater reliability was assessed by comparing estimates of intake from three dietitians. Resistant starch (RS) reaches the large intestine for fermentation and is considered a prebiotic. We wanted to determine if we could reduce fermentation for future mechanistic studies and how there is improved insulin sensitivity in human studies with or without increased incretin hormone glucagon-like peptide 1 (GLP-1). Two main studies were performed. In the first main study, low potency antibiotics (Ampicillin 1g/L and Neomycin 0.5g/L) were added to the drinking water of rats to reduce fermentation of RS. With larger numbers of immigrants coming to the United States who are of Hispanic origin, the individual risk and health burden of this disease will have a major impact on the quality of life and the health care system. Research into the influence of the timing of changes in lifestyle suggested an association between specific levels of socioecological exposures and certain health conditions. An increase in cellular inflammatory biomarkers directly increases the risk of cardiovascular disease (CVD). Using the social ecological and biomedical theories, the study examined quantitatively how specific inflammatory biomarkers are associated with cardiorenal syndrome (CRS), a potential complication of hypertension and diabetes, and how sociodemographic factors modify this association in the U.S. adult population. A retrospective secondary data analysis of the data collected from National Health and Nutrition Examination Survey (NHANES) 1999-2010 was utilized to evaluate these hypotheses. OBJECTIVE: The objective of this selective EBM review is to determine if a vegetarian diet is more effective in improving quality of life in Type 2 Diabetics compared to other diabetic diets.
STUDY DESIGN: Review of two randomized control trials published in 2011 and 2013, and one prospective clinical intervention study published in 2010.
DATA SOURCES: Data sources were articles published in peer review journals found using PubMed and Cochrane Databases.
OUTCOMES MEASURED: Quality of life was measured by amount of weight loss from baseline and Obesity and Weight-Loss Quality of Life (OWLQOL) questionnaires.

The purpose of this research was to examine food security for people with prediabetes participating in the Supplemental Nutrition Assistance Program (SNAP), focusing on their food choice decisions and coping strategies over a 30-day benefit cycle that potentially increases the risk of Type 2 diabetes. A cross-sectional, quantitative design based on food choice process model constructs was used. SNAP participants (n = 36) with prediabetes, aged 21-70 years, were recruited as outpatients from Bronx Lebanon Hospital and completed self-reported questionnaires on demographics and health, food security, and food frequency. The dramatic increase of type-2 diabetes within the Latino community is of great concern in the U.S., especially among Mexican im/migrant farmworkers. Anthropological scholarship shows that health issues within im/migrant groups in the U.S. are poorly understood from a sociocultural and local perspective. In Indian River County, Florida, farmworker leaders created a community garden in response to health problems in this community. Using institutional ethnography and its approach to mapping institutional sequences (Smith, 2005; Turner, 2006), this paper examines the social organization of School Health Support Services (SHSS) for children with diabetes in Ontario schools. The inquiry starts with my own situated experience as a mother of a child with diabetes starting kindergarten, and the trouble of securing the health supports necessary to care for my child’s health and safety while she is at school. The American Diabetes Association (ADA) data suggest that Latinos share a greater burden of disease than do non-Latino Whites with type 2 diabetes. As a result of poor glycemic control, Latinos also suffer consequences such as blindness, kidney disease, and limb amputation more often than do other ethnic groups. The effects of alpha lipoic acid and vitamin C supplementation on TGF \alpha and TGF \beta2 secretion in the testicular tissue of streptozotocin-induced diabetic rats were examined by immunohistochemical methods. Rats were divided into 5 groups; group 1 (control), 2 (diabetic control), 3 (diabetes + alpha lipoic acid), 4 (diabetes + vitamin C), and 5 (diabetes + alpha lipoic acid + vitamin C). Hematoxylin and eosin staining was performed in order to examine testicular tissue histopathologically. Diabetes is among the fastest growing diseases in the world. In the U.S., Hispanics are the third most affected population. Nonetheless, there are Hispanics diagnosed with diabetes who have, against all odds, found solutions to manage their disease. This study focused on the intrapersonal and interpersonal acts and behaviors of "positive deviant" Hispanics living on the U.S. - Mexico border that effectively managed their diabetes. They are referred to as "positive deviants" because they accrue "positive" outcomes and "deviants" because they are not the norm.

Multiple studies have found that certain dietary patterns can significantly influence the risk of T2DM. The purpose of this study was to understand the dietary patterns associated with diabetes by comparing the relative adherence to these dietary patterns by individuals in 5 stages of diabetes: no diabetes, undiagnosed prediabetes, diagnosed prediabetes, undiagnosed diabetes, and diagnosed diabetes. The Salt River Pima Indians, prior to colonization, had a strong tradition of harvesting and food sovereignity. As the tribe adapted to a more Westernized diet which consisted mainly of processed food rations, the rate of diabetes began to skyrocket on the reservation and, at one point, the tribe had one of the highest per capita diabetes rates in the world. The American Diabetes Association® (ADA, 2013) estimated that diabetics in the United States incur $176 billion annually in direct medical costs for treatment with hospitalization being the main component of the expenditures. California has the largest population of diabetics and the highest annual cost at $27.6 billion (ADA, 2013). Although the American Diabetes Association (ADA) adopted the use of the glycated hemoglobin (A1C) test as a method of diabetes and prediabetes diagnosis, the ADA has not developed firm guidelines concerning the use of the A1C test in children and adolescents, as research has not validated thresholds in this group. An Extension diabetes program (DP) was revised for Latinos; however, participation was limited. Factors influencing low participation rates were examined. Five Latinos interested in the DP participated in a focus group discussion. Transcripts were analyzed for themes. Preferred education programs were multi-session, local, group classes led by an engaging teacher during the summer. Participants learned of the DP through friends and Extension; preferred marketing strategies were Spanish radio, local health clinics, and Spanish-print media. Attendance barriers included scheduling conflicts. Limited use of culturally preferred marketing and scheduling conflicts were likely barriers. Depression is common among persons with diabetes and associated with adverse health outcomes. To date, little is known about the causal mechanisms that lead to depression in diabetes. The aim of the present study was to examine to which extent functional and self-rated health mediate the association between physical health and depressive symptoms in diabetes. Data of n = 3222 individuals with type 2 diabetes were analyzed cross-sectionally and longitudinally at three measurement occasions using path analysis. Periodontal disease is an inflammatory disease caused by polybacterial infection and inflammation plays key role that associates periodontal disease with different systemic diseases including diabetes and cardiovascular diseases. The main objective of this study is to evaluate and explore the association of serum IgG antibodies in four distinct clusters that were formed empirically from species specific 19 periodontal antibody titers with T2DM and pre-diabetes adjusting for known confounders such as age, sex, race-ethnicity, income to poverty ratio, education, smoking and drinking alcohol, BMI, WC, physical activity, missing teeth, dentist visits and other nutritional factors.

What You Don’T Know Can Hurt You: Early Life Course Racial Health Disparities In Undiagnosed Diabetes, Anna C. Bellatorre

This dissertation addresses several issues related to racial health disparities in undiagnosed diabetes in American young adults in a three-article format. The first chapter examines rates of diabetes severity across age-matched samples of young adults from two large nationally representative studies. Although the purpose of this study was to explore the impact of nonresponse on prevalence estimates, I find that the prevalence discrepancies have less to do with which respondents are missing blood samples and more to do with the samples coming from initial samples that are not equivalent.

Implementation and adherence to screening recommendations of Type 2 Diabetes (T2D) clinical practice guidelines are associated with earlier diagnosis and treatment. Standardized T2D screening helps ensure consistency of care and decrease unnecessary testing by targeting those at greatest risk for developing the disease. The aim of this quality improvement pilot project was to facilitate standardized T2D screening of inmates within a correctional system. The number and frequency of selected preexisting major T2D risk factors identified within a sample of inmates diagnosed during incarceration were described.
