This study tested a grounded theory model, Getting it Together: A Learning Model of Community Collaboration, developed during a six-year study (2004-2010) of a statewide substance abuse prevention program funded by the Substance Abuse and Mental Health Services Administration (SAMHSA) and the Center for Substance Abuse Prevention (CSAP). The model features the perceptions and experiences of those who were active in community-based collaborative and educational work in West Virginia focused on substance abuse prevention. This project was designed to evaluate the effectiveness in improving the quality of life among heart failure patients with the addition of the role of Community Health Worker (CHW) to a current continuum case management model. The study also addressed the increased need for more appropriate and cost-effective chronic care management for heart failure patients. Hospitals and healthcare systems are being challenged to find innovative ways to decrease readmissions, decrease unnecessary emergency room visits, increase patient adherence, and manage chronic disease, all improving the patient’s overall quality of life. Contemporary Rural Social Work Journal

This article presents an interprofessional case study approach to serving the social service and health needs of vulnerable persons living rural communities. This project, the Congregational Social Work Education Initiative (CSWEI), is funded by a health care foundation. Persons in rural areas are often at risk for poverty, homelessness and lack of access to needed health and social services. The case study demonstrates the opportunities for collaboration between professional social work, religiously affiliated organizations (RAOs) and nursing in order to reduce health and mental health disparities among residents in rural areas.

A qualitative research study was conducted and data were collected by interviewing university professors on their perceptions about the impact of integrating Google Apps, as a means of classroom instructional delivery, on students’ communication and collaboration skills. The participants consisted of eight university professors from a major university, who integrate, or had previously integrated at least two Google Apps Education Edition collaborative tools into their instructional strategies. Unlocking Public Value: An Evaluation Of The Impact Of Missouri's Great Northwest Day At The Capitol Program, Wilson Magee, Beverly A. Maltsberger

The study reported here is an evaluation of the public value of a regional public policy engagement program. Data were obtained through surveys and document analysis. The study observed peer-learning and networking opportunities as some of the most impactful elements of GNWD at the Capitol in creating public value. Building coalitions of interest among diverse sectors of the population strengthens the development of public value. For Extension, these findings suggest that programming that focuses on collaborative efforts holds promise for sustaining communities. Extension can be the glue that permeates professional/sectoral boundaries and bonds the different fields together.

This article is a case study of a collaboration between the Oregon Multicultural Archives of Oregon State University, Portland State University Library's Special Collections, the Chinese Consolidated Benevolent Association (CCBA), and the Northwest News Network to preserve and make accessible a recovered box of Oregon Chinese disinterment documents. By examining what influenced and engaged each partner, this case study offers an opportunity to better understand the motivations of diverse stakeholders in a "post-custodial era" project that challenges traditional practices of custody, control, and access.

This dissertation encompasses a case study and a Participatory Action Research project. The case study focuses on climate change mitigation activities within King County, Washington and its 39 cities and towns and discusses progress and challenges related to transportation issues, efficiency measures, and sustainability planning. The findings indicate there is a high level of activity in waste reduction, environmental outreach and education, bicycle and pedestrian promotion, tree canopy protection, sustainability policies, and green building. Other categories, such as energy efficiency, electric vehicle infrastructure, and greenhouse gas emission inventories and goal setting are on the rise. Twelve of the cities were …
