Examining Proning Covid-19 Patients With Continuous Enteral Feedings: A Retrospective Study, Michael Legal, Sari Winham, Jennifer Ernst

The COVID-19 pandemic introduced a unique patient population requiring a triad of therapies: oxygen, nutrition, and patient positioning support. This study investigates the risk of aspiration pneumonia in COVID-19 patients placed in a prone position while receiving continuous enteral nutrition (EN). The retrospective, single-center study reviewed 97 electronic health records (EHR) of patients admitted between March 15, 2020, and June 1, 2022, at a southwest academic medical center. Patients included were diagnosed with COVID-19, aged 18 or older, and received continuous EN via nasogastric or nasoduodenal tubes while on high-flow oxygen support, excluding those on mechanical ventilation.

IMPORTANCE: Studies of COVID-19, influenza, and respiratory syncytial virus (RSV) disease clinical epidemiology, disease burden, and vaccine effectiveness often rely on clinical testing to identify cases, particularly with increasing use of electronic health record data. Importance: COVID-19 remains a disease with high burden in the US, prompting continued debate about optimal targets for annual vaccination.

Objective: To project COVID-19 burden in the US for April 2024 to April 2025 under 6 scenarios of immune escape (20% and 50% per year) and levels of vaccine recommendation (no recommendation, vaccination for individuals at high risk only, vaccination for all eligible groups) and to assess the potential benefit of vaccine recommendations in reducing disease burden.

Background: 2D-speckle tracking echocardiography may help detect subclinical ventricular dysfunction, but data in multisystem inflammatory syndrome in children (MIS-C) are scarce. We investigated left ventricular (LV) strain parameters in MIS-C and their association with outcomes.

Methods: We performed an ambi-directional, 32-center cohort study on hospitalized patients with MIS-C (March 2020-November 2021) with at least 1 echocardiogram read by the Core Lab. Background: Multisystem Inflammatory Syndrome in Children is characterized by high rates of acute cardiovascular involvement with rapid recovery of organ dysfunction. However, information regarding long-term sequelae is lacking. We sought to characterize the systolic function and myocardial tissue properties using cardiac magnetic resonance (CMR) imaging in a multicenter observational cohort of patients with Multisystem Inflammatory Syndrome in Children.

Objective: Analyze the impact of COVID-19 on outpatient mental health (OP-MH) utilization patterns, particularly telehealth, across eight different MH conditions with a 2-year follow-up.

Methods: This population-based cohort study uses a 5% random sample of Medicare beneficiaries aged ≥18 and newly diagnosed with one of the eight MH disorders: psychotic, depressive, bipolar, anxiety, trauma-related, substance-use, other MH disorders, and two or more MH disorders. This study examined how COVID-19 disrupted adolescent education, social interactions, and mental health by conducting exploratory cluster analysis to identify distinct profiles of technology use and physical activities, then comparing resulting clusters on demographics, social support, and mental health dimensions. Using the first wave of the longitudinal Oxford Achieving Resilience during COVID-19 (ARC) study of 1,196 United Kingdom adolescents aged 13-18, three clustering algorithms (k-means, k-medoids, and agglomerative hierarchical) identified optimal groupings validated both statistically and empirically. MATERIALS AND METHODS: This retrospective study included ambulatory physicians in 11 specialties at 2 large academic medical centers (Washington University in St Louis [WashU], University of California San Francisco [UCSF]). Meaning-making systems underlie perceptions of the efficacy of threat-mitigating behaviors. Religion and science both offer threat mitigation, yet these meaning-making systems are often considered incompatible. Do such epistemological conflicts swamp the desire to employ diverse precautions against threats? Or do individuals—particularly individuals who are highly reactive to threats—hedge their bets by using multiple threat-mitigating practices despite their potential epistemological incompatibility? The International Network of Special Immunization Services (INSIS) was established to investigate the causes and risk factors of rare adverse events following immunizations (AEFIs) and develop immunization strategies for mitigating or preventing risk for individuals with prior AEFIs or at risk of AEFIs. INSIS integrates clinical data with multi-omic technologies (e.g., transcriptomics, proteomics, metabolomics) through a global consortium of clinical networks, leading immunology, pharmacogenomics teams to uncover the molecular mechanisms behind AEFIs. COVID-19 infection, particularly with the Delta variant, has been associated with a heightened risk of thromboembolic events due to a profound hypercoagulable state. While venous thrombosis is more commonly reported, arterial thrombosis—including acute limb ischemia—has emerged as a serious complication with significant morbidity and mortality.

We report the case of a 53-year-old man with severe COVID-19 pneumonia who developed recurrent acute limb ischemia secondary to an aortic mural thrombus. This qualitative study examines the long-term effects of the pandemic on these student populations, exploring shared and divergent experiences, coping strategies, and evolving priorities. While both groups faced heightened anxiety and academic disruptions, international students encountered additional stressors, including cultural displacement, visa uncertainties, and limited support networks. The Nucleocapsid protein (N) of SARS-CoV-2 plays a critical role in the viral lifecycle by regulating RNA replication and by packaging the viral genome. N and RNA phase separate to form condensates that may be important for these functions. Both functions occur at membrane surfaces, but how N toggles between these two membrane-associated functional states is unclear. Here, we reveal that phosphorylation switches how N condensates interact with membranes, in part by modulating condensate material properties. Environmental jolts are sudden, large-scale disruptions that challenge organizational and societal stability and demand new ways of understanding adaptation across individuals and firms. This two-paper dissertation explores the human and organizational dimensions of such jolts through the empirical context of the COVID-19 pandemic. Paper 1 examines how global tourism firms navigated the crisis, introducing the constructs of global stakeholder interconnectedness and stakeholder prioritization fluidity to refine stakeholder theory. The findings show how international exposure shaped communication strategies under duress. COVID-19-associated thrombotic microangiopathy has emerged as a severe complication that exacerbates morbidity and mortality in critical cases. Thrombotic microangiopathy, characterized by microvascular thrombosis and endothelial injury, includes conditions like thrombotic thrombocytopenic purpura and atypical hemolytic uremic syndrome. This review investigates therapeutic plasma exchange as a potential strategy to mitigate COVID-19-induced thrombotic microangiopathy, examining its role in removing pro-inflammatory cytokines, immune complexes, and pro-thrombotic factors. Additionally, it highlights the synergistic effects when therapeutic plasma exchange is combined with treatments such as complement inhibitors and immunosuppressants. As intrinsic differences in humoral immune response to SARS-CoV-2 between children and adults remain unclear, we improved characterisation by defining the kinetics, specificity and function of antibodies to SARS-CoV-2 in children (n = 146, aged 9.4 ± 4.8 years with n = 257 samples) compared to adults (n = 85, aged 39.5 ± 15.2 years with n = 122 samples). Aim: The aim of this study was to present three cases of COVID-19-related mucormycosis mimicking endodontic pain and to discuss the relevant dental and medical literature for this potential life-threatening disease. Summary: Mucormycosis (previously called zygomycosis) is a rare but serious infection caused by a group of fungi called mucormycetes. Mucormycosis mainly affects people who are medically compromised and have systemic health conditions or take drugs that lower their immune response. Debilitating symptoms for many years can follow acute COVID-19 ("long COVID"), myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS), and various post-acute infection syndromes (PAISs). Together, long COVID and ME/CFS affect 60-400 million individuals, globally. Many similar underlying biological abnormalities have been identified in both conditions including autoantibodies against neural targets, endothelial dysfunction, acquired mitochondrial dysfunction, and a pro-inflammatory gut microbiome. Each of these abnormalities may directly cause some of the symptoms. In addition, the symptoms also may be caused by ancient, evolutionarily conserved symptomatic and metabolic responses to vital threats-sickness behavior and torpor-responses mediated by specific, recently discovered neural circuits.
