HCA Healthcare Journal of Medicine

Background

Hospital-acquired infections are associated with increased morbidity, mortality, costs and length of stay. Prior studies have linked increased hand hygiene compliance with reduced hospital-acquired infection rate. With the increased vigilance for personal and institutional hygiene practices during the COVID-19 pandemic, we hypothesized increased hand hygiene compliance and, as a result, decrease in hospital-acquired infections in our hospital.

Methods

Hand hygiene compliance data was provided by the hospital’s Quality Department. We queried and empirically analyzed local hospital-acquired infection data obtained from our Quality Department and the National Healthcare Safety Network. We compared local hand hygiene compliance rates before and after …

Laboratory Diagnostic Testing For Covid-19, Nuntra Suwantarat, Sidayu Suriya

Asian Medical Journal and Alternative Medicine

Laboratory tests play an important role for Corona Virus Disease 2019 (COVID-19) diagnosis. The diagnostic key point is an appropriate testing during the course of disease. In the early course of COVID-19 infection, nucleic acid amplification tests (NAATs) especially real-time reverse-transcription polymerase chain reaction (RT-PCR) of the respiratory specimen is the gold standard test for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) detection due to the highest sensitivity and specificity during the first 2 weeks after the exposure. A rapid antigen test for SARS-CoV-2 has less complexity of the testing procedure and the faster TAT. However, a rapid antigen test …

Gastrointestinal And Liver Manifestations Of Covid-19, Keerati Kiattikunrat, Sith Siramolpiwat

Asian Medical Journal and Alternative Medicine

Almost every country in the world has been affected by the pandemic of Coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), first reported in Wuhan, China, in December 2019. The clinical manifestation is diverse, ranging from asymptomatic to severe pneumonia requiring mechanical ventilation. In addition, gastrointestinal symptoms have also been reported to occur during the course of the disease. In some patients, gastrointestinal symptoms may precede respiratorytract symptoms. Interestingly, the SARS-CoV-2 RNA could be detected in the stool of COVID-19 patients, implying that SARS-CoV-2 may be transmitted by fecal-oral route. Moreover, mild transient abnormal liver …

Covid-19 And Pulmonary Involvement, Narongkorn Saiphoklang, Nuttapol Rittayamai

Asian Medical Journal and Alternative Medicine

The COVID-19 pandemic results in the largest global health crisis. Most patients are asymptomatic. Pneumonia usually develops in about one week after infection, with approximately 5% of patients needing ICU admission due to hypoxemia or acute respiratory failure. The common symptoms of patients with COVID-19 pneumonia are fever, cough, and sputum production. Typical radiographic features of pneumonia are bilateral lower lobe lesions with ground-glass opacities or consolidations. N95 respirator in combination with personal protective equipment are necessary to prevent COVID-19 from spreading among healthcare workers during aerosol-generating procedures. Hemodynamic and ventilation management of critically ill patients with COVID-19 should follow …

Asian Medical Journal and Alternative Medicine

In the crisis of COVID-19 pandemic, there is no specific agent to treat SARS-CoV-2 infection. Repurposing medication can be an alternative agent. Remdesivir is the first antiviral drug that has been approved for COVID-19 treatment whereas lopinavir/ritonavir and hydroxychloroquine, which was early used in SARS-CoV-2 widespread, are against by well-known guidelines due to lack of efficacy and potential toxicity. Corticosteroids are an essential supportive care in COVID-19 patient with severe disease to reduce lung inflammation. Favipiravir may have a potential benefit for COVID-19 patient, but the data is still uncertain. Ivermectin, an antihelmintic agent, may be used against SARS-CoV-2 but …

Covid-19 Treatment, Suttichai Visuttichaikit

Asian Medical Journal and Alternative Medicine

The ongoing outbreak of COVID-19 has posed a great threat to public health. The various degree and dynamic of illness severity, together with the fact that only few drugs have been proven as an effective therapy has created critical challenges in disease management. Patients in acute viral phase usually present with upper respiratory tract and/or gastrointestinal illness, fever, and myalgia. Antiviral therapy might provide the greatest benefit in reducing disease progression in such patients with older age or significant comorbidities while immunomodulators are less likely to provide advantages. Patients in immune dysregulation phase usually present with pneumonia of different severity. …

Coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), is a pandemic in many countries necessitating the employment of massive public health responses. Clinical manifestations of children with COVID-19 are typically less severe than adult patients. Most children with COVID-19 remain asymptomatic or become mildly symptomatic. However, children can also get severely ill from COVID-19. Infants who are under the age of one year and children with preexisting comorbidities may be more likely to develop severe illness and are at an increased risk for hospitalization and critical care. Rarely, COVID-19 may result in multisystem inflammatory syndrome …

Covid-19 In Patients With Hematologic Malignancies: A Single Center Retrospective Study, Xuejun Wang, Adam Binder, Usama Gergis, Lindsay Wilde, Md

Department of Medical Oncology Faculty Papers

Initial studies that described the novel coronavirus (COVID-19) reported increased morbidity and mortality in patients with cancer. Of this group, patients with hematologic malignancies (HM) had the highest disease severity and death rates. Subsequent studies have attempted to better describe how COVID-19 affects patients with HM. However, these studies have yielded variable and often contradictory results. We present our single-institution experience with patients with HM who were diagnosed with COVID-19 from March 2020 to March 2021. We report 62 total cases with 10 patients who died during this time. The overall mortality was 16.1%. Mortality during the first two waves …

Saving The Nonessential With Radical Tax Policy, Rodney P. Mock, Kathryn Kisska-Schulze

University of Cincinnati Law Review

Under the Internal Revenue Code of 1986, as amended, for-profit entities are distinguishable from tax-exempt entities in that they, among other factors, pursue profits, and enjoy unrestricted commercial activities. The COVID-19 lockdowns prevented commercial activity for numerous for-profit small businesses. For the first time in United States history, a distinction was made between "essential" and "nonessential" businesses. Such distinction is historically absent in both legal scholarship and tax law; instead, it is a product of governmental reaction to the COVID-19 pandemic. Via executive order, nonessential businesses were characterized as being trivial to the fabric of society, and thus shuttered, while …

When The Conditions Are The Confinement: Eighth Amendment Habeas Claims During Covid-19, Michael L. Zuckerman

University of Cincinnati Law Review

The COVID-19 pandemic cast into harsher relief much that was already true about mass incarceration in the United States. It also cast into harsher relief much that was already true about the legal barriers confronting people seeking to make its conditions more humane. This Article offers a brief overview of the legal landscape as the COVID-19 crisis arose and then surveys eight prominent federal cases involving Eighth Amendment claims related to COVID-19 outbreaks at carceral facilities, most of which included significant litigation over whether they could secure release through habeas corpus. The Article then distills six key tensions from these …

Nebulization Of Low-Dose S-Nitrosoglutathione In Diabetic Stroke Enhances Benefits Of Reperfusion And Prevents Post-Thrombolysis Hemorrhage, Syed Kashif Zaidi, Farid Ahmed, Heba Alkhatabi, Nasrul Hoda, Muhammad Al-Qahtani

Neurology Articles

The COVID-19 pandemic has escalated the occurrence of hypoxia including thrombotic stroke worldwide, for which nitric oxide (NO) therapy seems very promising and translatable. Therefore, various modes/routes of NO-delivery are now being tested in different clinical trials for safer, faster, and more effective interventions against ischemic insults. Intravenous (IV) infusion of S-Nitrosoglutathione (GSNO), the major endogenous molecular pool of NO, has been reported to protect against mechanical cerebral ischemia-reperfusion (IR); however, it has been never tested in any kind of "clinically" relevant thromboembolic stroke models with or without comorbidities and in combination with the thrombolytic reperfusion therapy. Moreover, "IV-effects" of …

INTRODUCTION: The coronavirus disease 2019 (COVID-19) pandemic has been shown to increase levels of psychological distress among healthcare workers. Little is known, however, about specific positive and negative individual and organizational factors that affect the mental health of emergency physicians (EP) during COVID-19. Our objective was to assess these factors in a broad geographic sample of EPs in the United States.

METHODS: We conducted an electronic, prospective, cross-sectional national survey of EPs from October 6-December 29, 2020. Measures assessed negative mental health outcomes (depression, anxiety, post-traumatic stress, and insomnia), positive work-related outcomes, and strategies used to cope with COVID-19. After …

Georgetown Law Faculty Publications and Other Works

The 20th anniversary of the terrorist attacks on September 11, 2001, resulted in deep national reflection. Less remembered are the events that began to unfold 7 days later as anonymous letters laced with deadly anthrax (Bacillus anthracis) spores began arriving at postal facilities, media companies, and congressional offices. The first death from inhaled anthrax exposure occurred on October 5, with an additional 4 deaths and 17 infections over the ensuing months.

The anthrax attacks exposed a health system ill-equipped to respond to acute emergencies. This article explores the lessons learned, and unlearned, from the anthrax attacks, through to …
