Journal of Extension

Extension played a significant role, in collaboration with other key community organizations, by disseminating information that debunked COVID-19 myths, and promoting vaccinations at the community level. In this article we present insights from the experiences of Extension faculty at the University of Missouri who were engaged in promoting COVID-19 vaccinations for meat processing plant workers. Findings from this quality improvement project highlight opportunities for leveraging Extension field faculty, and challenges to consider in future public health promotion efforts.

During the COVID-19 pandemic early years, infection prevention measures suppressed transmission of seasonal influenza and other respiratory viruses. The early onset and moderate severity of the US 2022-2023 influenza season may have resulted from reduced use of nonpharmaceutical interventions or lower population immunity after 2 y of limited influenza virus circulation. We used a mathematical model of influenza virus transmission that incorporates vaccine-derived protection against both infection and severe disease to estimate the impact of influenza vaccines on healthcare burden. Clinicians frequently encounter patients in the hospital with mild-to-moderate COVID-19 who are at high risk for disease progression. Antiviral drugs reduce the rate of progression to severe COVID-19 when given to these patients within 5-7 days of symptom onset. To look at the utilization of antiviral medications in hospitalized patients with mild-to-moderate COVID-19, a detailed chart review was conducted of 500 randomly selected patients from the Charleston Area Medical Center COVID-19 Registry. We identified 124 patients with mild-to-moderate COVID-19 who were eligible for antiviral treatment. Only 41 of the 124 patients (33.1%) received antiviral treatment. INTRODUCTION: Influenza is a contagious respiratory infection with substantial health and economic consequences worldwide. Annual vaccination is the most effective preventive strategy; however, coverage among older adults remains suboptimal and varies considerably across the United States. This study aimed to examine state-level trends in influenza vaccination coverage among adults aged ≥65 years across fifteen influenza seasons (2009/10-2023/24).

METHODS: State-level vaccination coverage data for adults aged ≥65 years were obtained from the CDC FluVaxView platform, which reports annual estimates by influenza season. In recent times, the dominant psychological understanding of grief is as an emotion (Gravesen & Birkelund, 2021; Meiselman, 2021). However, grief is also socially constructed, meaning that the emotions and behaviors labeled as grief have their beginning in the social space. Grief, then, is a performative act shaped by cultural traditions and public rituals (Murrell et al., 2023). This dissertation investigated the trends in discipline rates for students in grades 6-12 with emotional disturbances. It compared data from the four years prior to the COVID-19 pandemic (2016-2019) to the four years following its onset (2021-2024). Emotional disturbances can significantly affect students' behavior, often leading to higher rates of disciplinary actions within school settings. However, the impact of COVID-19 on these students, particularly in terms of remote learning, school closures, and shifts in behavioral norms, has yet to be fully explored. Social Determinants Of Health Patterns In Children With Severe Disease Due To Sars-Cov-2 Infection-An Exploratory Approach, Joshua Prabhu, Sebastian Acosta, Fabio Savorgnan, Ananth V Annapragada, Usha Sethuraman

Background/Objectives: Research on the association of adverse social determinants of health (SDOH) with severe pediatric coronavirus disease (COVID-19) is limited. We examined associations between SDOH patterns and COVID-19 severity in children.

Methods: We conducted a prospective, observational study of children (< 18 years) with symptomatic SARS-CoV-2 infection evaluated in an urban pediatric emergency department (March 2021-April 2022) in Detroit, Michigan. Caregivers completed a 34-item survey based on the Healthy People 2030 framework. Severe disease was defined as the occurrence of respiratory/cardiac failure or death within four weeks of diagnosis. Continuous and categorical variables were described using medians and percentages, respectively. Associations between disease severity and risk factors were determined using chi-square tests. Association rule mining was used for feature selection, followed by multivariate logistic regression.

Results: We analyzed data from 354 children [6-12 years: 31.1%, Female: 51.1%, Black: 59%, not Hispanic: 84.7%, public insurance: 77.1%, chronic condition: 27.4%]. Of the total, 113 children had severe disease. Most caregivers were 30-44 years old (53.1%), had less than a college degree (70.4%), and income < USD 50,000 (75.2%). Adverse SDOH reported included food/housing insecurity (24.6%), no support (64.7%), unmet childcare needs (35.9%), and lack of transportation (12.7%). After controlling for age, sex, medical history, income, and obesity, severe disease was associated with caregiver use of drugs/alcohol (OR:5.92, p < 0.001) and social discrimination/lack of support (OR: 1.74, p = 0.030).

In recent years, scandals regarding the malpractices of many nonprofit organizations (NPOs) for selfish ends have eroded public trust in them. Therefore, it is necessary to consider whether the credibility of NPOs, as one of the three key implementers in cause-related marketing (CRM) campaigns, has a transfer effect on enterprise brand image. Background: Following SARS-CoV-2 infection, approximately 10%–35% of patients with COVID-19 experience long COVID (LC), in which debilitating symptoms persist for at least 3 months. Elucidating the biologic underpinnings of LC could identify therapeutic opportunities.

Methods: We utilized machine learning methods on biologic analytes provided over 12 months after hospital discharge from more than 500 patients with COVID-19 in the IMPACC cohort to identify a multiomics “recovery factor,” trained on patient-reported physical function survey scores. Immune profiling data included PBMC transcriptomics, serum O-link and plasma proteomics, plasma metabolomics, and blood mass cytometry by time of flight (CyTOF) protein levels. Recovery factor …

Immune checkpoint inhibitors (ICIs) extend survival in many patients with cancer but are ineffective in patients without pre-existing immunity1–9. Although personalized mRNA cancer vaccines sensitize tumours to ICIs by directing immune attacks against preselected antigens, personalized vaccines are limited by complex and time-intensive manufacturing processes10–14. Here we show that mRNA vaccines targeting SARS-CoV-2 also sensitize tumours to ICIs. In preclinical models, SARS-CoV-2 mRNA vaccines led to a substantial increase in type I interferon, enabling innate immune cells to prime CD8+ T cells that target tumour-associated antigens. Background: Complex socio-cultural, psychological, geographical, and service-related challenges are faced when recruiting people with dementia for clinical trials. The aim of Phase 1 of the Dementia PersonAlised Care Team (D-PACT) project was to assess the feasibility of recruiting (identifying, approaching and consenting) people with dementia, including those without capacity to consent, to a cluster randomized controlled trial of a primary care-based personalized dementia support intervention in England. OBJECTIVE: Explore the impact of the COVID-19 pandemic on pediatric acute mastoiditis (AM).

DATA SOURCES: CINAHL, Cochrane Library, PubMed, Scopus.

METHODS: Literature was searched from 2014 to 2025 for articles reporting AM in children. Specific outcomes included clinical presentation, bacterial epidemiology, complications, and management. Primary outcome measures included continuous measures (mean), proportions (%), mean difference (Δ), and relative risk (RR) with 95 % confidence intervals (CI).

RESULTS: There were seven included studies (N = 1001 children) with 632 patients in the pre-COVID cohort and 369 in the COVID-era cohort. There was a significant difference in the proportion of intracranial complications …

Humans instinctively adopt methods to reduce their risk of encountering harmful pathogens, yet their adherence to preventive health practices can often be erratic. Condoms exemplify one vital preventive tool against sexually transmitted infections (STIs) that, despite their effectiveness, are not consistently utilized. This pattern of behavior appears to persist even during periods of widespread disease transmission, with varied data from the COVID-19 pandemic indicating that condom usage remained inconsistent. The purpose of this case study was to better understand the decision-making process used by administrative leaders at a higher education institution during a time of organizational crisis. Post-traumatic growth (PTG) experienced by US nurses who provided care during the COVID-19 pandemic has not been examined. The findings from this multisite study of over 2,000 nurses provide information about the various factors of PTG that were experienced. Recommendations to promote PTG among nurses will be provided.

Abstract: Existing infection risk assessment methods mostly evaluate infection probability through mathematical models or simulation, but they lack analysis of the relationship between air circulation and individual infection probability. This study proposed a risk prediction model for infectious disease transmission based on indoor air circulation. At the microscopic scale, the space was discretized into grid points. By integrating CFD numerical simulation, the entire process of virus droplet release, transmission, and action was fully simulated. Background/aim: This study investigates the relationship between fear of COVID-19, measured using the Fear of COVID-19 Scale (FCV-19S), and the hygiene behaviors of secondary school students, assessed with the Hygiene Behavior Scale (HBS).

Materials and methods: Included in this cross-sectional study were 206 secondary school students. The study data were analyzed using multivariate linear regression.

Results: No relationship was found between the fear of COVID-19 scale (FCV-19S) score of the student sample and the mean total HBS and subdimension values (p > 0.05). It was noted that positive hygiene behaviors decreased as the scale score increased. Students with retired mothers scored …

In response to COVID-19, countries frequently adopted multiple types of policies to address the economic and financial effects of the pandemic. This paper analyzes the impact on bank lending of combinations or packages of policies (fiscal, monetary, and prudential) adopted across a broad sample of countries. Using a comprehensive policy announcement–level dataset together with bank-level information, we find that lending grew faster at banks in countries that announced large packages combining fiscal, monetary, and prudential measures (“all-out” packages), especially when uncertainty was high. The COVID-19 pandemic-driven lockdowns offer a unique opportunity to examine how reductions in anthropogenic emissions impacted atmospheric aerosol composition in urban environments. This study investigates the day-night variability of size-resolved water-soluble ions in ambient particulate matter (PM) collected in Metro Manila before (November 2019–February 2020) and after (November 2020–February 2021) lockdown implementation. Using tandem Micro-Orifice Uniform Deposit Impactors (MOUDIs), aerosol samples were collected during daytime (06:00–18:00) and nighttime (18:00–06:00) periods and analyzed for key ionic species (sulfate, ammonium, nitrate, oxalate, sodium, chloride, calcium, and magnesium) via ion chromatography. Purpose: This study aimed to evaluate retrospectively self-reported changes in tinnitus severity and distress associated with the COVID-19 pandemic and vaccination using validated self-report instruments. It further explored patient-reported onset of tinnitus following vaccination, gender differences in symptom severity, and associations with psychological distress. Method: A cross-sectional online survey was completed by 189 adults between December 2023 and April 2024. Participants retrospectively self-reported tinnitus onset and severity before, during, and after the COVID-19 pandemic using the Tinnitus Handicap Inventory (THI) and the Tinnitus Reaction Questionnaire (TRQ). SARS-CoV-2 variants have demonstrated distinct epidemiological patterns and clinical presentations throughout the COVID-19 pandemic. Understanding variant-specific differences at the respiratory epithelium is crucial for understanding their pathogenesis. Here, we utilized human nasal organoid air-liquid interface (HNO-ALI) cell cultures to compare the viral replication kinetics, innate immune response, and epithelial damage of six different strains of SARS-CoV-2 (B.1.2, WA, Alpha, Beta, Delta, and Omicron). All variants replicated efficiently in HNO-ALIs, but with distinct replication kinetic patterns. The Delta variant exhibited delayed replication kinetics, achieving a steady state at 6 days post-infection compared to 3 days for other variants. Cytokine analysis revealed …
