A Study Of Covid-19 Mortality Under Varying Patient Frailty, Alison N. Sifuentes
Theses and Dissertations
Modeled the spread and mortality of COVID-19 throughout the city of Chicago. By incorporating group frailty into a classic SEIR infectious disease model, we were able to differentiate the population of Chicago by their response to COVID-19. Three age groups with different COVID-19-induced death rates were examined, and the model sought to showcase the multiplicative deviation of each age group death rate from the average disease-induced death rate. This adjustment for different death rates among age groups accounted for heterogeneity within the population, and sought to introduce a more accurate manner for modeling the spread of infectious diseases.

This paper outlines governors’ powers to combat public health emergencies, and then analyzes specific measures taken by the states in response to the COVID-19 pandemic. State powers to quarantine, isolate, and take other measures to protect the public health and welfare are well-established, going back to the police power reserved for states in the Tenth Amendment and recognized by the U.S. Supreme Court over one hundred years ago. Additionally, most state constitutions and statutes specifically grant governors authority to take a variety of protective measures during emergencies. An online survey of a panel of 295 Anchorage residents 18 years old and older was conducted June 16-18, 2020. This was the fourth survey since May 2020 conducted by the Municipality of Anchorage (MOA) regarding COVID-19 related knowledge, attitudes, and behaviors. The first survey in the series was a population-based cell phone survey of Anchorage residents conducted May 6-10. The second (May 20-22) third (June 2-4) and fourth (June 16-18) surveys were conducted online with a panel of participants from the first survey. Neither the disease mechanism nor treatments for COVID-19 are currently known. Here, we present a novel molecular mechanism for COVID-19 that provides therapeutic intervention points that can be addressed with existing FDA-approved pharmaceuticals. The entry point for the virus is ACE2, which is a component of the counteracting hypotensive axis of RAS. Bradykinin is a potent part of the vasopressor system that induces hypotension and vasodilation and is degraded by ACE and enhanced by the angiotensin1-9 produced by ACE2. At the heart of the United States, disastrous response to the Covid-19 pandemic is a failure of diagnostic testing. That something so fundamental to medical care could be so botched evokes incredulity. From primary care offices to critical care settings, every patient encounter begins with a diagnostic workup. Diagnostic testing tools are key parts of the physician’s toolkit, and confirmatory testing is essential. But in the greatest public health challenge of the 21st century, the failures of diagnostic testing have been laid entirely bare.

We report a case of false-negative SARS-CoV-2 reverse transcriptase-polymerase chain reaction (RT-PCR) on a nasopharyngeal swab. Treating clinicians and infection preventionists should maintain a high suspicion for COVID-19 in the appropriate clinical setting despite negative test results. Utilization of chest computed tomography (CT) should be strongly considered in the diagnostic work-up for suspected COVID-19, particularly in areas with limited RT-PCR availability.

A 60-year-old male presented to the emergency department (ED) with a 1–2 day history of confusion, headache, and subjective fever. Because he had met with a contact two days prior to admission who had recently traveled from the Bahamas, a COVID-19 nasopharyngeal (NP) and oropharyngeal (OP) polymerase chain reaction (PCR) test was ordered. He was diagnosed with bacterial meningitis based on presenting neurologic symptoms and the identification of Streptococcus pneumoniae from blood cultures.
