Examining Therapists' Perceptions Of Barriers To Treatment With Youth And Their Families : A Mixed Methods Study, Gimel Rogers

The present study identified and quantified five main barriers to treatment categories, deducted first from the qualitative dataset, then consolidated with the results of the quantitative dataset. Clinicians (N=36) that worked with youth and their families participated. The main findings suggested five parent themes (practical obstacles, poor alliance with the therapist, therapist's perceptions, socioemotional, and cultural) and seven concept groups (transportation, financial, logistical, attendance, therapeutic relationship, lack of communication, and lack of engagement). Implications provide strategies to ratify some of these barriers, such as gathering data on youth clients and their families. For the purposes of this study, the terms …

Fear conditioning and extinction are central in the cognitive behavioral model of obsessive-compulsive disorder (OCD), which underlies exposure-based cognitive behavioral therapy (CBT). Youth with OCD may have impairments in conditioning and extinction that carries treatment implications. The present study examined these processes using a differential conditioning paradigm. Forty-one youth (19 OCD, 22 community controls) and their parents completed a battery of clinical interviews, rating scales, and a differential conditioning task. Skin conductance response (SCR) served as the primary dependent measure across all three phases of the conditioning procedure (habituation, acquisition, and extinction). Living in urban poverty has been linked to numerous negative conditions that disproportionately expose low-income urban youth and their families to severe and chronic stressors (Collins et al., 2010; DeNavas-Walt, Proctor, and Smith, 2012; Sznitman, Reisel, and Romer, 2011). Research has consistently shown a strong relationship between these stressors and numerous negative outcomes that can impact an adolescent emotionally, behaviorally, and academically (Conger et al., 2002). This study used the theory of traumagenic dynamics (TD) to examine how symptomatology resulting childhood sexual abuse (CSA) are related to the attitudes adolescent females have towards interpersonal relationships. Latino youth are members of an ethnic group that shares similar values, customs, beliefs, and, often, the Spanish language, that serve as protective factors for some youth. The extent to which these factors are protective across neighborhood contexts has yet to be explored. The present study adds to the literature on contextual correlates of mental health symptomatology in Latino adolescents by examining individual cultural dimensions as protective factors, and environmental risk and protective factors through the lens of the person-environment fit theory (Caplan, 1987). The focus for many of these individuals has been to develop a wide range of approaches to prevent youth problems and promote healthy youth development. Recently Attention-Deficit/Hyperactivity Disorder (ADHD) research has been confronted with questions regarding the subtype distinctions. Millich, Ballantine, and Lyman (2001) have claimed that ADHD – Combined Type (ADHD-C) and ADHD – Predominantly Inattentive Type (ADHD-I) are “distinct and separate disorders.” As important as this distinction is diagnostically, it is, possibly equally important with regards to treatment. Multiple pharmacological studies have compared the responsiveness of ADHD-C and ADHD-I to stimulant medications, yet the results are often conflicting (e.g. Grizenko, Paci, & Joober, 2010; Solanto et al., 2009; Stein et al., 2003). In the last decade of the 20th century, several large-scale studies suggested that the developmental trajectory for students diagnosed with emotional disturbance is bleak. Middletip School (MTS) is an alternative day treatment program that serves emotionally disturbed (ED) students (ages 12-19) through a daily offering of academic classes, and counseling and treatment groups. Using individually tailored, strength-based programming, MTS is designed to help ED youth in the areas of emotion regulation and behavior management, with a focus on building coping, relational, social, and communication skills. Children’s exposure to violence is a serious social problem, but little is known about the educational implications for adolescents witnessing violence between parents. This study uses social learning theory (SLT) to examine the relationship between high school-aged adolescent students who witness parental intimate partner violence (IPV) and academic performance demonstrated by their grade point averages (GPA). A secondary analysis of data collected from the survey of 1,132 adolescent students in a medium sized, suburban/rural city was conducted. Of the respondents, 83% of the students did not witness parental IPV between parents. Students witnessing the most parental IPV had the lowest …

Purpose
To examine longitudinal trends and associated factors in dental service utilization by adolescents progressing to early adulthood in the United States.
Methods
The data source was the National Longitudinal Study of Adolescent Health from Waves I (1994–1995), II (1996), III (2001–2002), and IV (2007–2008). This was a retrospective, observational study of adolescents' transition to early adulthood. We obtained descriptive statistics and performed logistic regression analyses to identify the effects of baseline and concurrent covariates on dental service utilization from adolescence to early adulthood over time.

The goal of this study was to identify the barriers and facilitators of successful mental health and/or behavioral programs implemented within inner-city schools. The impetus for this study came from prior meta-analytic research which demonstrated programs being offered within inner-city schools, as a whole, showed very low effect sizes, with many of the programs offered to youth within these settings showing iatrogenic effects. With this goal, city agencies in Boulder, Colorado, the Boulder Valley School District, the Children, Youth and Environments Center at the University of Colorado, and a number of community organizations have been working in partnership to integrate young people’s ideas and concerns into the redesign of parks and civic areas and the identification of issues for city planning. Adolescent youth in foster care are often exposed to multiple traumas and often leave the child welfare system without processing thoughts, feelings, or plans for the future. This study employed photo elicitation with a group of adolescent youth in foster care to discover what is important to them. Eight themes emerged. Family and friends, or the need to have someone who is always there for them was the most frequent theme photographed. Parent aggression exists on a continuum with corporal punishment at one end and abuse at the other. There is still controversy as to whether any kind of parent aggression toward an adolescent contributes to externalizing and internalizing behaviors. Relative to what is known about parent aggression during childhood, much less is known about parent aggression toward adolescents from a systemic and developmental perspective. Adolescent dating violence (ADV) is a significant community problem. In this study, we examine the perspectives of two groups (young adults who experienced ADV as teens and professionals who work with teens) on ADV prevention/intervention in a community context. We interviewed 88 young adults and 20 professionals. Our research team used Thorne’s (2008) interpretive description methods to determine participants’ perspectives on community views on ADV, community ADV prevention/intervention programs, and ideal ADV prevention/intervention strategies. Participants perceived most communities as being blind to ADV. They perceived prevention programs as unavailable, inappropriate, or impersonal. Mental health issues among American adolescents and children can negatively impact their potential for school success. As many as 10% of students among the general education population suffer from psychiatric disorders, yet only between 1% and 5% of those students are being served. The effects of mental health difficulties are problematic for students because they negatively impact academic performance, behavior, attendance, and school violence (Whelley, Cash, & Bryson, 2003). Objective—This randomized trial of a family-focused preventive intervention for Mexican American (MA) adolescents evaluated intervention effects on adolescent substance use, internalizing and externalizing symptoms, and school discipline and grade records in 8th grade, one year after completion of the intervention. The study also examined hypothesized mediators and moderators of intervention effects. The current study examined the relationship of parental support, parental psychological control, and best friend support among Latino adolescents and loneliness using self-esteem as a mediating variable. The sample included 839 Hispanic (primarily Mexican) 9th – 12th graders from west Texas area school districts who were given a self-reported survey to assess parental behaviors and adolescent depression. Using structural equation modeling (SEM), findings indicated that neither maternal nor paternal support was found to be significantly related to loneliness in girls. Only maternal support was found to be significantly related to loneliness in boys. On the other hand, neither maternal nor …

The present study examines the impact of child and family risk factors on service access for youth and families in a school-based system of care. Regression analyses examined the relationships between risk factors and services recommended, services received, and dosage of services received. Logistic regression analyses examined the relationship between risk factors and whether or not youth received specific types of services within the system of care. Results revealed that youth with a personal or family history of substance use had more services recommended than youth without these risk factors, while youth with a family history of substance use received …

This qualitative study examines clinician perceived barriers to successful discharge planning from inpatient and residential treatment settings, as well as factors that may contribute to recidivism. Eight clinicians participated in semi-structured interviews in an attempt to elicit their views and best practices when discharging clients from treatment. Participants worked in a multitude of settings at different levels of care and were asked to evaluate both their individual approaches to discharge planning as well as practices within their agencies. Findings suggest multiple factors contributing to successful discharge plans, though clinicians reported having control over only a fraction of these factors. For …

This quantitative, quasi-experimental study examined 57 adolescents, ages 13 to 18, who attended the Catherine Freer Wilderness Therapy Program 21-day trek. The program is based in Albany, Oregon. The purpose of this study was to conduct a follow-up to Ganapol's (2008) study in order to further assess the treatment modality, wilderness therapy, through the lens of trauma. More specifically, this study focused on wilderness therapy's potential to act as a transformative experience for adolescents with histories of trauma. The assumption of the current study is that ongoing church attendance is a foundational component of the quest for genuine discipleship in young people. Chapter 1 examines the problem of the exodus of youth from the church and defines the critical terms for this study. The research questions used to guide the study are introduced.

Gambling has become increasingly popular among minors and is easily accessible to them. This is alarming since research has indicated that minors are more susceptible to gambling pathology than adults. Additionally, gambling has devastating effects on minors that gamble as well as their families and communities. The Illinois Institute for Addiction Recovery (IIAR) developed a gambling awareness prevention program called “Don’t Gamble Away our Future™” to educate minors about gambling and the dangers associated with it. Due to continued fragmentation and gaps in mental health services and the increase in the prevalence of mental health problems for children, youth, and their families, these populations remain underserved. In 2003, the federal New Freedom Commission (Commission) responded by publishing policies to address these concerns. As directed in 2005, the Substance Abuse and Mental Health Services Administration (SAMHSA) funded planning incentive grants to states to transform their delivery of care.

Factors Affecting Long-Term Outcomes Following Intensive In-Home Services, Sarah Hurley

Intensive in-home services (IIHS) work with families toward building skills and resources to better manage their children with behavioral difficulties. Factors that impact long-term outcomes following IIHS are not well understood. This study examined the relative importance of youth demographic and clinical characteristics, family history variables, characteristics of program participation, and organizational-level factors in explaining placement stability, educational progress, and contact with legal authorities one year following discharge from IIHS.
The sample included all youth who received IIHS, were discharged between January 1, 2001 and December 31, 2006, and completed a follow up at one-year post-discharge (n = 2,649) from …
