背景

髓鞘少突膠質細胞糖蛋白抗體相關疾病(MOGAD)是一種罕見的自體免疫發炎性疾病,主要涉及中樞神經系統(CNS),其主要特徵是視神經、腦部和脊髓脫髓鞘。我們報告一例最初疑似復發性中風的病例,該病例在住院中風復健期間最終確診為MOGAD。

病例概要

49歲女性,左側大腦半球梗塞,在中風後復健期間出現多次右側肢體肌力下降伴隨失語症。這些神經功能波動偏離了典型卒中的預期復健軌跡。卒中發作兩個月後行腦部磁振造影(MRI)檢查,結果顯示右側內囊後肢、腦橋及C1脊髓出現多處新發病灶。後續血清學檢查顯示抗髓鞘少突膠質細胞糖蛋白(MOG)IgG抗體陽性,而水通道蛋白4(AQP4)抗體陰性,確診為MOGAD。患者接受了甲潑尼龍治療及充分的復健治療,追蹤期間肌力顯著改善,未見脫髓鞘病灶復發。

結論

本案例凸顯了復健科醫師在早期發現類似中風的神經發炎性疾病的關鍵作用。復健科醫師處於獨特的領先地位,能夠透過持續的功能監測識別「危險信號」;任何偏離預期復健路徑或無法解釋的神經功能波動都應促使醫生進行進一步檢查。早期識別此類非典型模式,並及時進行免疫治療和多學科康復,可以預防永久性功能衰退,並優化長期預後。

Background: Competency-Based Medical Education (CBME) emphasizes formative, learner-centred assessment. Blood pressure (BP) measurement, a core clinical skill, integrates knowledge, skills, communication, ethics, and attitude. While rubrics are widely used to standardize evaluation, their utility in triangulating faculty, peer, and self-assessment in BP measurement remains underexplored.

Aim: To evaluate the reliability and educational value of rubric-based faculty, peer, and self-assessment in BP determination among Phase 1 MBBS students using a mixed-methods approach.

Language barriers significantly hinder academic performance, psychological well-being, and clinical communication skills of Arabic-speaking healthcare students educated through English-medium instruction. The development of Artificial Intelligence (AI) in healthcare is largely dependent on the quality of medical datasets. However, these datasets often fail to accurately represent women (and those seen as women) due to historic, implicit and biological biases. This under-representation can lead to biased, inequitable and even harmful models. Building upon the findings of previously conducted qualitative semi-structured semantic interviews with clinicians on their perceptions of women’s health, this paper presents a framework for translating qualitative findings to dataset characteristics via operationalisation. The concept of neighborhood has been long studied with little agreement beyond the notion that neighborhoods are somewhat self-contained areas within a wider urban environment. Despite the lack of a finite definition, there is wide agreement of the importance of studying population dynamics at the neighborhood scale. This article conceptualizes classical guitar left-hand technique through a physiomechanical framework that places coordinated movement—particularly shifting—at the center of technical practice. Challenging traditional positional doctrines that privilege static hand forms and digital independence, the author argues that such approaches often disregard the functional anatomy of the limb, resulting in inefficiency, unnecessary tension, and potential injury.   Advancing the principle that form must follow function, the study proposes shifting as the fundamental technical category governing all left-hand procedures. BACKGROUND: Stroke leads to complex chronic structural and functional brain changes that specifically affect motor outcomes. The brain predicted age difference (PAD) has emerged as a sensitive biomarker of both sensorimotor and cognitive function after stroke. Our previous study showed a higher global brain PAD associated with poorer motor function after stroke. However, the association between local stroke lesion load, regional brain age, and motor impairment is unclear. This study aimed to investigate the associations between focal lesion damage, regional brain PAD in both hemispheres, and motor outcomes in chronic stroke, and to identify key predictors of motor impairment.

The performance of Intelligent Transportation Systems (ITS) critically depends on accurate and efficient road-condition monitoring. This paper presents IMER (Inspect–Map–Eliminate–Reduce), a novel AI-driven data-processing framework that extends the traditional Map-Reduce paradigm for infrastructure maintenance. IMER integrates confidence-based validation, redundancy elimination, and severity prioritization to enhance data quality and decision efficiency. Implemented within a multi-agent architecture, IMER enables autonomous agents to inspect, classify, and fuse multi-source road data in real time, supporting predictive and adaptive maintenance planning. In today's competitive market, it is increasingly important to understand how visual design shapes customer behaviour. This study examines the decision drivers influencing female customers' purchase choices when buying male-oriented products as gifts, identifying the visual components of advertisements that attract them by analysing the relationship between purchase intention and visual attention. Results show that participants with higher purchase intent focused more on product imagery and branding, indicating that visual appeal, perceived quality, and brand familiarity significantly guide their decisions, with brand awareness speeding up decision-making by reducing the need for repeated visual checks. Hydrogen fuel is gaining attention as a promising zero-emission energy source, aligning with global sustainability goals and supporting the transition to zero carbon emissions. This study examines the potential of using hydrogen as an alternative fuel for sustainable mobility in Muscat, Oman. We developed an integrated modelling framework that combines microscopic traffic simulation, energy demand modeling, refueling infrastructure station’ estimation, and well-to-wheel (WTW) emissions evaluation. The study examined the application of Personalized Digital Twins (PDTs) to prevent complications during the management of Type 2 Diabetes, especially in early intervention and prevention plans. Based on a high-quality dataset related to the CDC Behavioral Risk Factor Surveillance System (BRFSS) data, we tested multiple predictive models such as the Random Forest, Gradient Boosting machines (GBM), and Extreme Gradient Boosting (XGBoost). We developed a composite risk indicator from established clinical risk factors (hypertension, dyslipidemia, elevated BMI) to stratify complication risk. The Random Forest model achieved 99% accuracy (AUC: 0.98) at the population-level risk classification. The GBM model was optimized …
