Navigating Complex Esophageal Perforations: A Case-Specific Approach To Successful Outcomes, Harsimran Kalsi, Anushree Rai, Kobina Essilfie-Quaye, Neeka Akhavan, Yaseen Perbtani, Tony S. Brar
HCA Healthcare Journal of Medicine
Esophageal perforation is a life-threatening condition requiring prompt diagnosis and effective management. While self-expanding metal stents (SEMS) are a first-line treatment, persistent leaks often necessitate adjunctive endoscopic interventions. A structured, multimodal approach is critical for optimizing outcomes and avoiding surgical intervention. A 44-year-old man developed an iatrogenic esophageal perforation following surgical leiomyoma enucleation. Initial treatment with overlapping fully covered self-expanding metal stents (FC-SEMS) failed to achieve closure, necessitating endoscopic suturing for defect reinforcement. Despite suturing, persistent leakage required placement of a larger-diameter FC-SEMS, secured with an endoscopic fixation system to prevent migration. Serial imaging confirmed successful defect closure, allowing the …

Physicians and educators tend to be more versed in recognizing disease patterns and medical problems than in identifying malfunctioning patterns in the psychological aspects of their mentees and work environments. Just as hypertension and diabetes are the common soil for many acute medical problems, agency problems and organizational silos can lead to many acute managerial problems in graduate medical education (GME) settings.
Data show that most workers are lonely at work, which leads to negative emotions and undermines performance. Organizational problems seem to stem from an individual’s inability to align one’s motivation with the organization’s shared value. This can cause …

The intricate relationship among osteoporosis, vitamin D deficiency, and hormonal changes. Growth hormone, cortisol, parathyroid hormone, estrogen, testosterone, thyroid hormones, and thyroid hormones are a few of the hormones that affect bone turnover and density. Alterations in their concentrations, whether brought on by healthy aging or illnesses, can interfere with bone remodelling and increase the risk of osteoporosis. Moreover, inadequate levels of vitamin D increase the risk by decreasing the absorption of calcium and the mineralization of bone. Estrogen and vitamin D deficits increase bone loss, especially in postmenopausal women. Proactive treatment techniques, including hormone replacement therapy, vitamin D supplements, …

Photoaging caused by chronic sun exposure exhibits characteristics similar to intrinsic aging, including skin thinning, moisture loss, roughness, collagen degradation, and wrinkle formation. This process is driven by oxidative stress and the formation of malondialdehyde (MDA) as a result of lipid peroxidation. Topical intervention using antioxidant-based moisturizers could prevent further damage. Robusta coffee (Coffea canephora), high levels of chlorogenic acid and caffeine, is known for its antioxidant properties. When fermented with a Symbiotic Culture of Bacteria and Yeast (SCOBY) to produce kombucha coffee, its bioactive compounds are enhanced, potentially offering additional protection against oxidative stress and photoaging.
