Cisgender women face an increased risk of HIV acquisition during pregnancy and postpartum. Oral pre-exposure prophylaxis (PrEP) with emtricitabine and tenofovir disoproxil fumarate (F/TDF) is recommended in pregnancy despite limited published pharmacokinetic (PK) and efficacy data during pregnancy. Altered PK during pregnancy may reduce prophylactic efficacy and necessitate higher adherence. Tenofovir alafenamide (TAF) is a newer tenofovir prodrug with higher tenofovir diphosphate (TFV-DP) exposure in peripheral blood mononuclear cells (PBMCs), but the prophylactic efficacy of F/TAF across different reproductive stages in women is understudied. INTRODUCTION: Chronic myeloid leukemia (CML) is a hematopoietic malignancy driven by constitutive tyrosine kinase activity. Dasatinib, a second-generation (2G) tyrosine kinase inhibitor (TKI), is a preferred treatment due to its superior, durable, and rapid response rates. While dasatinib has known gastrointestinal (GI) side effects, colonic polyposis is a rare complication, not well described in the literature.
CASE PRESENTATION: Upon routine colonoscopy, a 66-year-old woman with chronic-phase CML was incidentally discovered to have asymptomatic colonic polyposis after 11 months of dasatinib therapy. Histopathology revealed reactive inflammatory changes without evidence of dysplasia or neoplasia. The polyposis resolved completely after cessation of dasatinib. …

INTRODUCTION: Human immunodeficiency virus (HIV) therapies now support survival into advanced ages by suppressing HIV replication and preserving immune function. Alzheimer's disease (AD), the most common neurologic degenerative condition, may now be treated with anti-amyloid therapies (AAT). HIV was an exclusionary criterion in clinical trials for AAT, leaving the safety and efficacy of these medications unknown in this setting.
METHODS: We report the development of AD supported by careful evaluations and biomarkers over a 10-year period in a person living with well-controlled HIV infection.
RESULTS: AD was diagnosed with consistent clinical and biomarker evidence. AAT was successfully administered without complications. …

BACKGROUND: Interpretation of neurofilament light chain (NfL) and glial fibrillary acidic protein (GFAP) levels in blood requires reference ranges from healthy cohorts, accounting for demographic features.
OBJECTIVE: To compare serum and plasma NfL and GFAP in healthy individuals, accounting for age, sex, and body mass index (BMI).
METHODS: Adolescents through older adults were recruited at Colorado community events and a senior clinic. A neurologist reviewed self-reported medical histories. Paired serum and EDTA plasma samples were assayed using the Quanterix SR-X Simoa Neuro 4-Plex. Log-linear models estimated percent change per year of age, per BMI unit, and sex ratios. GFAP used …

OBJECTIVE: Cerebral palsy (CP) is the most common lifelong motor disability worldwide. Yet, data is limited on how CP manifests in the US. Our objective was to characterize and determine factors affecting functional outcomes in a large population of young people with CP in the Midwestern US.
METHODS: We integrated caregiver and clinician-facing standardized data entry into routine clinical care at a tertiary care CP center (https://bit.ly/CP-Intake-Methodology). Thirty-eight states have enacted “anti-BDS” laws. These laws require people or entities doing business with the state to certify that they do not participate in the Boycott Divestment Sanction movement against Israel. Typically, these laws define boycott as a “refusal to deal, terminating business, or other actions” that limit commercial relations with Israel or Israeli entities. It is highly contested in the courts and commentary whether these anti-boycott laws violate the First Amendment. Assuming arguendo that the “refusal to deal” and “terminating business” clauses do not implicate the First Amendment, that leaves the residual clause. At least one court and …

Rhabdomyolysis is a condition with a myriad of causes, including both traumatic and non-traumatic. Traumatic conditions are observed in crush injuries, compartment syndrome, and fractures, while non-traumatic rhabdomyolysis is commonly seen due to exertional heat stroke, extreme physical exercise, and dehydration. This case report explores two cases of extreme non-traumatic rhabdomyolysis caused by indoor cycling, also known as spinning. A common complication of severe rhabdomyolysis is acute kidney injury (AKI), which increases the risk of renal failure. Background: With >10 million new tuberculosis (TB) cases/year, a limitation to TB control is the lack of simple and accurate tests for TB diagnosis and drug-susceptibility testing (DST) in endemic regions. We evaluated the accuracy of the first-generation, low-complexity phenotypic TB test (1G test), designed for simultaneous Mtb detection and resistance to isoniazid, rifampicin and moxifloxacin, suitable for resource-limited settings.

Oral cancer, particularly oral squamous cell carcinoma, remains a serious health concern, with a poor prognosis and a late diagnosis. Leukoplakia, erythroplakia, lichen planus, and submucous fibrosis are examples of oral potentially malignant disorders that must be detected early but are not always so by traditional, laborious, and subjective diagnostic techniques. Objective: Screening is an effective tool for detecting colorectal lesions in asymptomatic subjects. There is a positive correlation between fecal immunochemical test (FIT) values and the size of tumors. Despite the efficacy of screening, the detection of colorectal cancer (CRC) remains low. The primary objective was to evaluate the best FIT cutoff value for detecting advanced adenomas and CRC among individuals at average risk in a country with a high incidence and morbidity from CRC.

Racial discrimination is recognized as a race-based traumatic stressor that has been associated with posttraumatic stress symptoms (PTSS) and suicide ideation (SI). However, there remains a paucity of research investigating the impact of racial and other forms of discrimination on understudied populations, including Black Americans residing in Appalachian regions. The current study used an online self-report survey to examine PTSS as a mediator in the association between perceived discrimination and SI and to examine the moderating effect of the number of identity factors with which one experienced discrimination (i.e., race, gender, region) on the relationship between perceived discrimination and PTSS. …

SIGNIFICANCE. The rewarding properties of nicotine increase when administration is paired with external stimuli, or “cues”, including those related to vaping products. Given the visual similarity between nicotine vaping products (NVPs) and THC vaping products (TVPs), exposure to one product may elicit reactivity (e.g., craving, urge to use) to the other. The present study aimed to develop a set of cues capable of differentiating NVPs from TVPs to inform investigations of cross-cue reactivity. METHODS. This qualitative single-case study examined how a private-interest, nonprofit youth soccer club in the Midwestern United States recruits coaches, what coach qualities are prioritized in hiring, and how organizational and sociocultural conditions shape those practices. The study was guided by three research questions focused on (a) recruitment and early support processes, (b) hiring criteria and decision rationales, and (c) the organizational and sociocultural contexts influencing recruitment. This dissertation looks at the ongoing issue of tiny airborne particles created during medical and dental procedures and evaluates ways to reduce the risk of exposure for healthcare workers and patients to these particles. The first study focuses on the size, how long particles stay in the air, and how far they travel from common medical procedures that create aerosols, showing that tiny particles (less than 5 µm) can stay in the air long enough to escape basic droplet controls. Introduction: Operating room debriefing is an evidence-based strategy shown to improve communication, efficiency, patient safety, and safety culture; however, debriefing is inconsistently performed in obstetric operating rooms due to workflow, cultural, and time-related barriers.
Purpose: The purpose of this Doctor of Nursing Practice (DNP) project was to increase the frequency of debriefings in the Birthing Center operating rooms at WVU Medicine Children’s Hospital through implementation of a nurse-driven, evidence-based debriefing initiative.

A three-month pre-intervention data collection showed 1.8% of patients had initial CPs documented. Inadequate documentation and monitoring of ETT CPs can lead to patient complications such as tracheal injury and aspiration making this an important area for quality improvement.

Background: Research demonstrates that music therapy is a low-cost, low-risk intervention that reduces perioperative anxiety and sedative requirements. Literature supports the use of music as an adjunct to anesthesia care, especially in orthopedic populations.

While emotion recognition deficits have been found in several anxiety and related disorders (e.g., social anxiety, obsessive-compulsive), consistent results have not been established. To clarify the relationship between emotion recognition abilities and anxiety, the current study tested if transdiagnostic factors related to anxiety are predictors of emotion recognition abilities. Data was analyzed from participants (N = 188) recruited via CloudResearch; they completed measures of three transdiagnostic factors related to anxiety (e.g., anxiety sensitivity, intolerance of uncertainty, and rumination), followed by an emotion recognition task. The toddler age (12-36 months) is characterized by a toddler’s increasing desire for autonomy, which can result in caregiver-child conflict and toddler externalizing problems (Kohlhoff & Morgan, 2014). Clinical levels of disruptive behavior in toddlerhood can be predictive of a host of negative outcomes, including problems with academic achievement and delinquency (Copeland et al., 2007; Milledge et al., 2019). Introduction: The use of multiple sedatives, anxiolytics, and analgesics to prevent emergence agitation (EA)in pediatric patients undergoing myringotomy (MT) placement at West Virginia University Medicine Children's is leading to prolonged lengths of stay in the post-anesthesia care unit (PACU). These extended stays are causing delays in subsequent procedures and raising concerns for staff, patients, and their families.

The target of glyphosate is absent in animals, but findings reporting toxicity in model organisms without a known mechanism or target are increasing. Introduction: Awake paralysis (AWP) occurs when long-acting neuromuscular blockers outlast short-acting sedatives during intubation, leaving patients chemically paralyzed but conscious. This highlights the critical need for timely post-intubation sedation.
Background: A 2024 review in the Neurological Critical Care Unit (NCCU) revealed that 72% of patients who received rocuronium were not given a post-intubation sedative. The difference in duration of action of paralyzing agents versus sedatives can result in patients being conscious while paralyzed, potentially resulting in hemodynamic instability and psychological harm.

Collegiate women athletes report high rates of body dissatisfaction and wanting more support for improving their body image within their athletic departments. Guided by the Developmental Theory of Embodiment (DTE; Piran, 2017), the purpose of this study was to examine collegiate women athletes’ perceptions of the relationship between their embodiment and aspects of their sport experience. Participants were 311 women currently competing in 24 different sports within NCAA and NAIA athletics. Participants completed the Experience of Embodiment Scale, Subjective Performance Questionnaire, Sport Commitment Questionnaire-2, and open-ended response questions about the perceived relationship between these variables.
