This case report describes a 55-year-old woman presenting with acute dysphagia and neck swelling after a "popping" sensation in her neck. CT and MRI revealed a large retropharyngeal cystic mass and a left thyroid lobe mass, causing airway displacement. Despite significant effacement of the oropharyngeal and supraglottic airway on flexible nasopharyngolaryngoscopy, the patient remained asymptomatic regarding respiratory distress. Subsequent ultrasound-guided fine needle aspiration biopsy diagnosed malignant papillary thyroid carcinoma. Concurrently, previously undiagnosed Graves' disease was identified. The clinical presentation was attributed to suspected rupture or hemorrhage of the malignant thyroid tumor, leading to a serosanguineous fluid collection and inflammatory edema. …

Equity regarding access to potable water is paramount. Municipal Water Distribution Systems supply potable water to users and are essential to public health. To find the optimal price of water to maximize utility revenue and allow changes to the system components for capital and maintenance expenditures, network modeling was performed for 15 real MDWSs. A new power-law price-usage elasticity curve, entitled the Power Law Elasticity Function (PLEF), was used that allowed for various users, both high- and low-income, and a non-discretionary level of usage. Chemotherapy is an effective weapon in the fight against pediatric cancer. However efficient, treatment can lead to side effects that last long after a patient finishes their last round of chemotherapy. This thesis examines current research on chemotherapy-induced cardiotoxicity, a side effect of chemotherapy treatment caused by a class of chemotherapy drugs called Anthracyclines (such as doxorubicin and daunorubicin). While effective chemotherapy agents, these drugs have been known to cause late long-term heart damage when used to treat pediatric cancers such as Acute Lymphoblastic Leukemia (ALL), Acute Myeloid Leukemia (AML) and Pediatric Sarcoma. บทความวิชาการนี้เป็นการสังเคราะห์องค์ความรู้เกี่ยวกับสมรรถนะของพยาบาลเฉพาะทางการบำบัดทดแทนไตและแนวทางการพัฒนาคุณภาพการดูแลผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายโดยทบทวนวรรณกรรมและเอกสารทางวิชาการที่เกี่ยวข้องทั้งในและต่างประเทศรวมถึงแนวปฏิบัติทางคลินิกและมาตรฐานวิชาชีพพยาบาลเพื่อนำเสนอกรอบสมรรถนะหลักที่จำเป็นสำหรับพยาบาลเฉพาะทางในการให้การดูแลผู้ป่วยที่ได้รับการบำบัดทดแทนไตทั้ง3รูปแบบได้แก่การฟอกเลือดด้วยเครื่องไตเทียม การล้างไตทางช่องท้อง และการปลูกถ่ายไต  ผลลัพธ์ที่ได้จากการศึกษาพบว่าสมรรถนะหลักของพยาบาลเฉพาะทางการบำบัดทดแทนไตประกอบด้วย 9 หัวข้อสำคัญ ได้แก่ 1)สมรรถนะด้านการปฏิบัติการพยาบาลทางคลินิก 2)สมรรถนะด้านเทคโนโลยีและทักษะการใช้เครื่องมือฟอกไต 3)สมรรถนะด้านการสอนและการให้คำปรึกษา 4)สมรรถนะด้านจริยธรรมและความเป็นมืออาชีพ 5)สมรรถนะด้านการใช้เทคโนโลยีสาร 6)สมรรถนะด้านการสื่อสารและการทำงานเป็นทีม 7)สมรรถนะด้านการป้องกันและควบคุมการติดเชื้อ 8)สมรรถนะด้านคุณภาพและงานวิจัย 9) สมรรถนะด้านผู้นำและบริหารจัดการการพัฒนาคุณภาพการดูแลสามารถดำเนินการผ่าน4แนวทางหลักคือการพัฒนาศักยภาพบุคลากรอย่างต่อเนื่องผ่านการฝึกอบรมและการพัฒนาทางคลินิกการจัดทำและใช้แนวปฏิบัติทางคลินิกที่เป็นมาตรฐานการพัฒนาระบบติดตามประเมินผลและตัวชี้วัดคุณภาพและการส่งเสริมการทำงานเป็นทีมสหสาขาวิชาชีพ ซึ่งส่งผลให้ผู้ป่วยได้รับการดูแลที่มีคุณภาพ ลดภาวะแทรกซ้อน และมีคุณภาพชีวิตที่ดีขึ้น

ในยุคแห่งการเปลี่ยนผ่านสู่เทคโนโลยีดิจิทัล ปัญญาประดิษฐ์ (Artificial Intelligence: AI) มิได้เป็นเพียงเครื่องมือที่ช่วยวิเคราะห์ข้อมูลเท่านั้น แต่กำลังพลิกโฉมบทบาทของวิชาชีพพยาบาลอย่างรอบด้าน บทความนี้มีวัตถุประสงค์เพื่อวิเคราะห์ผลกระทบของปัญญาประดิษฐ์ต่อบทบาทของพยาบาลระดับปฏิบัติการขึ้นไปผ่านกรอบแนวคิด 3 มิติ ได้แก่ มิติด้านการปฏิบัติงาน มิติด้านความสัมพันธ์และจริยธรรม และมิติด้าน อัตลักษณ์ทางวิชาชีพ  ผลการวิเคราะห์พบว่า มิติด้านการปฏิบัติงาน ปัญญาประดิษฐ์ปรับบทบาทพยาบาลจากผู้ปฏิบัติตามกระบวนการสู่ผู้วิเคราะห์และกำกับการใช้ข้อมูลเพื่อสนับสนุนการตัดสินใจทางคลินิกมิติด้านความสัมพันธ์และจริยธรรมแม้ปัญญาประดิษฐ์จะเพิ่มความแม่นยำในการประเมินและเฝ้าระวังภาวะสุขภาพแต่ยังมีข้อจำกัดในด้านความฉลาดทางอารมณ์ ซึ่งไม่สามารถทดแทนความเข้าอกเข้าใจ การสัมผัสและเอาใจใส่ต่อผู้ป่วยได้ สำหรับมิติด้านอัตลักษณ์ทางวิชาชีพ การประยุกต์ใช้ปัญญาประดิษฐ์นำไปสู่การปรับขอบเขตความเชี่ยวชาญของพยาบาลโดยจำเป็นต้องพัฒนาสมรรถนะด้านดิจิทัลและการคิดเชิงวิพากษ์ควบคู่กับการดูแลที่ยึดผู้ป่วยเป็นศูนย์กลาง บทความนี้เสนอแนวคิดว่า พยาบาลควรเป็นผู้ใช้ปัญญาประดิษฐ์อย่างมีวิจารณญาณ โดยยึดหลักแนวคิดการบูรณาการระหว่างศักยภาพมนุษย์และปัญญาประดิษฐ์เพื่อยกระดับบทบาทของพยาบาลให้ สามารถคิด วิเคราะห์ และสร้างนวัตกรรมการดูแลที่ผสมผสานระหว่างเทคโนโลยีและหัวใจของความเป็นมนุษย์ได้อย่างสมดุล

บทบาทพยาบาลในการป้องกันภาวะแทรกซ้อนและฟื้นฟูสมรรถภาพหัวใจในผู้ป่วยกล้ามเนื้อหัวใจตายเฉียบพลันกลุ่มเสี่ยงต่ำหลังได้รับการรักษาด้วยการถ่างขยายหลอดเลือดหัวใจ, ดารา วงษ์กวน

บทความวิชาการนี้มีวัตถุประสงค์เพื่อสังเคราะห์บทบาทของพยาบาลในการป้องกันภาวะแทรกซ้อนและการฟื้นฟูสมรรถภาพหัวใจในผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลันกลุ่มเสี่ยงต่ำภายหลังได้รับการรักษาด้วยการถ่างขยายหลอดเลือดหัวใจ โรคกล้ามเนื้อหัวใจตายเฉียบพลันเป็นภาวะที่รุนแรงที่สุดในกลุ่มอาการกล้ามเนื้อหัวใจขาดเลือดเฉียบพลัน เกิดจากการปริแตกของคราบไขมันบริเวณผนังหลอดเลือด ส่งผลให้เกิดการก่อตัวของลิ่มเลือดและการอุดตันของหลอดเลือดหัวใจ การรักษาด้วยการถ่างขยายหลอดเลือดหัวใจเป็นวิธีการรักษาที่มีประสิทธิภาพสูง ช่วยให้สามารถเปิดหลอดเลือดหัวใจได้อย่างรวดเร็ว โดยมีระยะเวลาเฉลี่ยตั้งแต่ผู้ป่วยมาถึงโรงพยาบาลจนสามารถเปิดหลอดเลือดหัวใจได้ประมาณ 54 นาที ส่งผลให้ความรุนแรงของโรคลดลง ผู้ป่วยฟื้นตัวได้เร็ว และมีระยะเวลาการนอนโรงพยาบาลสั้นลง ผู้ป่วยบางรายจึงสามารถจัดอยู่ในกลุ่มเสี่ยงต่ำและสามารถจำหน่ายออกจากโรงพยาบาลได้ภายใน 48–72 ชั่วโมง พยาบาลจึงมีบทบาทสำคัญในการดูแลผู้ป่วยตั้งแต่ระยะวิกฤตจนถึงระยะเตรียมความพร้อมก่อนจำหน่าย โดยการพยาบาลสามารถแบ่งออกเป็น 3 ระยะ ได้แก่ การป้องกันภาวะแทรกซ้อนใน 24 ชั่วโมงแรก การฟื้นฟูสมรรถภาพหัวใจและการปรับเปลี่ยนพฤติกรรมเสี่ยงในช่วง 24–48 ชั่วโมง และการส่งเสริมการฟื้นฟูสมรรถภาพหัวใจอย่างต่อเนื่องในช่วง 48–72 ชั่วโมง การดำเนินบทบาทของพยาบาลในแต่ละระยะมีส่วนสำคัญในการลดภาวะแทรกซ้อนส่งเสริมการฟื้นฟูสมรรถภาพหัวใจ และเตรียมความพร้อมก่อนจำหน่ายจากโรงพยาบาล ซึ่งจะช่วยลดการกลับเป็นซ้ำของโรคและส่งเสริมคุณภาพการดูแลผู้ป่วยอย่างต่อเนื่อง

วัตถุประสงค์:เพื่อศึกษาผลของโปรแกรมการให้ข้อมูลแบบรูปธรรม-ปรนัยต่อพฤติกรรมการป้องกันการพลัดตกหกล้มของผู้ป่วยวัยผู้ใหญ่ที่รับการรักษาในโรงพยาบาลศิครินทร์ หาดใหญ่


แบบแผนงานวิจัย:การวิจัยกึ่งทดลอง ชนิดสองกลุ่มวัดก่อนและหลังการทดลอง


วิธีดำเนินการวิจัย:กลุ่มตัวอย่าง คือ ผู้ป่วยวัยผู้ใหญ่ทั้งเพศชายและเพศหญิงที่เข้ารับการรักษาในโรงพยาบาล ศิครินทร์ หาดใหญ่ โดยกำหนดขนาดกลุ่มตัวอย่างด้วยโปรแกรม G*Power ได้กลุ่มตัวอย่าง จำนวน 50 คน แบ่งเป็นกลุ่มทดลองที่ได้รับโปรแกรมการให้ข้อมูลแบบรูปธรรม-ปรนัยและกลุ่มควบคุมที่ได้รับการพยาบาลตามปกติ กลุ่มละ 25 คน เครื่องมือที่ใช้ในการวิจัย คือ โปรแกรมการให้ข้อมูลแบบรูปธรรม-ปรนัย ที่ผู้วิจัยสร้างขึ้นตามแนวคิดทฤษฎีการปรับตนเองของ Leventhal & Johnson และแบบสอบถามพฤติกรรมการป้องกันการพลัดตกหกล้ม จำนวน 12 ข้อ ผ่านการตรวจสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิ จำนวน 3 คน มีค่าดัชนีความตรงตามเนื้อหาเท่ากับ 1 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และเปรียบเทียบความแตกต่างของคะแนนเฉลี่ยพฤติกรรมการป้องกันการพลัดตกหกล้มระหว่างกลุ่มด้วยสถิติ Independent t-test


ผลการวิจัย:พฤติกรรมการป้องกันการพลัดตกหกล้มกลุ่มที่ได้รับโปรแกรมการให้ข้อมูลแบบรูปธรรม-ปรนัยหลังได้รับโปรแกรมสูงกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติ (p < .001) และกลุ่มที่ได้รับโปรแกรมการให้ข้อมูลแบบรูปธรรม-ปรนัยมีคะแนนเฉลี่ยพฤติกรรมการป้องกันการพลัดตกหกล้ม (=7.32, SD=5.30) สูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ (=1.92, SD=4.39) อย่างมีนัยสำคัญทางสถิติที่ระดับ .001 (p < .001)


สรุปผล:โปรแกรมการให้ข้อมูลแบบรูปธรรม-ปรนัยส่งผลให้ผู้ป่วยวัยผู้ใหญ่มีพฤติกรรมการป้องกันการพลัดตกหกล้มดีขึ้น ทั้งในการเปรียบเทียบระหว่างกลุ่มและก่อน-หลังการทดลอง จึงควรนำไปใช้เพื่อลดความเสี่ยงการพลัดตกหกล้มในสถานพยาบาล

วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ป่วยที่อายุ 60 ปีขึ้นไป เตรียมเข้ารับการผ่าตัดต้อกระจกและใส่เลนส์แก้วตาเทียมที่ศูนย์ตา โงพยาบาลศิครินทร์ กรุงเทพ โดยคัดเลือกกลุ่มตัวอย่างแบบเจาะจงที่มีคุณสมบัติตามเกณฑ์คัดเข้าจำนวน 34 คน กลุ่มตัวอย่างได้รับโปรแกรมการให้ข้อมูลการดูแลผู้ป่วยต้อกระจกที่เข้ารับการผ่าตัดต้อกระจกและใส่เลนส์แก้วตาเทียมซึ่งเป็นโปรแกรมที่ผู้วิจัยสร้างขึ้น ผู้วิจัยดำเนินการเก็บรวบรวมข้อมูลโดยใช้แบบประเมินพฤติกรรมการดูแลตนเองของผู้ป่วยหลังได้รับโปรแกรม โดยดำเนินการเก็บข้อมูลเป็น 2 ระยะ คือ หลังผ่าตัด 7 วัน และ 1 เดือน และแบบประเมินความพึงพอใจของผู้ป่วยหลังได้รับโปรแกรมเมื่อหลังผ่าตัด 1 เดือน เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลทั้ง 2 ชุดมีค่าดัชนีความตรงตามเนื้อหา เท่ากับ 1.00 โดยมีค่าสัมประสิทธิ์อัลฟาของครอนบาค เท่ากับ .84 และ .80 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา (ค่าเฉลี่ย และ ร้อยละ) และการทดสอบค่าแบบกลุ่มตัวอย่างไม่เป็นอิสระต่อกัน (Dependent T-Test)


ผลการวิจัย:คะแนนเฉลี่ยพฤติกรรมการดูแลตนเองของผู้ป่วยผ่าตัดต้อกระจกภายหลังได้รับโปรแกรมการให้ข้อมูลการดูแลผู้ป่วยต้อกระจกที่เข้ารับการผ่าตัดต้อกระจกและใส่เลนส์แก้วตาเทียมเมื่อหลังผ่าตัด 1 เดือน มีความแตกต่างจากหลังผ่าตัด 7 วัน อย่างมีนัยสำคัญทางสถิติที่ระดับ (t = 4.61, p< .05) และคะแนนเฉลี่ยความพึงพอใจของผู้ป่วยต้อกระจกที่เข้ารับการผ่าตัดต้อกระจกและใส่เลนส์แก้วตาเทียมหลังได้รับโปรแกรมการให้ข้อมูลการดูแลอยู่ในระดับมากคิดเป็นร้อยละ 91.20


สรุป:โปรแกรมการให้ข้อมูลการดูแลผู้ป่วยต้อกระจกที่เข้ารับการผ่าตัดต้อกระจกและใส่เลนส์แก้วตาเทียม ส่งผลให้ผู้ป่วยมีพฤติกรรมการดูแลตนเองที่ดีขึ้นรวมถึงมีระดับความพึงพอใจอยู่ในระดับมากเมื่อภายหลังการผ่าตัด 1 เดือน

This dissertation examines how African immigrant employees make sense of organizational socialization in U.S. workplaces. As global mobility reshapes the labor force, nearly one in five U.S. workers is foreign-born, making organizational socialization an increasingly complex communicative process. Classic socialization models assume linear, uniform adjustment, yet international employees often face ambiguity, cultural mismatch, and racialized expectations that complicate early workplace experiences. Guided by sensemaking theory, this study conceptualizes socialization as an ongoing, nonlinear process through which newcomers notice cues, construct interpretations, act on provisional understandings, and revise meanings as contexts shift. Inherited retinal diseases (IRDs) affect millions of people worldwide. Majority of IRDs are caused by degeneration of rod and cone photoreceptor cells (PRCs) due to gene mutations. The overarching goal of my dissertation is to model and evaluate the molecular role of various gene candidates involved in IRDs such as cone rod dystrophy (CRD) and retinitis pigmentosa (RP).

This thesis examines the liturgical and musical heritage of Roman Catholic apostolic women religious in the United States and argues that their contributions represent a significant yet understudied dimension of American Catholic life. As many communities face declining numbers, the preservation of their liturgical and musical traditions becomes increasingly urgent. Using an interdisciplinary methodology that combines historical research, liturgical theology, pneumatology, phenomenology, narrative inquiry, and qualitative interviews, the study explores how women religious have shaped and preserved communal identity through liturgy, music, and apostolic ministry.

Objective: To evaluate the effect of a competency-based cross-training program on perinatal nurses’ preparedness to care for medical-surgical (med-surg) overflow patients and to support patient flow.

Design: Quality improvement project using a quasi-experimental pre–post design.

Setting: A 44-bed perinatal unit within a university-affiliated medical center in the United States.

Participants: Registered nurses employed in the perinatal unit for ≥6 months; 24 completed the preintervention survey and 28 completed the postintervention survey.

Background: Instrument tray defects contribute to surgical delays, workflow disruptions, rework in sterile processing, inefficient instrument utilization, and patient safety risks.

Purpose: The purpose of this quality improvement project was to reduce orthopedic instrument tray defects through targeted education and standardized tray audits with double-check verification in a large academic medical center.

Methods: A quasi-experimental pre-post design was utilized to evaluate the impact of the intervention. Tray errors were captured using a standardized binary indicator. Staff satisfaction and confidence surveys were measured using Likert-scale surveys.

Background: Burnout among nurses and nursing students is a widespread and well‑documented problem that adversely affects professional performance and psychological well‑being. Resilience serves as a key protective factor, helping buffer the effects of academic stress and reducing burnout over time. Incorporating brief, low‑stakes cognitive challenges—such as a riddle—can help foster resilience.
