Introduction: Titin (TTN) mutations are a well-known cause of dilated cardiomyopathy (DCM), occurring in approximately 25% of familial cases of idiopathic DCM and 18% of sporadic cases. Recognition and referral for genetic testing remains underutilized but has important prognostication and helps the patient better understand the disease transmission. This case report stresses the importance of a complete evaluation for genetic causes of DCM.

Background: Cor triatriatum (CTS) is a rare congenital heart defect in which a fibromuscular membrane divides the atrium into two chambers, disrupting normal blood flow and potentially leading to circulatory impairment. Accounting for 0.1% to 0.4% of congenital heart diseases, CTS presents variably depending on the degree of left atrial obstruction. Patients may report exertional dyspnea, orthopnea, or palpitations; severe cases can progress to heart failure, pulmonary hypertension, or thromboembolic events. We present the case of a young adult male who developed symptomatic heart failure and atrial fibrillation and was incidentally found to have CTS on imaging.

Background: Cardiovascular diseases (CVD) account for nearly one-third of global mortality, with ischemic heart disease caused by a mismatch between myocardial oxygen supply and demand. Despite advances in percutaneous coronary intervention (PCI) and contemporary pharmacotherapy, patients with non–ST-segment elevation myocardial infarction (NSTEMI) continue to experience substantial residual risk for major adverse cardiovascular events (MACE). BACKGROUND Acute pancreatitis (AP) has become one of the leading gastrointestinal diagnoses resulting in inpatient hospitalization¹. AP is diagnosed by meeting two of three criteria: characteristic clinical presentation, biochemical evidence (elevated lipase or amylase levels), and imaging findings with pancreatic inflammation. Gallstones and heavy alcohol use account for most cases (40%–70% and 25%–35%, respectively). Hypertriglyceride induced AP (HTG-AP) accounts for only about 7-10% of AP cases³ and often tends to present in younger, male, obese, patients with a higher rate of diabetes¹. Background: The effect of biologic immunosuppressant therapy on postoperative outcomes following ankle fracture open reduction and internal fixation (ORIF) remains unclear, and concerns regarding infection and impaired healing often influence perioperative decision-making. We hypothesized that biologic immunosuppressant use would be associated with higher rates of postoperative complications following ORIF of closed ankle fractures compared with non-biologic immunosuppressant therapy.

Methods: We conducted a retrospective cohort study using a multi-hospital administrative dataset (2017-2025) to identify adult patients undergoing ORIF for closed bimalleolar or trimalleolar ankle fractures who were prescribed immunosuppressive therapy. Introduction Intraoperative and postoperative blood loss is a major concern and challenge for surgeons in total joint arthroplasty (TJA). Patients with large blood loss can develop postoperative anemia and associated complications. Novel hemostatic agents, such as oxidized regenerated cellulose (ORC) have grown in popularity as intraoperative hemostatic adjuncts. Introduction: Hypertrophic obstructive cardiomyopathy (HOCM) and anomalous origin of the right coronary artery (RCA) are well-known but rare congenital heart diseases that are recognized as independent leading causes of sudden cardiac death(SCD). The independent prevalence of both these conditions is less than 1%. The co-occurrence of these two conditions is seldom ever reported in the literature. We present an extremely rare case of HOCM and the anomalous origin of the RCA in the same individual.

The Staggers Rail Act of 1980 requires the Surface Transportation Board ("STB ") to be proactive in making sure freight railroads ("carriers") earn adequate revenues. Forty-five years later there is still debate on whether carriers are earning adequate revenues, how adequate revenues are to be defined and calculated, whether the Act allows the STB to put an upper bound on a carrier's revenues when they are determined to be earning adequate revenues, and how revenue adequacy impacts rate setting in market dominant situations.

An Examination Of Remittance Inflow Variation In Net-Emigration Countries, Andrew Rizko

This paper examines the structural and institutional determinants of remittance inflows in a panel of 40 net-emigration countries from 2000 to 2024. By restricting the sample to countries with negative average net migration, the analysis holds migration pressure constant and isolates the role of domestic conditions in explaining cross-country variation in remittance dependence. Using fixed effects and correlated random effects (Mundlak) estimators, I find that labor force participation is the most robust within-country determinant: a one percentage point increase is associated with approximately a 2.3 percent decline in remittance inflows, consistent with remittances functioning as a structural need in contexts …

This thesis investigates the relationship between architecture, human perception, and well-being through the lens of human-centered and biophilic design. Drawing from philosophical traditions, phenomenology, and contemporary neuroscience, this thesis argues that the built environment is an active agent in shaping cognition, emotion, and lived experience. Architecture is understood as multisensory, embodied, and temporally experienced, influencing individuals in both conscious and nonconscious ways. [Patient presentation] A 68-year-old man presented via emergency medical services (EMS) with right hip pain following a traumatic fall from his bicycle. On arrival at the emergency department, he was alert and oriented. Physical examination revealed only tenderness over the greater trochanter of the left femur, with no significant pain on passive range of motion of the left hip. Electric vehicles (EVs) and rooftop solar photovoltaic (PV) systems are increasingly being integrated into residential settings, creating new opportunities for vehicle-to-grid (V2G) and vehicle-to-home (V2H) operations. In these systems, the EV battery functions as a controllable energy storage unit that can charge from the grid or PV and discharge energy to supply household load or export to the grid for a profit. By intelligently scheduling this bidirectional power exchange, households can reduce electricity costs and enhance PV utilization. The integration and modeling of high-dimensional, heterogeneous biological data remain central challenges in computational biology due to complex feature dependencies and pervasive missingness. This dissertation addresses these challenges by developing novel generative frameworks for multimodal data reconstruction, imputation, and interaction prediction. In generative modeling, we focus on capturing structural and causal dependencies in sparse biological systems. We first introduce CausalGeD, a causality-aware diffusion framework that leverages Granger-causal attention for biologically coherent spatial gene expression generation. Deucravacitinib is an oral, selective, allosteric tyrosine kinase 2 (TYK2) inhibitor approved for the treatment of moderate-to-severe plaque psoriasis and psoriatic arthritis. Deucravacitinib may offer a more selective approach compared to traditional Janus kinase (JAK) inhibitors. Emerging data support the use of deucravacitinib in inflammatory dermatoses beyond psoriatic disease. We present our clinical experience using deucravacitinib off-label in patients with challenging inflammatory skin disease and contextualize our findings within the current literature. This study highlights the green synthesis of CuO nanoparticles using coconut coir extract and evaluates their photocatalytic performance in the degradation of the Brown G dye. The CuO nano-particles were synthesised through sol-gel method, and the biomolecules like polyphenols, tannins and flavonoids acted as reducing agents to stabilise the nanoparticles. The characterisation of these nanoparticles was carried out through XRD, FTIR and FESEM methods. This study investigates the influence of transition metal (Fe, Co, Mn) doping on the surface properties and in-vitro bioactivity of TiO2 nanowires. It aims to elucidate how transition-metal doping alters the surface behavior and biological response of TiO2 nanowires, enabling their potential use in biocompatible and magnetically responsive materials. Magnetic TiO2 nanowires doped with transition metals (Mx+/TiO2) were successfully prepared by a hydrothermal method using titanium dioxide in alkaline solution. Ageism has befuddled the law’s attempts at remediation. Each “ism” (e.g., sexism, racism) differs from the others. Ageism, however, differs fundamentally from other “isms.” That difference explains the law’s failure to limit it. This article describes the difference, and it explains why that difference has rendered ageism resistant to legal remedies. Legal scholars and gerontologists have asked why the law’s efforts to limit the bias, stigma and discrimination that stem from ageism have largely failed. A remarkable framework for responding to that query derive from literary and artistic depictions of old people over time.

Employers may now use analytics powered by artificial intelligence (“AI”) to identify which workers are most likely to develop future disabilities, raising the threat that these workers will face limited opportunities and discrimination before such action is clearly illegal.

Title VI of the U.S. Civil Rights Act prohibits institutions receiving federal financial assistance from discriminating based on race, color, or national origin. The term “national origin” includes people with limited English proficiency (“LEP”). The number of LEP residents has increased in the United States to over 10% of the residents in the most populous states (Texas, California, New York, Florida, and Nevada). Siting energy infrastructure has long been contentious, but growing local opposition to new energy projects in the U.S. – notably, wind and solar facilities – has drawn nationwide headlines. The legal context for energy siting varies across U.S. states, with local control being a dominant model. Against this backdrop, several states have adopted legislation to facilitate renewable energy siting by adjusting decisional authority between the local and state level.

A Terrorist In Death Only: How The Death Penalty Fills The Gap In Federal Terrorism Law, Caroline Alden Mcbride

In the absence of a federal statute criminalizing domestic terrorism, the United States turned to an unlikely proxy: the death penalty. This Comment argues that capital punishment is used to define domestic terrorism, particularly when statutory tools cannot capture white supremacist attacks. Although former President Biden commuted most federal death sentences to life imprisonment without parole, three individuals whose attacks met the legal definition of domestic terrorism were excluded from clemency. Examining several cases from Timothy McVeigh to Luigi Mangione reveals a trend of defendants whose crimes meet the legal definition of domestic terrorism receiving the death penalty, even when …

In the United States, white, non-Hispanic individuals possess approximately seven times the wealth of Black individuals. One recent study found that for every 100 dollars in wealth held by a white household, a Black household holds about fifteen dollars. This disparity in wealth accumulation is referred to as the wealth gap, and it has historically stratified along racial lines.
